Type of visit: Required - 1 Year

On 5/23/2026 at approximately 01:20 PM, Licensing Program Analyst (LPA), Angelica Segovia conducted an annual visit to the facility. LPA was greeted by the facility’s representative, David Slattery who was designated to sign today’s report per the Administrator, Jennifer Fogelstrom. LPA conducted a physical plant tour at approximately 1:30 PM and the following was noted: The facility is a single-story building with four (4) bedrooms and two (2) bathrooms. The facility is currently non-operational and occupying zero (0) residents. There is no designated staff room. The facility has an approved fire clearance for six (6) non-ambulatory residents of which one (1) may be bedridden. Hospice waiver approved for six (6). Common areas: The living room and dining room were observed. The rooms were observed to be properly furnished and in good repair. Required postings such as Long-Term Care Ombudsman, Emergency Disaster Plan, and Personal Rights will be posted before residents’ arrivals. LPA observed the fireplace to be covered. Kitchen: Sufficient supplies of seven (7) day nonperishable food and two (2) day perishable foods will be purchased prior to residents’ arrival. Kitchen appliances were observed to be in proper condition. (continued LIC 809-C) Bedrooms: Maintenance of residents’ bedroom will need to be updated to regulations prior to residents’ arrival. Bathrooms: Maintenance of the facility’s restrooms will need to be updated to regulations prior to residents arrival. LPA observed appropriate grab rails to be in proper condition. Backyard: LPA observed there to be a locked shed. LPA observed there to be a pool. LPA observed the pool to be fenced. Garage: The garage can be accessed from inside of the facility. The garage was observed to be kept locked and used for storage purposes. Laundry Room: The laundry appliances were observed to located within the garage. The laundry appliances were observed to be in proper condition. Smoke detectors and carbon monoxide observed to be working properly and were tested. There were no immediate health and safety hazards observed during the day of inspection. Type of visit: Prelicensing

At 9:00 AM Licensing Program Analysts (LPA), Leslie Ngo-Castaneda arrived at the facility to conduct an announced pre-licensing visit to the above facility LPA met with administrator at 9:30 AM. Fire Clearance was approved on 1.27.2025 for a maximum capacity of six (6) non-ambulatory residents; one (1) bedridden; hospice waiver for six (6). The purpose of today’s visit is to inspect the facility to ensure that the facility is in compliance with rules and regulations under California Code of Regulations, Title 22, Division 6. The facility is a single-story building. Today's site visit consisted of team touring the physical plant inside and outside and observed the following: KITCHEN: The kitchen is equipped with a refrigerator, microwave oven and sink. At 9:38 AM, LPA observed adequate supplies of perishable and nonperishable food and dining ware to accommodate a maximum capacity of six (6). All knives and sharps are observed to be locked in a kitchen cabinet and inaccessible to residents. FIRE EXTINGUISHER was last purchased on 12/18/2024 is located in the kitchen. First-aid is complete and new. BEDROOMS: There are four (4) bedrooms designated for client use. All bedrooms are furnished with beds, dresser and required bedding and linen. Extra linen is available in the facility cabinet by the dining table. The bedrooms have sufficient closet space and have sufficient lighting. Facility will have awake staff. Bedroom #1 and bedroom #3 are shared and is located by the hallway. Bedroom #2 and bedroom #4 are for private use. Bedroom #2 is located at the hallway and bedroom #4 is located beside the living room. Continue to LIC 809-C BATHROOMS: LPA observed two (2) bathrooms are clean and in good repair. Properly supplied with toilet papers, soap, and paper towels. The hot water temperature measured between 109.1°F. Bathroom #1 is located in the hallway across bedroom #1. Bathroom #2 is located inside bedroom #1 of the facility. Hot water for bathroom #2 measured at 110.4°F. COMMON AREAS: The facility maintains a comfortable temperature at 72°F. The living room and dining appeared clean and were properly furnished. No obstructions and or tripping hazards throughout the facility. Office is located, by the living room. MEDICATION: The medications will be kept in a cabinet in the living room and LPA observed the cabinet kept locked and inaccessible to residents in care. SMOKE DETECTORS/CARBON MONOXIDE. Smoke detectors and carbon monoxide were located throughout the facility. At 10:30 AM they were tested and observed to be operational. SURROUNDING GROUNDS: In the back of the facility has sufficient yard space. LPA observed appropriate outdoor furniture, with a covered shaded area for clients. The backyard is fenced. LPA discussed the importance of maintaining the care and supervision to meet the needs of clients. There is a swimming pool or bodies of water in the backyard, this is lock and fenced. The driveway, passageway and entrance to the home was clear of obstructions. GARAGE – is used as a storage. LPA observed laundry room is located in the locked garage. The washer/dryer appear to be in good condition. Laundry supplies are kept lock and inaccessible when not in use with supervision. Component III was conducted with the administrator and licensee. Facility is in compliance with Title 22 Regulations at this time.
