Licensing Program Analyst (LPA) Tihesha Smith visited this facility to conduct the past due annual inspection that was originally due in April 2026. However, the past due inspection had already been completed on 08/21/26, the day prior to this visit. This brief report is being created to document this visit. No hazards observed at time of visit. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Required-1 year visit. LPA was met by caregivers, Kristina Grigoryan and Evelina Maghakyan and explained the purpose of the visit. Shortly after, the administrator, Arusyak Ohanyan and Lilit Gasparyan, manager arrived and assisted LPA. The facility is licensed to serve (6) non-ambulatory residents ages 60 and above, of which (1) may be bedridden (bedrooms 3 & 4 cleared for 1 bedridden), hospice waiver for (5). LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were maintained. Infection control plan was last reviewed/updated on 03/04/2025. Staff are trained in the proper use of required PPEs. Operational Requirements: Facility is equipped with operable signaling systems at all exit points. Liability insurance is valid, expires on 05/11/2027. The facility has a dementia care plan to accept or retain residents with dementia. There is (1) fire extinguisher located in the kitchen, purchased on 05/07/2026. Physical Plant/Environment Safety: The facility is a single story home located in a residential neighborhood that consists of (5) resident bedrooms, office, (3) bathrooms, living room, kitchen, dining area, laundry area, backyard with a covered patio and equipped with outdoor furniture. The interior and exterior physical plant was inspected. Resident bedrooms were toured. Each bedroom has a smoke detector, linen, light, chair and sufficient closet space. There are cameras in the common areas and the backyard. Knives, cleaning solutions, and disinfectants are locked. There are no firearms or weapons stored at the facility. Water temperature reading measured at 107.1 deg F in bathroom #1 and 105.3 deg F in bathroom #2 which are within the required 105 - 120 degrees Fahrenheit. Licensee posted the personal rights, non discrimination notice and complaint information near the entrance.*****REPORT CONTINUED ON LIC809-C**** Staffing: A total of (8) staff members including the administrator provide care and supervision to the residents. Staff employed are over the age of 18 and have criminal background clearance, fingerprint cleared, have the required training and associated to the facility. Personnel Records/Staff Training: Administrator's proof of health clearance, fingerprint clearance, vaccinations and 1st Aid/CPR training are current. Administrator's certificate is valid and expires on 09/18/2026. Resident Rights-Information: Rights of individuals with disabilities are posted. Facility provides internet services to all residents and have access to the facility phone. Planned Activities: Facility has sufficient space to accommodate indoor and outdoor activities. Food Service: The kitchen was inspected and has sufficient supply of 2-day perishable & 7 day non-perishable food. The food is properly stored in the refrigerator. Pesticides and cleaning supplies are kept away from the food preparation areas. Incidental Medical Services: Residents' medications were reviewed. Residents medication are centrally stored in a locked cabinet and inaccessible to residents. Resident Records-Incident Reports: All (6) resident files were reviewed containing admission agreements, Physician's Report, Medical assessments, Needs and Services Plans, Personal rights, Medical Consent and Hospice Notes/Records. Disaster Preparedness: The facility has a complete Emergency Disaster and Mass Casualty Plan. The facility conducts emergency drill on a quarterly basis. Last emergency (earthquake) drill was conducted on 07/03/2026. The emergency & disaster plan was last updated on 06/12/2022. Residents with SHN: One (1) resident is under hospice care and (1) resident is bedridden. Appraisals were observed in resident files. No deficiency cited, Technical advisories were issued. Type of visit: Required - 1 Year

On 06/16/25 Licensing Program Analyst (LPA) Lorena Casillas met with Licensee Arusyak Ohanyan for a Required One (1) Year visit. LPA was greeted and granted access by Licensee. LPA explained the reason for the visit, an entrance interview was conducted. A tour of the physical plant was conducted at 12:45 PM with the Licensee. The facility has a total of five (5) bedrooms and three (3) bathrooms. The facility is fire cleared for six (06) non-ambulatory. The facility is currently occupied by five (05) non-ambulatory residents. Infection Control: LPA reviewed infection control plan approved on 06/01/2022. LPA also reviewed emergency plan to make sure it is updated yearly. Kitchen: LPA toured the kitchen and observed that it is sufficiently stocked with two (2) days perishable and seven (7) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. Common Areas: The living and dining room are neat and clean. The facility maintains a comfortable temperature at 75°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located in the kitchen, observed to be fully charged and last purchased 05/09/2025. (continued on LIC 809-C) Resident Rooms: LPA toured resident rooms and observed that they are adequately furnished with appropriate lights, bedding, dressers and closet space. Residents have enough personal hygiene product provided by the licensee. Bathrooms: LPA observed the bathrooms, and they were observed to be clean and properly stocked. The hot water temperature was measured at 127.2°F, however high hot water temperature signs are posted in all areas with a sink or access to water per regulation. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. Laundry room: Laundry room is adjacent to the kitchen and laundry detergents, cleaning agents and other toxins were observed to be locked away in a cabinet. Garage: Garage is not included in the lease therefore Licensee has no access it. Surrounding grounds: The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. Staff and Resident Files: At 12:00 pm LPA reviewed staff and resident files. Administrative: LPA collected LIC500, Resident Roster, Liability Insurance and Administrator Certificate. Annual fees are current. Interviews: At 1:30 pm LPA interviewed staff and residents. Medication: LPA observed medication and first aid kit to be locked and inaccessible to residents. LPA reviewed medication and logs with Administrator. No citations issued. Exit interview conducted. Licensing Program Analyst (LPA) Abeye Duguma met with Arusyak Ohanyan for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at 12:45 PM and the following was noted: There is one entrance being utilized at the facility. The facility has a total of five (05) bedrooms and three (03) bathrooms. The facility is fire cleared for six (06) non-ambulatory. The facility is currently occupying five (05) non-ambulatory residents. The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. Laundry detergents, cleaning agents and other toxins are locked away. Kitchen is sufficiently stocked with at least two (02) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. The living and dining room are neat and clean. The facility maintains a comfortable temperature at 70°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located in the kitchen, observed to be fully charged and last purchased 05/18/2023. (continued on LIC 809-C) The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. Residents have enough personal hygiene product provided by the licensee. The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 114.9°F. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. LPA observed medication and first aid kit to be locked and inaccessible to residents.
