Type of visit: Required - 1 Year

On 09/16/2025 at 9:15 AM, Licensing Program Analyst (LPA) Gina Saucedo and conducted an unannounced, Annual Inspection and met with Caregiver, Kumushai Mizamova. Ruzanna Manukyan, Designee Administrator was called and arrived about twenty (20) minutes after and was advised of visit. LPA asked for the census, staff and resident files. The physical plant was toured inside and out at 9:50am. Living/Dining Room Area: LPAs observed the living room furniture to be clean and in good repair. The facility maintains a comfortable temperature at 74 degrees Fahrenheit with a large television. Bedrooms: There are six (6) bedrooms. All six (6) bedrooms are used for residents. There are three (3) bedrooms on the right-hand side and three (3) bedrooms on your left-hand. LPAs observed rooms to have bedding sheets, pillowcase, blankets, nightstands, televisions, and sufficient lighting for each of the resident’s room. There are two (2) hallways, the hallway on your right-hand side has two (2) closets, one (1) with extra linen and hygiene. The other hallway has two (2) closets filled with linen and extra hygiene. Bathrooms: There are six (6) bathrooms that were toured and checked to make sure bathrooms were clean and in good repair. The hot water temperatures were measured within regulations of 103.1 and 113.9 degrees. The showers have non-slip bathmats and grab bars. There are three (3) bathrooms on your right-hand side and three (3) bathrooms on your left-hand side. LIC809C-continued Medications: There is a black and grey cabinet on your right-hand side of the facility that contains the medication, first aid and files. There is also a signal system in the facility. There are two (2) fire extinguishers fully charged and dated March-2025. One (1) is located by the kitchen and the other one (1) is located at the entrance of the facility. Kitchen Area: LPA inspected the kitchen area. There is one (1) refrigerator which was clean and in good operation. LPA observed sufficient supply of seven (7) day non-perishable and perishable foods in the cabinets. There is one (1) a washer and dryer enclosed inaccessible to the residents on the right hand side of the kitchen. The knives/sharps and chemicals are kept next to the kitchen area on your right-hand side, inaccessible to the residents. The chemicals are located under the sink locked and inaccessible to the residents. Outside: LPA toured the outside area. LPA observed a covered shaded area for residents and appropriate outdoor furniture. The facility has no body of water on the premises. There is one (1) gate that is unlocked leading to the outside area towards the street. The carbon monoxide and the smoke detector were tested, and they were operable, interconnected. There is no garage. Administration: The Liability Insurance was reviewed and will be renewed on 01/03/2026. There are several Covid 19 signs on the wall, hygiene sanitation signs, Infection Control, House Rules, Personal Rights, Ombudsman, Insurance and Disaster Plans on the wall at the entrance of the facility, Earthquake and Fire Drills were conducted and are located in a separate binder. Disaster Drill July 18, 2025 and Fire Drill July 08, 2025. Type of visit: Required - 1 Year

On 09/18/2024 at 9:05 AM, Licensing Program Analysts (LPAs) Gina Saucedo and Angelica Segovia conducted an unannounced, Annual Inspection and met with Caregiver, Kumushai Mizamova. LPA asked for the census, staff and resident files. The physical plant was toured inside and out at 10:50 am. Living/Dining Room Area: LPAs observed the living room furniture to be clean and in good repair. The facility maintains a comfortable temperature at 77 degrees Fahrenheit with a large television. Bedrooms: There are six (6) bedrooms. All six (6) bedrooms are used for residents. There are three (3) bedrooms on the right-hand side and three (3) bedrooms on your left-hand. LPAs observed rooms to have bedding sheets, pillowcase, blankets, nightstands, televisions, and sufficient lighting for each of the resident’s room. There are two (2) hallways, the hallway on your right-hand side has two (2) closets, one (1) with extra linen and hygiene and one (1) with a washer and dryer enclosed inaccessible to the residents. The other hallway has two (2) closets filled with linen and extra hygiene. Medications: There is a black and grey cabinet on your right-hand side of the facility that contains the medication, first aid and files. There is also a signal system in the facility. There are two (2) fire extinguishers fully charged and expires on April-2025. One (1) is located by the kitchen and the other one (1) is located at the entrance of the facility. The showers have non-slip bathmats and grab bars. There are three (3) bathrooms on your right-hand side and three (3) bathrooms on your left-hand side. Type of visit: Prelicensing

Licensing Program Analysts (LPAs) Gary Tan and Gina Saucedo conducted an announced Pre-Licensing visit to this facility and met with Licensee representative Lusine Meliksetyan. The applicant is "Sunrise AT Lindley II". Fire Clearance dated 06/01/23 was received for six (6) residents-five (5) non-ambulatory and one (1) bedridden in bedroom #6. Purpose of today’s visit is to inspect the facility to ensure that the facility is in compliance with the rules and regulations of California Code of Regulations, Title 22, Division 6. Facility is a single story home. Today's site visit consisted of LPA's touring the physical plant at around 10:00 AM inside and outside and observed the following: The facility smoke alarm system is hard wired and interconnected and have a sprinkling system. The fire extinguishers are located near the main entrance door and dining area and were observed to be fully charged and last bought on 09/11/2023. The facility uses a dual Carbon Monoxide/Smoke alarm detectors all over the common areas of the facility. Alarms were tested and observed to be operational. Hot water was tested in the common bathrooms and measured between 114.4F and 116.6 F. There is a functioning telephone on the premises which was tested. An emergency exit plan/sketch is posted on the hallway wall with other posting requirements. There are six (6) resident bedrooms, room #1 and room #6 have their, own private bathrooms. There is a total of six (6) bathrooms. All resident bedrooms were observed to be appropriately furnished. The common areas (living room, kitchen, and dining areas) were appropriately furnished and lighting was adequate. The living room has a comfortable furniture and dining room table. Residents, staff records and medications will be stored in a designated secured cabinet in the dining area. The first aid kit is readily available. The bathrooms have appropriate grab bars installed and non-skid mats. (continued to LIC 809-C) (continued from LIC 809) The kitchen knives and sharps are stored in a locked drawer in the kitchen using a magnetic lock. Kitchen cleaning supplies are stored in a locked cabinet below in the kitchen. Laundry detergents, cleaning supplies and other toxins are stored in a locked cabinet below the kitchen sink. The laundry area is located in the hallway near the kitchen. The necessary precautions have been made to the facility to safely house dementia residents such as auditory alarms on all doors. Auditory alarms were tested and observed to be operational. Facility appears to be clean and in good repair. Appliances in the kitchen appeared to be functional. There is a screening station immediately upon entrance. Sign in sheet, hand sanitizer, gloves and masks are available. Required covid 19 posters are posted all over the facility. The facility had submitted a Mitigation and Infection plan. There is a sitting area in the back yard for residents to conduct outdoor activities. The backyard is fenced. There is no body of water in the facility. Component III was waived as approved by LPM Troy Agard as the licensee has one (1) other existing facility. Facility is in compliance with Title 22 Regulations at this time. This report will be forwarded to the Centralized Application Bureau (CAB).
