Allegation investigated: Staff does not ensure meals are nutritionally balanced for residents in care Staff served undercook meat to residents in care
At 10:00am, Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced visit in response to the above-mentioned allegations. LPA met with the Administrator and explained the reason for the visit. At 10:05am, LPA requested resident and staff roster. At 10:10am, requested copies of pertinent information which include, but not limited to Admission Agreement, Physician's Report, Appraisal Needs and Services Plan, Menu, etc. relevant to the investigation. At approximately 10:20am, LPA conducted a physical plant tour, to ensure health and safety of the residents are protected. Between 10:30am - 11:30am, LPA conducted an interview with the Administrator, one (1) staff and two (2) out of three (3) residents who were able to communicate. Continue on LIC9099-C Unsubstantiated Allegation: Staff does not ensure meals are nutritionally balanced for residents in care It was alleged that staff do not ensure meals are nutritionally balanced for residents in care. To investigate this allegation, LPA conducted an interview with the Administrator and was informed that all meals are prepared according to the facility’s posted menu and are designed to meet residents’ nutritional needs. The Administrator reported that staff follow the planned menu and ensure meals contain appropriate portions of protein, vegetables, fruits, and grains. Interview with one (1) staff member revealed that meals are prepared based on the facility menu and dietary guidelines, if any. Staff reported that residents are offered balanced meals daily and substitutions are made only when medically necessary or based on resident preference. Staff denied serving meals that were not nutritionally balanced. Two (2) out of three (3) residents interviewed expressed no concerns regarding this allegation. During the visit, LPA observed the lunch meal prepared for residents. The meal included a protein item, a cooked vegetable, fruit, and a grain/starch component. The meal appeared well-portioned and consistent with a nutritionally balanced menu. Based on interviews and direct observation, there was insufficient evidence to support the allegation that staff do not ensure meals are nutritionally balanced for residents. Therefore, the allegation is Unsubstantiated at this time. Allegation: Staff served undercook meat to residents in care The investigate this allegation, LPA conducted an interview with the Administrator who denied the allegation and stated that all meat served at the facility is fully cooked and prepared according to safe food-handling practices. The Administrator stated that staff are trained to check food temperatures and ensure meat is fully cooked. One (1) staff interviewed confirmed this and added that any residents’ concerns about meals would be addressed immediately with a replacement or alternative. Two (2) of the three (3) residents interviewed denied ever receiving undercooked meat and reported that meals are generally well prepared. No residents expressed concerns regarding this allegation. During the visit, LPA observed the lunch meal prepared for residents, which included a fully cooked protein entree. The meat appeared properly cooked, with appropriate texture and temperature. No undercooked food items were observed. Based on interviews and direct observation, there was insufficient evidence to support the allegation that staff served undercooked meat to residents. Therefore, the allegation is Unsubstantiated at this time. No deficiency issued during today's visit. Type of visit: Required - 1 Year
On 12/08/25 at 09:35 AM, Licensing Program Analyst (LPA) Gina Saucedo arrived to conduct an unannounced, annual inspection at the facility. Upon arrival, LPA met with caregiver Larisa Bobakova and disclosed the purpose of the visit. LPA asked for the census, resident, and staff rosters. A physical tour was conducted at 10:30 AM and observed the following: The Kitchen area was toured, and LPA observed there to be sufficient seven (7) day supply of non-perishable foods and perishable food for all residents. The kitchen area was clean at the time of the tour. There are two (2) fire extinguishers. One (1) is located against the wall on your left-hand side of the facility. It is fully charged with an expiration date of 03/2026. There is extra, food in the kitchen pantries. The first aid kit is on the kitchen counter area on the left-side of the sink. The knives are at the top of the kitchen counter on your right-side in a locked black box and inaccessible to the residents. The medications are locked and inaccessible to the residents in one of the top, black cabinets on your left-hand side at the entrance of the facility. Outside/Backyard: The outside/backyard has furniture for the residents with proper seating. The facility does have a signal system. There are cameras in common areas. The facility does not have a pool/body of water. LIC 809C-continued Bedrooms: There are six (6) bedrooms and four (4) full bathrooms. There is two (2) half bathrooms. Two (2) of the resident’s rooms have private bathrooms. Only five (5) resident rooms occupied. One (1) room is currently vacant. All bedrooms and bathrooms were toured and were properly furnished and have appropriate bedding, linens, toiletry, and lightning. The bathrooms have proper toiletry, grab bars and non-skid mats. The bathroom temperatures of the water are within regulations reading at 112-114 degree Fahrenheit. The dining/living room area has enough seating for the residents and the staff. There Is another fire extinguisher on your right-hand side fully charged. There is no fireplace. There is a pantry/room in the hallway that has extra linen and food storage. It is locked and inaccessible to the residents. The house temperature is at 75-degree Fahrenheit. The smoke detector/carbon monoxide is in the hallway and is operable. The washer/dryer along with the chemicals is in the hallway in a locked room, inaccessible to the residents. There are three pantry rooms with extra food, linen, incontinence, and chemicals that are also locked and inaccessible to the residents and other storage items. Administrative: In the dining room/living room are on your right-hand side there is a billboard with House Rules, Disaster Plan, Resident Rights, Theft and Loss Policy, Rights of Resident Council, Infection Control and the surety bond with an expiration date of 01/2026. Allegation investigated: Licensee did not post ombudsman program information as required by law
On 11/12/24, at 12:45pm, Licensing Program Analysts (LPAs) Gina Saucedo and Angelica Segovia arrived at the facility to conduct an unannounced, initial complaint visit and was greeted by Lead Caregiver Larisa Bobakova. LPA disclosed the purpose of the visit. LPA explained the purpose of this visit was to gather information, conduct staff and resident interviews and deliver findings for this complaint. The investigation consisted of the following: LPAs asked for the census, requested the staff and resident roster. At 12:50pm, LPA toured the physical plant. During the tour, residents and staff were interviewed. 9099C-continued Unsubstantiated Regarding the allegation: Licensee did not post ombudsman program information as required by law. It is being alleged that the LTCOP (long term care ombudsman program) poster was given to the above facility, and it was not posted. At the time of the LPA’s visit, the LTCOP (long term care ombudsman program) poster was posted on the left side of the entrance of the facility against the wall. LPA took pictures of the LTCOP (long term care ombudsman program) poster. LPA interviewed three (3) staff and confirmed that it was given to them in September, they signed a paper that it was given to them, and they posted it immediately. LPA interviewed three (3) residents that confirmed the poster has been posted against the wall for a while now. Therefore, based on the LPA's record reviews, staff and resident interviews the above allegation(s) above is UNSUBSTANTIATED at this time. On 11/12/24 at 09:50 AM, Licensing Program Analysts (LPAs) Gina Saucedo and Angelica Segovia, arrived to conduct an unannounced, annual inspection at the facility. Upon arrival, LPAs met with caregiver Larisa Bobakova and disclosed the purpose of the visit. LPA asked for the census, resident, and staff rosters. A physical tour was conducted at 11:00 AM and observed the following: The Kitchen area was toured, and LPAs observed there to be sufficient seven (7) day supply of non-perishable foods and perishable food for all residents. The kitchen area was clean at the time of the tour. The fire extinguisher is located against the wall on your left-hand side. It is fully charged. The expiration date is 04/2025. There is a telephone line on the counter in the kitchen. There is extra, food in the kitchen pantries. The first aid kit is on the kitchen counter area on the left-side of the sink. The knives are at the top of the kitchen counter on your right-side in a locked black box and inaccessible to the residents. The medications are locked and inaccessible to the residents in one of the top, black cabinets on your left-hand side at the entrance of the facility. Outside/Backyard: The outside/backyard has furniture for the residents with proper seating. The facility does have a signal system. The facility does not have a pool/body of water. It is locked and inaccessible to the residents. The house temperature is at 71-degree Fahrenheit. The smoke detector/carbon monoxide is in the hallway and is operable. The washer/dryer along with the chemicals is in the hallway in a locked room, inaccessible to the residents. There are three pantry rooms with extra food, linen, incontinence, and chemicals that are also locked and inaccessible to the residents. Administrative: There is no annual fee that is due right now. In the dining room/living room are on your right-hand side there is a billboard with House Rules, Disaster Plan, Resident Rights, Theft and Loss Policy, Rights of Resident Council, Licensee Expiration dated 01/2025, Infection Control and the surety bond with an expiration date of 01/2025. On 03/06/24 at 10:58 AM, Licensing Program Analyst (LPA) Gina Saucedo, arrived to conduct an unannounced, annual inspection at the facility. Upon arrival, LPA Saucedo met with administrator Armenuhi Avetisyan and disclosed the purpose of the visit. LPA asked for the census, resident, and staff rosters. A physical tour was conducted at 11:25 AM and observed the following: The Kitchen area was toured, and LPA observed there to be sufficient seven (7) day supply of non-perishable foods and perishable food for all residents. The kitchen area was clean at the time of the tour. The fire extinguisher is located against the wall on your left-hand side. It is fully charged. The expiration date is 07/2024. There is a telephone line on the counter in the kitchen. There is extra, food in the kitchen pantries. The first aid kit is on the kitchen counter area on the left-side of the sink. The knives are at the top of the kitchen counter on your right-side in a locked black box and inaccessible to the residents. The medications are locked and inaccessible to the residents in one of the top, black cabinets on your left-hand side at the entrance of the facility. Outside/Backyard: The outside/backyard has furniture for the residents with proper seating. The facility does have a signal system. The facility does not have a pool/body of water. All six (6) resident rooms are singe, occupied. There is no staff room. The bathroom temperatures of the water are within regulations reading at 117–118-degree Fahrenheit. The dining/living room area has enough seating for the residents and the staff. There Is another fire extinguisher on your right-hand side fully charged and dated 07/2024. There is no fireplace. There is a pantry/room in the hallway that has extra linen and food storage. It is locked and inaccessible to the residents. The house temperature is at 74-degree Fahrenheit. The smoke detector/carbon monoxide is in the hallway and is operable. The washer/dryer along with the chemicals is in the hallway in a locked room, inaccessible to the residents. There are three pantry rooms with extra food, linen, incontinence, and chemicals that are also locked and inaccessible to the residents. Administrative: There is no annual fee that is due right now. In the dining room/living room are on your right-hand side there is a billboard with House Rules, Disaster Plan, Resident Rights, Theft and Loss Policy, Rights of Resident Council, Licensee Expiration dated 01/2025, Infection Control and the surety bond with an expiration date of 01/2025. There was a fire drill that was conducted in January 2024.
