Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Angela Panushkina conducted a Pre-Licensing Inspection and met with the Administrator and explained the reason for the visit. A tour of the physical plant was initiated at 10:20am and the following were observed: The facility maintains a comfortable temperature at 70°F. KITCHEN: The facility has a kitchen area that is equipped with a refrigerator, microwave oven and sink. LPA observed there to be sufficient stock of one-week perishable foods and two-day non-perishable foods. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. The fire extinguisher is located in the kitchen and was last serviced on 07/10/2025. BEDROOMS: The facility has six (6) bedrooms designated for residents’ use. All bedrooms are furnished with beds, dressers and required bedding and linen. The bedrooms have sufficient closet space and lighting. Facility also has an awake staff at night. BATHROOMS: The facility has three (3) bathrooms. LPA observed all bathrooms have non-skid mat, soap, paper towels and hand washing signs. Extra towels and linens were readily available. MEDICATION ROOM: At approximately 10:30am, LPA observed medications are centrally stored and locked, in the kitchen cabinet, and inaccessible to residents in care. The first-aid kit has been inspected which has at least the following: tweezers, scissors, antiseptic, bandages, gauze, thermometer; including a current First Aid manual. Continue on LIC809-C LAUNDRY ROOM: The laundry room is located by the garage. The washer/dryer appears to be in good condition. Laundry supplies are kept inaccessible when not in use with supervision. COMMON AREAS: These include a dining area and a living room. The Common areas are furnished with adequate furniture to accommodate a maximum capacity of six (6) residents. At 10:40am, smoke and carbon monoxide detectors were tested and observed to be operable. There were no visible immediate hazards. SURROUNDING GROUNDS: At approximately 10:45am LPA toured the outside area of the facility. LPA observed appropriate outdoor furniture, with a covered shaded area for residents. LPA discussed the importance of maintaining care and supervision to meet the needs of clients. There are no bodies of water. Between 11:00am to 12:30pm, LPA reviewed records of six (6) residents and three (3) staff. Resident and staff records appeared to be complete and updated. LPA observed Administrator certificated renewed on 11/30/2025 with an expiration date of 11/29/2027. LPA collected Certificate of Liability Insurance and LIC500. Oct 23, 2025Complaint investigation reportUnsubstantiated
Allegation investigated: Staff does not ensure resident's shower drain is in good repair.
This is an Amendment to the original report, issued 10/23/2025, due to a typographic error on LIC9099-C. At 9:00am, Licensing Program Analyst (LPA), Angela Panushkina conducted subsequent visit to deliver finial finding. LPA met with the Staff, Dondi Tolentino, who granted access to the facility. Administrator was contacted and LPA explained the reason for the visit. The Administrator was unable to come and designated the staff to sign for the report. During the initial visit conducted on 10/22/25, LPA requested resident and staff roster. At 1:40pm, requested copies of pertinent information which include, but not limited to Admission Agreement, Physician's Report, Appraisal Needs and Services Plan, relevant to the investigation. At approximately 1:50pm, LPA conducted a physical plant tour, to ensure health and safety of the residents are protected. Between 2:00pm - 3:00pm, LPA conducted an interview with the Administrator, three (3) staff and four (4) residents out of six (6) residents, who were able to communicate. Continue on LIC9099-C Unsubstantiated Allegation: Staff does not ensure resident's shower drain is in good repair. To investigate this allegation, LPA conducted an interview with the Administrator, during the initial visit, and was informed that the facility has three (3) full bathrooms available for residents use. Additionally, LPA was informed that the Administrator made aware of this issue on 10/15/25 and she immediately contacted the plumber. On 10/16/25 at 9:35am, the shower drain was cleaned with the drain snake, and all blockage was removed. Interview with three (3) staff members also confirmed the statement provided by the Administrator. Four (4) out of six (6) residents interviewed expressed no concern regarding this allegation. LPA also observed the shower drains in all three (3) bathrooms were in good repair. Therefore, based on interviews, LPA observation and information gathered during the initial visit, this allegation is deemed Unsubstantiated at this time. No deficiency issued during today's visit. Feb 20, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
At 9:45 AM Licensing Program Analyst (LPA), Huma Rahimi, conducted an unannounced annual inspection at the facility mentioned above. LPA met with the staff Nemi Salinas who granted access to the facility. The staff contacted Paige Esquivel the Administrator via phone and LPA explained the reason for the visit. The Administrator arrived later at the facility. Physical tour was conducted with the staff and LPA observed the following: LPA observed that currently there are six (6) residents in care, two (2) out of six (6) residents are under hospice care and one (1) is ambulatory. Kitchen: At 9:55 AM, LPA toured the kitchen area and observed enough supplies of two (2) days perishables and seven (7) days of non-perishables at the facility. All knives and sharps are observed to be locked in a kitchen drawer and inaccessible to residents. There is a fire extinguisher in the kitchen area and it was last serviced on 07/09/2024. Bedrooms: There are six (6) bedrooms designated for residents use. All bedrooms are properly furnished, clean and have appropriate bedding and linens. Auditory alarms were tested and observed to be operational. Facility has awake staff at night. Common Areas: The facility maintains a comfortable temperature at 78 °F. The living room and dining area appeared clean and were properly furnished. No obstructions or tripping hazards throughout the facility. LPA observed all trash cans throughout the facility have fitted lids. Bathrooms: LPA observed all bathrooms are clean and in good repair. Properly supplied with toilet paper, soap and paper towels. The hot water temperature measured 119.8 °F. LPA observed appropriate grab bars and non-skid mats. LPA observed appropriate hand washing signs posted in each bathroom. All trash cans in bathrooms had fitted lids to protect from cross contamination. Continued on LIC809-C Outside Areas: LPA toured the outside area of the facility. LPA also observed a clean covered patio and backyard furniture to accommodate the six (6) residents. There were no bodies of water observed. Laundry: Laundry area is in a bathroom and LPA observed laundry soap to be locked and inaccessible to residents in care. Garage: LPA observed an attached locked garage door inaccessible to residents. Extra supplies and food storage were also observed. Smoke Detectors/Carbon Monoxide. Dual smoke and carbon monoxide detectors were located throughout the facility, and at 12:35 PM they were tested and observed to be operational. Between 11:15 AM to 12:30 PM, LPA reviewed records of four (4) residents and two (2) staff and the Administrator. Residents and staff records/files were completed and updated. Administrative: LPA collected Certificate of Liability Insurance, LIC500, and Resident Roster. No citations issued during this visit. Exit interview conducted. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA), Lorena Casillas, met with Administrator Paige Esquivel for an unannounced one (1) year Required visit for this facility. At 9:45 am LPA arrived at the facility and was granted access by a Caregiver who then called the Administrator. Administrator arrived shortly after, and LPA explained the reason for the visit. LPA observed that currently there are six (6) residents in care, four (4) are under hospice care of one (1) of which is bedridden and one (1) are ambulatory. At 10:50 am, LPA conducted a tour of the facility, and the following was observed: Infection control: The facility had submitted and approved Mitigation Plan and Infection Control Plan. Proper signs were observed inside along the hallway and in the restrooms. Kitchen: At 11:00 am LPA toured the kitchen area and observed enough supplies of two (2) days perishables and seven (7) days of non-perishables at the facility. All knives and sharps are observed to be locked in a kitchen drawer and inaccessible to residents. There is a fire extinguisher in the kitchen area and it was last serviced on 07/14/2023. Bedrooms: There are six (6) bedrooms designated for residents use. All bedrooms are properly furnished, clean and have appropriate bedding and linens. Auditory alarms were tested and observed to be operational. Facility has awake staff at night. Common Areas: The facility maintains a comfortable temperature at 74 °F. The living room and dining area appeared clean and were properly furnished. No obstructions or tripping hazards throughout the facility. LPA observed all trash cans throughout the facility have fitted lids. Continued on LIC809-C Bathrooms: LPA observed all bathrooms are clean and in good repair. Properly supplied with toilet paper, soap and paper towels. The hot water temperature measured 113.9 °F. LPA observed appropriate grab bars and non-skid mats. LPA observed appropriate hand washing signs posted in each bathroom. All trash cans in bathrooms had fitted lids to protect from cross contamination. Outside Areas: LPA toured the outside area of the facility. LPA also observed a clean covered patio and backyard furniture to accommodate the six (6) residents. There were no bodies of water observed. Laundry: Laundry area is in a bathroom and LPA observed laundry soap to be locked and inaccessible to residents in care. Garage: LPA observed an attached locked garage door inaccessible to residents. Extra supplies and food storage were also observed. Smoke Detectors/Carbon Monoxide. Dual smoke and carbon monoxide detectors were located throughout the facility, and at 1:10 pm they were tested and observed to be operational. Resident Files: LPA conducted a file review of resident records to ensure compliance of licensing forms. Staff Files: LPA conducted a file review of staff records to ensure compliance of licensing forms. Medications: LPA and Administrator reviewed medication and medication records for proper documentation. Staff Interviews: At 12:38 pm LPA interviewed staff. Client Interviews: At 11:30 am LPA interviewed clients. Administrative: Annual fees are current. LPA collected Certificate of Liability Insurance, LIC500, and Resident Roster. Dec 14, 2023Complaint investigation reportUnsubstantiated
Allegation investigated: Facility does not maintain accurate staff records. Facility does not have a qualified administrator. Facility housed staff in the facility garage.
At 9:40am, Licensing Program Analysts (LPAs) Angela Panushkina and Huma Rahimi conducted an unannounced complaint visit at this facility to investigate the above allegations. LPAs met with the Administrator and explained the reason for the visit. During course of the investigation, interviews and record reviews were made. At 9:45am, LPAs requested resident and staff roster. At 9:50am, LPAs requested copies of pertinent information which include, but not limited to facility staff files, Administrator certificate, etc. relevant to the investigation. At approximately 9:55am, LPAs conducted a physical plant tour, to ensure health and safety of the residents are protected and physical plant is in compliance with Title 22 Regulations. Between 10:00am – 12:30pm, LPAs interviewed the Licensee, Administrator, two (2) staff and three (3) out of six (6) residents, who were able to communicate. Continue on LIC9099-C Unsubstantiated Allegation: Facility does not maintain accurate staff records. It was alleged that the facility listed non-staff as staff members for the purpose of obtaining Covid-19 vaccinations for them. To investigate this allegation LPAs conducted an interview with the Licensee and the Administrator and were informed that during the COVID (in 2021), facility had six (6) to seven (7) staff members working and during that time, the staff would call out, quite, etc. In addition, Licensee informed LPAs that during the COVID, staff would come and go and every new hired staff member got vaccinated, since it was a requirement at that time. Moreover, LPAs conducted an interview with two (2) staff members and both denied the allegation and reported no one, other than the facility staff member, received vaccinations. Lastly, LPAs observed former staff files in a cabinet that were archived and kept locked in the garage and current staff files were located in a kitchen cabinet and kept locked. LPAs reviewed the current facility staff schedule (LIC500) and the schedule accurately reflected who the LPAs observed working at the facility today. Allegation: Facility does not have a qualified administrator. To investigate this allegation LPAs conducted review of Administrators' certificate issued on 04/16/2021. LPAs observed the expiration date was 11/29/2023. Interview with the Administrator revealed that all required courses have been completed and all documents have been already mailed out around November 15th , 2023. At 11:39am, LPA Panushkina contacted "Administrator Certification Unit" and confirmed that the application received and is currently being processed. Allegation: Facility housed staff in the facility garage. To investigate this allegation, LPAs observed/reviewed the facility sketch and it was noted that the garage is used as a garage not as a room. At 9:58am, during the walk through, LPAs entered the attached garage, located by the kitchen, and observed many storage boxes in the garage. According to staff, the facility has only one awake staff at night, and the garage is being used for an extra storage and emergency supply. Based on inspection, observation and interviews there is no sufficient evidence to support the allegations. Therefore, all three (3) allegations are Unsubstantiated at this time.
