On April 6, 2026, Licensing Program Analyst (LPA) Eboni Bentley arrived unannounced for the purposes of conducting a required 1-Year annual visit using the CARE Inspection Tool. LPA Bentley was greeted and granted entry by staff, after stating the reason for the visit. Administrator (AD) Clara Han was contacted by telephone, unable to be present during the visit, and designated Caregiver Florentino "Jesse" Pedralvez to sign the report. The facility is licensed to operate forage range 60 and over, for six (6) non-ambulatory elderly residents, with a Hospice Waiver for four (4). The building is a single story structure located in a residential neighborhood. It consists of the following: six (6) resident bedrooms, one (1) staff bedroom, three (3) bathrooms, living area, dining area, kitchen, an outdoor covered seating areas, and an attached two car garage. During the inspection, LPA Bentley toured inside and outside of the physical plant with staff. There were no obstructions on the premises. A comfortable temperature of 73 degrees F was maintained in the facility. LPA observed the facility to be appropriately furnished at the time of visit and free of clutter and debris. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked and available at the time of visit. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available. Bathrooms were found to be clean and operational. The water temperatures in bathrooms measured at 111.9 degrees F to 117.0 degrees F. Continues on LIC 809-C…. The first aid kit is maintained and contains all the necessary elements. Smoke alarms and carbon monoxide detectors were operable. Emergency food, emergency water, and emergency supplies were available. The facility has one (1) fire extinguisher that was charged, mounted, and serviced on April 1, 2026. Liability Insurance is effective July 1, 2025, and expires on July 1, 2026. A working telephone (714-968-4909) remains available, however the facility does not have a device that can be used for video teleconference purposes. A Technical Violation was provided. The facility last conducted an emergency safety drill on March 26, 2019 and a deficiency was cited. During the visit, LPA conducted an audit of six (6) resident files (R1-R6), three (3) staff files (S1-S3), and medication and medication administration records. A total of three (3) resident and two (2) staff were interviewed. Based on today’s observations, a deficiency was cited, as per Title 22, Division 6, Chapter 8 of the California Code of Regulations. Licensing Program Analyst (LPA) Kimberly Lyman conducted a collateral visit to continue the investigation into complaint #22-AS-20220325133723 at closed facility 306005269. LPA was greeted and granted entry into the facility and explained the reason for the visit. During the visit, LPA reviewed and obtained documentation for complaint ##22-AS-20220325133723 as well as delivered findings to Licensee. Licensing Program Analyst (LPA) Jessica Cho arrived unannounced for the purpose of conducting the Required 1-Year annual evaluation using the Care Inspection Tool. LPA was greeted and granted entry by Caregiver Jesus Benbinen and explained the reason for the visit. Administrator (Admin) Clara Han arrived on premise to assist with the inspection approximately 9am. Administrator Han has a valid administrator's certificate expiring on February 10, 2027. The facility is a single story structure and is licensed to operate for six non-ambulatory and has a hospice waiver for four residents. LPA observed two caregivers providing care to six residents. During the tour of the physical plant, LPA observed six resident bedrooms and two resident bathrooms. There is a private staff bedroom occupied by two staff and one private staff bathroom. Resident bedrooms had all required furnishings. Bathrooms were found to be in compliance, clean, and operational except the private bathroom of Bedroom #3 evidenced by an unclean sink and shower. The private bathroom is not in use which was confirmed by Resident #5 (R5). The hot water temperature measured at 112.2 and 111.7 degrees Fahrenheit. All common areas were inspected including the attached two car garage which doubled up as an office and laundry area. LPA observed sufficient emergency food and water. Emergency supplies were unavailable. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food available in the kitchen. LPA toured the exterior portion of the facility. The outdoor passageway is free of obstruction. LPA observed sufficient seating and shading. The outdoor shed was secured and inaccessible. The outdoor fountain did not have water. The fire extinguisher was mounted, charged, and serviced on April 4, 2025. The auditory devices and smoke/carbon monoxide detectors were tested and operational in all areas. Emergency evacuation drills are not conducted quarterly. The first aid kit contains all necessary elements. The Complaint Poster, 'See Something, Say Something,' (PUB 475) was available and posted in the incorrect size of 8.5"x11." LPA conducted a file review of all residents and two staff. No discrepancies noted. Medications were audited for three of six residents. No discrepancies noted. Interviews were also conducted. The following items were addressed during the exit interview: to conduct quarterly emergency disaster drills, to obtain the emergency supplies as indicated on the Emergency Disaster Plan (LIC610E), to increase the size of the Complaint Poster (PUB 475) to 20"x26," and to deep clean the private bathroom of R5. Based on the observations made during today's visit, no deficiencies are being cited. Advisory Notes (LIC9102s) are being issued. On today's date, Licensing Program Analysts (LPAs) LPA Rosie Quiroz made an unannounced visit on this day for the purpose of conducting an Annual Required-1 year inspection visit. LPA Quiroz was greeted by Caregiver (CG) 1. Licensee/Administrator (L/AD) Clara Han arrived shortly after and discussed purpose of the visit. The facility is a one story home which consists of: 4 private resident bedrooms, 1 shared resident bedroom, 1 caregiver bedroom, 2 bathrooms for residents, 1 bathroom designated for staff and visitors, living-room, kitchen with dining area, back yard with ample shade and outdoor furniture, garage with operational washer and dryer. The facility is licensed for age range 60 and over, 6 Non-Ambulatory residents, and has a hospice waiver for 4 residents. There is currently (1) one resident currently receiving hospice care services. Administrator Clara Han has a current Administrator's certificate which expires on 2/10/2025. LPA Quiroz along with CG1 toured the interior and exterior of facility. LPAs observed kitchen for proper sanitation and cleanliness. LPAs observed Sharp objects locked and secured in cabinet in kitchen area, and observed toxin substances to be locked and inaccessible to residents in care in locked and secured closet in garage area. The food was observed to meet the minimum requirement of two days perishable and seven days nonperishable food supply for residents in care. Six of six resident bedrooms were observed to have required furnishings including but not limited to: lamp, bed, mattress, night stand and clean linen. LPA Quiroz interacted and interviewed 6 of 6 residents during facility tour. The water temperature in 3 of 3 resident bathrooms were measured to be within normal limits. The Grab bars and non-slip mats were observed in resident bathrooms. Room temperature inside the facility was recorded to be 74 degrees F. CONTINUED NEXT LIC 809-C PAGE... CONTINUED...The outside grounds were inspected during facility tour, and no bodies of water were observed. No pool available at this facility. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. LPAs observed one fire extinguishers mounted and charged in kitchen area, which was last serviced on 4/23/2023. LPAs observed and tested functional and operational carbon monoxide and smoke alarms. (AD) Clara Han reports fire drills are conducted bi-monthly with staff and residents. Last facility fire drill was conducted on October 7,2023 indicating " We will be conducting a fire drill this week." (AD) Clara Han reported offering the following indoor and outdoor activities at the facility according to weather conditions: "Ball throwing exercises, puzzles, outdoor walks in wheelchairs, board games and bike peddling exercises." LPA Quiroz reviewed whether the facility is operating within capacity limitations and reviewed repeated violations, no repeated violations assessed. During today's visit, LPAs reviewed resident and personnel records. 6 of 6 Resident files and 4 of 4 personnel files were found to be within Title 22 California Code of Regulations. LPA Quiroz reviewed centrally stored medications during today's visit.
