On 06/18/2026 at 1230pm, the Department conducted an unannounced required Annual visit with a primary focus on infection control. Licensing Program Analyst (LPA) Zina Brown was met by caregiver Rissa Angenrani. Administrator France Reeder was not available to be at the facility and was notified by telephone of the visit. The facility is licensed for adults 60 years and over for (6) non-ambulatory residents. Facility is approved for (2) hospice residents and currently has (1) residents in hospice care. The facility has a administrator certification for Frances Reeder #7007037740 valid 12/01/2024 - 11/30/2026. The facility fees are current and paid in full. The facility has liability insurance with Kinsale Insurance Company with each occurrence at $1,000,000 and general aggregate at $3,000,000 which is valid from 03/22/2026 - 03/22/2027. The facility last fire and earthquake safety drill was conducted on 03/16/2026. The facility is a single-story structure with a ramp and front porch located in a residential neighborhood. The facility consist of (6) resident rooms, (1) staff room, (3) full bathrooms, kitchen, living / dining room, and patio area. Between the hours of 12:55pm - 1:10pm, the Department and caregiver Angenrani toured the physical plant. There are no bodies of water or on the premises. All resident rooms were checked. Beds and bedding were in good condition, adequate lighting provided, storage for resident's personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were clean and operational. The water temperature measured at (120.F in 1 half bathroom/ 2 full bathroom/den area, 119.8 in kitchen, 118.9 in half bathroom and 116.6F in full bathroom). A comfortable temperature is maintained in the facility. LPA observed the facility to be clean and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning agents, toxins, and sharps were inaccessible to resident. The kitchen was inspected and there is enough perishable and non-perishable food available which is stored properly. Fire extinguisher was charged, smoke detectors and Carbon Monoxide were operable. Between the hours of 1:15pm - 3:30pm the Department conducted a records review of (6) residents records, (5) staff records and (6) Record Medication Administration Records. During today's visit the Department did observe deficiencies. DEFICIENCIES: Resident 1 (R1), Resident 3 (R3), Resident 4 (R4) and Resident 6 (R6) has half bed rails with no is not on hospice care nor had physician's orders for bed rails. Administrator did not have personnel file available at the time audit for review for Staff 1 which is the Adminstrator. Staff #5 no associated to the facility in Guardian nor LIS. As a result, civil penalties are being assessed and a deficiencies are being cited under California Code of Regulation Title 22, Division 6, Chapter 8 are being cited on the LIC 809-D. On 2/06/25, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced required Annual visit with a primary focus on infection control. LPA was met by caregiver Rissa Angenrani. Administrator France Reeder was not available to be at the facility and was notified by telephone of the visit. The facility is licensed for adults 60 years and over for (6) non-ambulatory residents. Facility is approved for (2) hospice residents and currently has (2) residents in hospice care. The facility is a single-story structure with a ramp and front porch located in a residential neighborhood. The facility consist of (6) resident rooms, (1) staff room, (3) full bathrooms, kitchen, living / dining room, and patio area. LPA and caregiver Angenrani toured the physical plant. There are no bodies of water or firearm/ammunition on the premises. All resident rooms were checked. Beds and bedding were in good condition, adequate lighting provided, storage for resident's personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were clean and operational, yet outside of Title 22 regulations. The water temperature measured at 118.3 F. A comfortable temperature is maintained in the facility. LPA observed the facility to be clean and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning agents, toxins, and sharps were inaccessible to resident. The kitchen was inspected and there is enough perishable and non-perishable food available which is stored properly. Fire extinguisher was charged, smoke detectors and Carbon Monoxide were operable. Evaluation Report continues LIC 809-C During the visit, LPA observed the facility infection control practices. LPA observed screening protocols for visitors, staff and residents, sanitizing stations (located in common areas and restrooms). All mandated CCL posters were posted. Audit of (4) resident service files and (3) staff personnel files were determined to be maintained in order. DEFICIENCIES: Resident #1 had full bed rails and was not on hospice care nor had physician's orders for bed rails. Staff #4 did not have personnel file available for audit/review. Administrator Certificate for Frances Reeder expired December 2024 and no renewal on record on file. On 06/03/24 Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced Annual required visit using the CARE Tool. LPA met with Rissa Angerani, Caregiver and the purpose of today’s visit was explained. LPA was granted access to the facility. Facility is licensed to serve six (6) residents aged 60 and above, five (5) non-ambulatory and one (1) bedridden. There are currently five (5) residents in the facility. The facility is a single-story home located in a residential neighborhood. The facility consists of six (6) bedrooms, one (1) staff room, four (4) bathrooms including a separate shower room for residents, a living room, dining room, kitchen, laundry area, and an attached garage with a backyard shaded area. LPA Gonzalez and Rissa Angerani toured the inside and outside grounds of the facility. There were no bodies of water or obstructions on the premises. Resident bedrooms had the required furniture, and adequate lighting was provided. LPA observed bed linens and closet/drawer space to accommodate each resident comfortably. Bathrooms were checked and found to be within title 22 regulations and were operational. The water temperature measured between 105.0 F and 120.0 F in all bathrooms. LPA observed linens, bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. A comfortable temperature was maintained in the facility. The kitchen was inspected and there is sufficient perishable and non-perishable food supply maintained adequately. All sharps, toxins, cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. LPA Gonzalez observed the facility to be sanitary and appropriately furnished at the time of the visit. Medications were centrally stored and properly locked. A review of Medication Administration Records was maintained in order and accurate. Continued on LIC809-C The facility has a landline telephone on-site in working condition. Smoke detectors and carbon monoxide detectors were operational and working properly. Fire extinguishers were charged and operable. LPA observed a stocked First Aid kit along with manual. During the visit, LPA observed the facility's infection control practices. LPA observed screening protocols for visitors, staff, and residents. There are sanitizing stations in common areas and restrooms. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. During this inspection LPA did not observe any deficiencies, therefore no citations were issued at this time.
