Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) N. Galarza arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual evaluation. LPA was greeted and granted entry by Administrator Bryan Bernardino. Administrator has a valid administrator's certificate expiring on 12/5/2026. *Note: Administrator on record is not current. The following was observed during the inspection: Infection Control: The facility has an Infection Control Plan. Operational Requirements: A Dementia and hospice waiver for 6 residents has been approved. A fire clearance for 6 bedridden adults 60 and over. Facility does not handle resident P & I monies. Liability Insurance in the amount of at least ($1,000,000) per occurrence and ($3,000,000) in total annual aggregate is current with an expiration date of 7/1/2027. Physical Plant/Environment Safety: The interior and exterior physical plant was inspected. The facility is a single-story home located in a residential area consisting of six (6) private bedrooms, four (4) bathrooms, kitchen with laundry area, dining room, living room, attached 2-car garage, backyard outdoor covered patio, storage shed in the rear of the backyard. Front and backyards have pavers. Resident rooms are equipped with required furniture, mattress pads, and lighting. Cleaning supplies and toxic substances are inaccessible to residents. The facility has one fire extinguisher. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. Electrical smoke and carbon monoxide detectors were tested and are operational. It is equipped with sprinklers. The facility maintains emergency food supply and water. Emergency Phone numbers, exit plan and programming schedules were posted. The building contains central air conditioning and heating. Exit doors are free of any obstruction and there are no pools. The last Emergency Disaster drill was conducted on 7/2/2026. * A citation was issued: The kitchen door connects to garage does not open & there are missing pavers on the side of the home that pose a tripping hazard. Staffing: A total 11 staff members provide care and supervision to the clients. Personnel Records/Staff Training: Staff have criminal background clearance and training. Five (5) staff files were reviewed. Staff files were incomplete, personnel documents had names of other facilities, and health clearance was not in the file, as well as 1st Aid/CPR was not current in some staff files. A citation was issued. Resident Records/Incident Reports: A total of six (6) resident files were reviewed. They contained admission agreements, Physician's Reports, Appraisals, TB clearance, Physician's Orders, medical consent. Resident (R6's) medical assessment was missing TB exam/results information. A citation was issued. RCFE complaint poster and Personal rights were observed posted. Planned Activities: Sufficient space to accommodate both indoor and outdoor activities was observed. The facility does not have a Resident Council. Food Service: Sufficient food supply is stored in the kitchen and pantry areas consisting of: 2-day perishables, 7-day non-perishables, and emergency food supplies. Residents have modified diets. Incident Medical and Dental: Three (3) centrally stored / 30-Day supply of medications were reviewed. Medical and dental transportation is provided by family. Disaster Preparedness: Emergency and Disaster Plan LIC 610E was reviewed. Facility has a First Aid Kit and Manual. Residents with Special Health Needs: Two (2) residents receive hospice services and two (2) residents receive home health services. Two residents are bedridden. R1 and R5's beds have 2 half rails (split rails) on each side of the bed. A citation was issued. Pursuant to California Code of Regulations, Title 22, deficiencies were cited. Jul 24, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On 07/24/2025, the California Department of Social Services (CDSS) – Community Care Licensing Division (CCLD) staff conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Administrator, Bryan Bernardino. CCLD staff explained the purpose of the visit and was accompanied by a staff member inside and outside the facility during this inspection. This facility is licensed to serve 6 bedridden adults ages 60 and above. The facility has an approved hospice waiver for 6 residents. A total of 5 residents are currently residing in this facility. The Annual Licensing Fees are current. Facility Layout: The facility is a one-story house located in a residential street. The home consists of 6 resident bedrooms, 1 staff bedroom, 4 full bathrooms, 1 kitchen, 1 dinning room area, 1 living room area, 1 attached garage, 1 front yard patio area, 1 backyard patio area with shaded seating. Outside Grounds: were toured no bodies of water were observed, walkways around the home were clear of hazards, and there are no security bars or weapons on the premises. Kitchen Area/Facility Food: The facility has supplies of nonperishable foods for a minimum of one week and fresh perishable foods for a minimum of two days. Knives and toxins were kept inaccessible to residents in care. There is a fire extinguisher in the kitchen area, and it was last serviced on 04/14/2025. There is a landline phone in the kitchen. Living Room: There are games/activity work (i.e. board games and books) for residents in the living room area. There are couches and chairs for residents to sit at. Resident Bedrooms: 5 out of 6 resident bedrooms were toured. There is adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Bathrooms: Toilets, showers, and water faucets worked properly, grab bars were secure, and a non-skid mat was in place. Adequate lighting and toiletries are accessible to residents. The hot water temperature measured between 102 Fahrenheit to 110 Fahrenheit. Medications: were inaccessible to residents in care. All medications observed were labeled and maintained in compliance with label instructions and State and Federal law. 5 out of 5 resident Medication Administration Records (MARs) were reviewed and they were current and up to date. Garage: is used as a storage room and extra facility food and supplies are kept in the garage. Miscellaneous: Documents are posted as mandated. Last fire drill was conducted on 07/2025. The first aid kit is fully stocked with manual. Smoke and carbon monoxide detectors were in compliance and operational. There is a videoconferencing device in the facility for resident use. 5 staff records were reviewed, 5 out of 5 staff records have required criminal record clearance. 2 out of 5 staff records were not complete and current by not having the health screening report, training's, and current first aid certificate. 5 resident records were reviewed, 3 out of 5 resident records were not complete and current. Resident 2 did not have: Physicians Report, Preplacement Appraisal Information, Tuberculosis Test Results, Personal Rights, and Safeguards for Property/Valuables. Resident 3 and Resident 4 did not have an updated Appraisal within the 12 months. A technical violation is being provided regarding maintaining complete and current staff records in the facility. A deficiency is being cited based on record review in accordance with the California Code of Regulations, Title 22, see LIC809D. A violation regarding maintaining complete and current resident records in the facility. An exit interview was conducted, Plans of Corrections were reviewed and developed. Type of visit: Required - 1 Year
On 06/28/24, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced annual visit to the facility listed above. LPA met with Caregiver, Milagros ‘Mila’ Santos, and the purpose of today’s visit was explained. The facility is licensed to serve six (6) bedridden residents over the age of 60 and has been approved for a hospice waiver for six (6). The facility currently has six (6) residents. Physical Plant/Structure The facility is a single-story structure located in a residential neighborhood and consists of the following: 6 resident bedrooms, 1 staff bedroom, 2 common bathrooms, 2 private bathrooms, living room, dining area, kitchen, staff work area, laundry area, and attached garage. There is an outdoor activity area with a table with umbrella and chairs. There is an additional table and chairs in the front of the home. LPA observed all walkways around the home to be clean, clear, and free of obstructions, debris, and hazards. LPA did not observe any bodies of water on the premises. Bedrooms LPA inspected all rooms and found them to be clean and in good repair. LPA observed the rooms have the required furniture including a bed, dresser, nightstand, chair, and storage space for personal belongings. LPA observed the beds have the required linens including a mattress cover, fitted sheets, blanket, comforter, and pillows. LPA observed an additional supply of linens stored in a cabinet in the office area. All linens and mattress were observed in good condition. All rooms were observed to have ample lighting. Bathrooms LPA observed all bathrooms to be operable and within Title 22 regulations. The toilet, facets, and shower work properly. The bathrooms were observed clean. The showers were observed with secured safety handrails, nonskid mats, and a shower chair. The showers were observed to be free of mold and mildew. LPA observed storage space for residents’ hygiene products. LPA observed an ample supply of towels and hygiene products available for residents. The water temperature measured between 110.2-degrees and 114.0-degrees Fahrenheit. Kitchen LPA observed the kitchen to be clean and sanitary. LPA observed all appliances to be operable. LPA observed an ample supply of cookware, dishware, and cutleries. LPA observed a 3-day supply of perishable foods and a 7-day supply of nonperishable foods. All foods were observed properly labeled, packaged, and stored. LPA observed knives and sharps to be secured in a locked drawer and are inaccessible to residents. LPA observed a sample menu posted in the dining room. LPA observed cleaning supplies to be secured in cabinet in the laundry area and are inaccessible to residents. The water measured 113.4-degrees Fahrenheit. Common Rooms LPA observed the living room to have four (4) recliners and a large couch to accommodate all residents. LPA observed in the dining room a large table to accommodate all residents. LPA observed activities and reading material available for residents. LPA did observe a screened fireplace that is inaccessible to residents. All walkways and hallways in the home were observed to be clean, clear, and free of hazards and obstructions. The facility was kept at a comfortable temperature. All rooms and hallways were observed to have ample lighting. File Review/Interviews LPA reviewed six (6) resident files and found they contained the required documents. LPA interviewed three (3) residents who are happy with the service and care they receive at the facility. LPA reviewed the Administrator file, and two (2) staff files and found they contained the required documents, certification, and training. LPA interviewed two (2) staff who were able to answer questions regarding resident care, policies, procedures, and resident personal rights. LPA observed the administrator’s Administrator Certificate is valid till 09/17/25. LPA observed all licensing fees are current. Safety LPA observed the smoke detectors and carbon monoxide detector to be operable. LPA observed a fully charged fire extinguisher last serviced on 10/20/23, located at the entrance of the kitchen. The last inspection conducted by the Torrance Fire Department was on 06/17/24. The last emergency drill was conducted on 06/04/24. All doors exiting the facility has a sensor/alarm that informs staff when a door is opened. The facility has a working landline telephone. LPA observed the facility’s Emergency and Disaster Plan posted in the office area in the dining room. LPA observed all required posting, posted throughout the facility. Medications LPA observed medications secured in a locked closet in the office space in the dining room and are inaccessible to residents. LPA observed medications to be in their original packaging. LPA reviewed the medications and Medication Administration Record (MAR) for six (6) residents. LPA observed Six (6) out of six (6) resident’s MARs and medication are consistent with properly documented records. Infection Control During today’s visit, LPA observed the facility’s infection control practice. Before you enter the facility there is a table with a visitor log and hand sanitizer. Upon entering the facility, LPA observed a cart with masks and Personal Protective Equipment (PPE). LPA observed all required Infection Control signs posted throughout the facility. LPA observed a 30-day supply of PPE. During today’s visit, LPA did not observe or cite any deficiencies. Allegation investigated: Staff do not ensure that resident's needs are met at night.
On 03/25/2024, Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced complaint visit. Upon arrival at the facility. LPA Richard met with staff Milagros Santos, Frederico Santos. LPA Richard explained the purpose of today's visit. Later was joined by administrator Edilberto Bernardino. The investigation consisted of the following: LPA Richard and administrator Bernardino toured the facility grounds, and interviewed Administrator, Staff (S1-S3) Resident (R3-R5) and witness (W1). LPA Richard requested and obtained a Resident roster, staff roster, Admission Agreement, Medications (MAR), Needs of service plan, and Physician report for Resident care LPA Richard requested and reviewed facility records. Unsubstantiated Allegation: Staff do not ensure that resident's needs are met at night. It was alleged that “staff do not ensure that resident’s needs are met at night.” On 03/25/2024, LPA Richard interviewed three (3) staff (S1-S3), three (3) residents (R3-R5) and one (1) witness (W1). LPA interview two residents (R1 and R2), but LPA was unable to obtain statements due to the residents’ medical conditions. Based on interviews with (S2 and S3), the facility has scheduled two staff at night. There always a staff around to provide and assist the resident when the resident ring the bell or move out the bed. The staff stated that not true and residents’ needs are being met. Staff (S1-S3) stated the facility ensure there is sufficient, trained, and competent staff to provide the services needed to meet resident’s needs. The administrator (S1) stated that the facility has one (1) Resident (R1) who was agitated at night, couldn’t sleep. In February the resident physician prescribed a new medication and increasing the doses of one of R1 medications now the resident (R1) is sleep through the night. Interview with residents (R3-R5), indicated that the staff provided them with their needs, and staff always around to assist them. LPA interview with witness (W1) stated that the problem with (R1) was resolved. The physician changes the medications and doses now, there is no more concerned about (R1), staff does ensure resident needs are met at night. Continued LIC9099-C. Continue LIC9099-C Based on observations, interviews and record review, there is no sufficient evidence to prove the above allegation. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
