Type of visit: Required - 1 Year
Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today for the facility’s annual inspection. LPA introduced themselves and was allowed entrance into the facility. LPA met with Khushbir Mann Administrator. Facility is licensed for 6 residents and has a current census of 3. There are 3 residents on hospice and 0 with home health care. Water temperature was checked in the kitchen which read at 114.8 degrees Fahrenheit and the common bathroom which read at 113.5 degrees Fahrenheit. Fire Extinguisher was purchased June 30, 2026 and is within the safety regulation period. Smoke and carbon monoxide detectors were are in working order. Khushbir Mann Administrator's Certification expires July 7, 2027. Staff files were reviewed, are complete, and current. Resident files were reviewed, complete, and current. Facility provided a log for disaster drills that are conducted quarterly. First aid kit on site. Toxins and cleaning supplies are locked and inaccessible. There is a locked storage for medications. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms, bathrooms, medication storage, kitchen, garage, and outdoor areas. Bedrooms were clean, properly furnished, with adequate lighting, and in good repair. Food supply is adequate for 2-day perishable and 7-day nonperishable. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA is requesting the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Listed documents shall be sent to Licensing June 12, 2026. Type of visit: Required - 1 Year
On 07/08/2025, Licensing Program Analyst (LPA) Daiquiri Boyd made an unannounced visit to the facility for the purpose of conducting an Annual Inspection. LPA arrived at the home and after ringing the Ring Video Doorbell, spoke to Licensee/Administrator (AD) Khushbir Mann, through the video doorbell. Licensee stated that they were not home and that they don't have any residents at this time. LPA explained that it is still required that I enter the home to verify the census and to review any files and observe the facility. AD arrived at the facility a short time later. LPA was allowed entry into the facility, which was found to be clean and in "like new" condition. This facility currently has no residents. There are no client living at the facility nor files to review at this time. LPA reviewed the AD file. This is a four bedroom home with two bathrooms. All rooms are nicely and completely furnished per regulation, with appropriate bedding, chair, and a bedside light for reading. All window screens are intact. The fire extinguisher is dated 7/1/25. Water temp was set at 120 degrees F. All postings are in place. There are linens in the hall closet. There is a locked closet in the hallway that will store medications. All sharps were locked in a kitchen drawer. Trash cans were observed through out the facility with tight shutting lids. Facility has it's own generator stored in the garage. There are no citations at this time. LPA advised AD to notify Licensing when they receive their first client. Type of visit: Prelicensing
On 6/7/2024 Licensing Program Analyst (LPA) B. Miranda arrived at facility announced to complete an Pre-Licensing visit. LPA met with Administrator (AD) Khushbir Mann. LPA observed the previous deficiencies to be corrected. The window screens had been replaced, grab bars were added to the master bathroom, a wall was added to the master bedroom. The wall was added due to the master bedroom having two closets. This allows one of the closets to be locked and used for cleaning supplies and allows the resident who will resident in the master bedroom to have privacy. Pre-licensing inspection has been completed. COMP III was provided to Administrator Khushbir Mann to complete and was completed. Type of visit: Prelicensing
On 5/17/2024 Licensing Program Analyst (LPA) B Miranda arrived at facility announced to complete an Pre-Licensing visit. LPA met with Administrator (AD) Khushbir Mann. LPA toured the facility inside and out. During the tour LPA observed 4 bedrooms and 2 bathrooms to be properly furnished for residents. Two bedrooms are for single resident use and two bedrooms are for shared. LPAs observed the following deficiencies: 1. Outside screens facing the street need to be repaired. 2. Grab bars need to be added to the master bathroom. 3. Cleaning supplies will be moved from closet in master bedroom to different locked area Pre-licensing is incomplete with deficiencies to be resolved by 5/31/24.
