On 06/04/26, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced annual visit using the CARE Inspection Tool. LPA met with Administrator Jennifer Block and explained the purpose of today’s visit. The facility is licensed to serve two non-ambulatory and four ambulatory residents. The facility has a hospice waiver for three residents. Two residents are currently receiving hospice services. The facility is a two story structure located in a residential neighborhood. The first-floor licensed facility consists of the following: four resident rooms, three bathrooms of which two are private, living room, dining room, kitchen, office and an enclosed patio/activity area. The Administrator accompanied LPA inside and outside the facility during this inspection. Outside grounds were toured and no bodies of water were observed. Walkways around the facility were clear of hazards. Resident bedrooms had bed linens and closet/drawer space to accommodate each resident comfortably. There are no security bars or weapons on the premises. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew... Continue to LIC809-C. a non-skid mat was in place, and water temperature measured at 117.2 degree Fahrenheit. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Doorways were free of obstructions. LPA toured the kitchen area and observed a two-day supply of perishable and a seven-day supply of non-perishable food. Knives and toxins were kept in a lock cabinet. First aid kit was available. Two fire extinguishers, last serviced on 02/24/26, were observed. A fire drill with lecture was conducted on 04/28/26. Five staff records were reviewed; five out of five staff records had the required criminal record clearances or criminal record exemptions. Four resident records were reviewed; four out of four resident records had medical assessments and pre-appraisal or reappraisals. Two residents’ medication was reviewed. Deficiencies were observed and citation issued, see LIC809-D. An immediate civil penalty of $500 was issued, see LIC421IM. During record review, medical assessments/physician’s report for Resident #1 (R1) and Resident #2 (R2) revealed residents are non-ambulatory. Review of Register of Residents revealed R1 lived in room 11 and R2 lived in room 14. Review of Fire Safety Inspection Request revealed rooms 11 and 14 are for ambulatory residents only. On 3/28/2025, At 1:22 PM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual comprehensive inspection. LPA was greeted by Jennifer Block- Administrator upon arrival, LPA introduced herself, explained the purpose of the visit and was granted access into the facility. The facility is licensed for six (6) residents, two (2) non-ambulatory, (4) ambulatory and there is a hospice waiver for (3). Currently, there are five (5) residents, all are over the age of 60. The facility is a two story structure located in a residential neighborhood. The first-floor licensed facility consists of the following: four (4) resident rooms, three (3) bathrooms, living room, dining room, kitchen and enclosed patio/activity room area. LPA observed five (5) residents in care at the time of visit. At 1:45 PM, LPA and Svetlana Dagala- support staff toured the physical plant. There were no bodies of water or obstructions on the premises. The beds and bedding supplies were in good condition, adequate lighting is provided, and there is storage space for residents personal belongings. LPA also observed additional bed linens, comforters, and bath towels were adequately stocked at the time of visit. The bathrooms were clean and operational. The water temperature ranged from 105-117.5 F° and the facility temperature ranged from 76F° – 78F°. At 2:15 PM, LPA observed that facilities kitchen to have a supply of 5-days of perishables and 7-days of non-perishables food items for the number of clients in care which were stored and maintained properly. There was also a menu available for review. At 2:30 PM, LPA observed the storage area where the cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. Smoke detectors were observed to be hardwired and interconnected. Facility has fire extinguishers, fully charged and the drill was conducted on 1/14/2025. During the inspection LPA did not observe deficiencies therefore no citations were issued at this time. On 05/23/2024 Licensing Program Analyst (LPA) David España conducted an unannounced annual required visit with a primary focus on using the CARE Inspection Tool. LPA met with Administrator Gary Strathearn and explained the purpose of today’s visit. The facility is licensed for (6 residents), (2) non-ambulatory, (4) ambulatories and Hospice Waiver for (3). Currently, there are 4 residents, all are over the age of 60. The facility is a one-story structure located in a residential neighborhood. The first-floor licensed facility consists of the following: four (4) resident rooms, three (3) bathrooms, living room, dining room, kitchen and enclosed patio/activity room area. LPA observed four (4) residents in care at the time of visit. LPA and Administrator toured the physical plant. There were no bodies of water or obstructions on the premises. Beds and bedding supplies were in good condition, adequate lighting provided, storage for client personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature ranged from 117.5F°. The room temperature ranged from 76F° – 78F°. CONTINUE ON LIC 809C LPA observed the facility to be sanitary and appropriately furnished at the time of visit. Storage area cleaning supplies, toxins, and sharps objects were stored and not accessible to clients. The kitchen was inspected and there is sufficient perishable and non-perishable food available maintained properly. Smoke detectors were observed to be hardwired and interconnected. Facility has fire extinguishers, fully charged. During the visit, LPA observed the facility infection control practices. LPA observed screening protocol for visitors, staff, and residents, sanitizing stations in common areas and restrooms. LPA observed staff were wearing face coverings. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did not observe deficiencies therefore no citations were issued at this time.
