Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Nacole Patterson conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by and discussed the purpose of the visit to Administrator Isela Vega. The facility's license shows a maximum capacity of twenty eight (28) non-ambulatory residents, ages 60 and above. Bedridden waiver for nine (9), hospice waiver for fourteen (14). During today’s inspection there were twenty six (26) residents in care. LPA and Administrator Isela Vega toured the interior and exterior of the facility and inspected a sample of rooms. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility contained at least 2 days of perishable food, and at least 7 days non-perishable food, all safely stored. Cooking, dining equipment, and utensils were present. No toxic chemicals or poisons were accessible to clients. Medications were labeled, as required, and stored in locked areas. No pools or bodies of water exist on the premises. Per Administrator Isela Vega, no firearms or ammunition are kept at the facility. Carbon monoxide detectors, emergency lighting, and facility telephone were all in working order. Fire extinguisher(s) were serviced within the last 12 months. First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA interviewed staff and clients, and reviewed facility records. The files reviewed by LPA contained required documents. Confidential records were stored in locked areas. No deficiencies were cited during the inspection. Technical Advisory was provided regarding expired foods. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) David Roman conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA D. Roman entered the facility and identified himself to, Administrator, Isela Vega. LPA D. Roman discussed the purpose of the visit with the Administrator. According to the facility’s license, the facility has a maximum capacity of twenty eight (28) clients, of whom all may be non-ambulatory and nine (9) of whom may be bedridden on the first floor only. LPA D. Roman along with Administrator toured the interior and exterior of the facility and inspected each room. The facility was sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. Doors, windows, toilets, and showers were in working order. On the 1st floor the facility had sufficient space and equipment to facilitate dining, visitation, meetings, and resident activities. Laundry equipment is found on both floors. LPA D. Roman along with Administrator, Isela Vega, entered the kitchen area, reviewed non-perishable foods for expiration dates, observed all refrigerators in working conditions and with plenty of food. Cooking/dining equipment and utensils were present. There were no toxic chemicals/poisons accessible to residents. Medications were labeled, as required, and stored in locked room. Water temperature was measured at 115 degrees F. Two (2) exit doorways were tested for signal system. Administrator reported to have the auditory signal systems replaced due to malfunction. No pools or bodies of water on the premises. LPA inquired if firearms or ammunition are kept in the facility, Administrator reported, no. On 02/07/25 fire system and carbon monoxide detectors were tested by 3rd party vendor. Emergency lighting and facility telephone were all working. Fire extinguisher(s) were present and serviced. First aid kit(s) were complete and readily accessible. Resident records reviewed had required documentation. Staff records reviewed had required documentation. An exit interview was conducted with Administrator, Isela Vega, to whom a copy of this report and the Licensee/Appeal Rights were provided during the visit. May 8, 2024Complaint investigation reportUnsubstantiated
Allegation investigated: Staff do not respond to residents' calls for assistance timely Staff handled resident roughly
Licensing Program Analysts (LPAs) Juliana Barfield and Sabel Martinez conducted an unannounced visit to conduct interviews and deliver investigative findings. LPAs introduced themselves, were granted entry into the facility, and met with Isela Vega, to whom they disclosed the reason for the visit. Community Care Licensing (CCL) has investigated the complaint allegation. The investigation consisted of a tour of the facility, review of facility records, and interviews of residents and staff and outside sources. It was reported to CCL that staff do not respond to resident calls for assistance timely. It was also reported that staff handled a resident roughly. Interviews with residents reported that there was no untimely assistance by staff. All residents interviewed including resident (R1) said that staff respond within a few minutes when they press their call pendants that they wear around their necks at all times. Unsubstantiated It was reported in staff interviews that there are two centralized call monitors on the first floor in the common area. All pendant activations are heard by staff in the common area and one staff member is dispatched within minutes of the alarm. There is always one staff member in the area that can hear the alarm. It was also alleged that staff handled resident roughly. Interviews with residents did not conclude any rough handling by staff. Staff also reported no mishandling of residents. Outside source interviews revealed that abuse has not been observed and there are no concerns for abuse. Based on interviews and record reviews there is insufficient evidence to support that the alleged violations occurred. Therefore the complaint is unsubstantiated. Type of visit: Required - 1 Year
Licensing Program Analysts (LPA) Amy Rodgers, made an unannounced visit to conduct the required One-Year Inspection to ensure substantial compliance with Title 22 regulations. LPA Rodgers was granted entry into the facility by Marjorie Galang, Administrator, after identifying herself and stating the purpose of the inspection. This facility serves twenty-eight residents, ages 60 and above; of which all are non-ambulatory. Nine (9) of whom may be bedridden on the first floor. Approved for hospice for fourteen residents. Facility is equipped with a secured perimeter. A tour of the facility was conducted which included a sample of resident units, the dining area, common gathering areas, and food storage areas. There are no water features on site. Each resident had clean and sufficient bed linens. All residents’ rooms were equipped with the required furnishings. Lighting was present in the bedrooms. Residents’ bathrooms were observed to be sanitary and operational. Showers were equipped with grab bars and non-slip mats. Hot water temperature in residents’ bathrooms were compliant. The facility is operating in accordance with their fire clearance. The smoke and carbon monoxide alarms were present in the building. Emergency lighting, and facility telephone were all working. First aid kit(s) were complete and readily accessible in the medical rooms. Required licensing postings were observed in visible areas of the facility. PPE supplies are onsite. Passageways were free from obstructions. Facility has a two-day supply of perishable food and a seven-day supply of nonperishable food items.Food supply is replenished frequently by staff. Food was observed to be properly labeled. The food service area was observed to be neat and clean. Food menus and activities schedule were posted. Centrally stored medications were properly stored and locked in a medication room. [CONTINUED ON LIC 809-C] [CONTINUED FROM LIC 809] Staff records review verified that all staff records were complete and compliant. Resident records were reviewed and confirmed compliant. Administrator’s certification is current. LPA reviewed the theft and loss policy and procedures. LPAs conducted a review of In-service training procedures. LPA interviewed Administrator Galang and was assured transportation procedures as well as outside medical and dental assistance procedure are compliant. There is a large common room used for dining and activities such daily exercises, game activities, and arts/crafts.. At the time of visit, LPA observed a large group activity, in which many residents were participating. LPA observed that residents were being treated with dignity by staff, and there were sufficient staff on duty to meet resident’s needs. No deficiencies were issued at the time of visit; however, technical violations were issued at today’s visit.
