Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced and met with Licensee Birbahadur Moktan to conduct an annual inspection utilizing the inspection tool. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. LPA observed resident rooms, common area bathrooms, kitchen, common areas and perimeter of care facility. LPA observed rooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition and properly maintained. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two day perishable and (7) seven day non-perishable food supply on hand. Smoke detectors and carbon monoxide detectors are operational in the care home. Fire extinguishers and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed five (5) resident files, two (2) staff files and resident medications. Facility has a current copy of certificate of liability insurance and LPA obtained a copy. As a result of this visit, no deficiencies were cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. LPA Hiratsuka, conducted this unannounced annual visit. LPA toured the facility with Danny Moktan. There are five residents in care at this time. There are three private and one shared resident room on the left side of the main entrance to this facility. There is a licensee quarters located behind the utility room next to the kitchen. There is also a private resident room next to the kitchen that has an exit to a patio. There is a caregiver's quarters connected to the living room. There is one full common bathroom and one private bathroom in the shared room. The resident room in the front of the facility to the right of the entrance and the room between that one and the shared room have a door connecting the two rooms on the shared wall. The middle room has an exit to the outside. Precautions are taken for privacy when they are occupied; however, the connecting door cannot be blocked on either side per fire code because in case of emergency, the middle room is the exit for both rooms. The shared room has an exit to the outside. There are two patio areas that the facility has exits to. There is a wood deck in the back patio that has a ramp leading to the side of the facility. There are sheds in the backyard. The second patio is next to the dining room and private client room. Kitchen was inspected. There is a locked cabinet under the sink that stores soaps for the dishes and dishwasher. There is an ample perishable supply and the nonperishable supply is adequate. Medications are stored in a locked cabinet in the kitchen. Staff and resident records were reviewed during visit. Several topics were discussed during this visit. On 3/14/2024 LPA Tryon visited the facility to do an annual visit. LPA met with Danny Moktan. The facility currently has 2 residents. LPA toured the facility including common areas, kitchen, bedrooms, hallways, laundry, patio, yard, bathrooms. The facility appears to be clean and in good condition. Food supplies appear adequate to meet the requirement of 2 days perishable and 7 days non-perishable. Medications are centrally stored and locked. Cleaners and other potentially hazardous items are secured. No hazards were noted. Smoke detectors and carbon monoxide detector installed and functioning, fire extinguisher present and charged. Furniture, furnishings and plumbing in good and functional condition. There is a large patio out the back door with various chairs and tables for residents to spend time outside, all in good condition. LPA reviewed the CARE Tool with Administrator. LPA reviewed staff training, and reviewed 2 of 2 resident files. LPA was not able to interview a resident, as one resident was out with family and the other resident only communicates via sign language.
