Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Melissa Parks arrived on Thursday May 28, 2026, to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed resident (6) and staff (3) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training. LPA and Administrator Michael toured the facility together to ensure the health and safety of residents in care. The following areas were inspected: resident bedrooms, bathrooms, kitchen, garage and backyard. Facility was clean and well organized. Water temperatures were within the required range. Facility was current on fire drills. Facility had a fully stocked first aid kit. All required postings were observed. All chemicals and knives were kept locked and inaccessible to residents. LPA requested a copy of current liability insurance. Exit interview conducted. No deficiencies cited. Licensing Program Analyst (LPA) Graham Gunby arrived on Thursday June 5, 2025 to conduct the unannounced annual inspection. LPA met with Administrator, Michael Demyan, and explained the purpose of the visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA Gunby reviewed resident (5) and staff (5) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training. LPA and Admin toured the facility together to ensure the health and safety of clients in care. The areas toured included bedrooms, common areas, kitchen, bathrooms, laundry room, back yard and garage. All chemicals, toxins and sharps were kept locked and inaccessible to residents. Facility has one fire extinguisher (entry). In the areas toured, there were no health or safety violations observed. No deficiencies cited. Exit interview conducted. The areas toured included resident rooms, bathrooms, living room, kitchen, garage, and backyard/deck. In the areas toured, there were no health or safety violations observed. Nov 30, 2023Complaint investigation reportUnsubstantiated

Allegation investigated: Staff are preventing resident from having visitors Staff do not answer the facility phone

LPA Parks arrived on Thursday November 30, 2023, to conclude a complaint investigation regarding the above allegations. LPA met with Administrator Michael and explained the purpose of the visit. Throughout the course of the investigation, LPA interviewed R1-R5. Additionally, LPA interviewed POAs for R1-R3. R’1 POA revealed that they have been having a difficult time assimilating to the facility after moving-in. Per POA, prior to visitors, R1 was engaged, walking, and eating. After a visitor came to the facility, R1 began to refuse to eat and sleep. R1’s POA then asked for their visitors to refrain from visiting until R1 can maintain a healthy routine at the facility. LPA learned that R1 had a visitor when they were sleeping. Unsubstantiated The Administrator asked for the visitor to come back on a different day to allow R1 their rest. LPA interviewed R1-R3’s POAs who revealed that they have not had any difficulty in visiting the facility. Interviews revealed that they were given the gate code upon move-in. Additionally, interviews revealed that staff are responsive to phone calls and texts. The facility has one main line at the facility and two cell phones. All three numbers are given out to POAs, family, and friends of residents. Based on information obtained during the investigation, LPA finds the allegations to be UNSUBSTANTIATED- a finding that the complaint is unsubstantiated means that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred, Exit interview.
