Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced and met with Administrator Hakim Hazziez Jr. to conduct an annual inspection utilizing the inspection tool. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. LPA observed resident rooms, common area bathrooms, kitchen, common areas and perimeter of care home. LPA observed rooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition and properly maintained. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two day perishable and (7) seven day non-perishable food supply on hand. Smoke detectors and carbon monoxide detectors are operational in the care home. Fire extinguishers and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed resident files, staff files and resident medications. Facility has a current copy of certificate of liability insurance and LPA obtained a copy. As a result of this visit, no deficiencies were cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. Allegation investigated: Staff members consumed alcohol during working hours, impairing their ability to provide adequate care and supervision, which presents a risk to residents in care.

On 8/4/26, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Administrator Danielle Hazziez. Allegation- Staff members consumed alcohol during working hours, impairing their ability to provide adequate care and supervision, which presents a risk to residents in care . UNSUBSTANTIATED Based on documents reviewed and interviews conducted with staff members, the Department found insufficient evidence to support the allegation. Staff stated that consuming alcohol while on duty is strictly prohibited under the facility’s employee handbook and policies. Staff further reported that personal social media use is only permitted outside of working hours in accordance with the facility’s social media policy. No evidence was obtained during the investigation indicating that staff consumed alcohol while on duty or that resident care and supervision were impaired as a result. No evidence were found that this incident occurred at facility premises or when staff were on duty. Therefore, the allegation is unsubstantiated. A finding that the complaint is UNSUBSTANTIATED means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. On 8/4/26, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Administrator Danielle Hazziez. Allegations- Staff made inappropriate comments towards residents Staff scolded residents. Unsubstantiated Based on interviews conducted, the Department found insufficient evidence to support the allegations. Staff consistently stated they treat residents with kindness, compassion, dignity, and respect, and consider residents to be like family. No evidence was obtained during the investigation to substantiate that staff scolded residents. Staff further reported that residents’ family members are actively involved in their care and frequently present in the facility. Therefore, the allegation is unsubstantiated. A finding that the complaint is UNSUBSTANTIATED means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Allegation investigated: Staff did not safeguard resident's personal items Staff did not issue refund to resident's authorized representative Staff modified resident's admission agreement without notifying resident's authorized representative

On September 18, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived at the facility to deliver the findings of the allegations. LPA met with and explained the purpose of the visit. During the course of the investigation, LPA reviewed staff records, facility records, and conducted interviews. LPA finds that facility met Tittle 22 requirements For the result of the allegations: Staff did not safeguard resident's personal items; Staff did not issue refund to resident's authorized representative; and Staff modified resident's admission agreement without notifying resident's authorized representative; please continue on LIC 9099-C. Unfounded LIC 9099-C Allegation: Staff did not safeguard resident's personal items Based on the interview conducted with the Administrator, it revealed that the resident (R1) was not able to gather all personal belongings during the time of moving out. However, on February 14, 2025, R1's responsible party arrived at the facility to gather R1’s belongings including the painting set. The interview conducted with R1’s responsible party revealed that all of R1’s personal belongings were gathered and retrieved. No indication of R1 having any missing items. Allegation: Staff did not issue refund to resident's authorized representative Interview conducted with Administrator revealed during the time out R1 moving out on January 24, 2025, R1's responsible party failed to take all of R1's personal belongings. Remaining was approximately R1's shaver set, battery charger, a foldable walker, a set of bed sheets and three painting canvases. Text messages provided revealed that responsible party agreed to grab the remaining of the belongings "next week" however, home security video revealed personal belongings were not retrieved until three weeks later on February 14, 2025. Admission agreement signed by R1's responsible party on September 2, 2024 revealed that it was agreed basic rate will continue to accrue until all personal belongings are removed from the facility. Allegation: Staff modified resident's admission agreement without notifying resident's authorized representative Based on file review of responsible party's agreement agreement and facility's admission agreement, LPA has determined they are the same admission agreement with no alteration. With the information provided above, LPA has determined the allegations are unfounded. Unfounded meaning that the allegations were false, could not have happened and/or is without a reasonable basis. Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with co-Administrator and explained the purpose of the visit. LPA and co-Administrator conducted a tour of the interior and exterior of the facility to ensure the health and safety of residents in care. Areas toured included but not limited to: residents rooms, bathroom, kitchen, backyard and the common areas. LPA observed facility to have at least two days of perishable and seven days of nonperishable foods. LPA observed sharps to be locked and secured. LPA observed medications to be locked and secured. LPA observed fire extinguisher present with service date of August 25, 2025. LPA observed facility to be clean, safe, sanitary and in good repair. LPA and co-Administrator discussed no concerns at the facility at this time. LPA observed Emergency And Disaster Plan For Residential Care Facilities For The Elderly LIC610E, document recently reviewed by Administrator on June 16, 2025. LPA conducted a file review for six residents records via facility laptop, and confirmed the required documents were present. CARE tool was completed and found facility to be in compliance. On 7/25/2024, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the care tool. LPA met with Administrator, Danielle Hazziez, and explained the purpose of the visit. Facility is licensed for six non-ambulatory residents, hospice waiver of three. Facility currently has four residents in care . LPA provided facility an updated LIC 203A. LPA and Administrator conducted a tour of the interior and exterior of the facility to ensure the health and safety of residents in care. Areas toured included but not limited to: three residents rooms, staff room, kitchen, backyard and the common areas. LPA observed chemicals to be locked in cabinet. LPA observed facility to have 2+ days of perishable and 7+ days of nonperishable foods. LPA observed sharps to be locked and secured. LPA observed facility to be clean, safe, sanitary and in good repair. LPA and Administrator discussed no staffing concerns at the facility at this time. LPA observed Emergency And Disaster Plan For Residential Care Facilities For The Elderly LIC610E, document was reviewed by Administrator on 06/13/2024. LPA conducted a file review for three residents in care. LPA was informed current staff working at the facility are Administrator and co-Administrator. Today's visit, LPA obtained a copy of LIC 500, LIC 308, LIC 309, LIC 610E and Liability Insurance Result of today's visit, no deficiencies observed.
