Type of visit: Required - 1 Year

Licensing Program Analyst (LPAs) J. Duarte and J. Leffall arrived unannounced to conduct an Annual inspection. LPA was greeted by staff and staff contacted Administrator Diana Diaz. The administrator arrived shortly after. A tour was conducted with staff. The facility was observed to be clean and the temperature was set at 75 degrees F. Common areas were furnished well with adequate seating and lighting available. The kitchen was toured and it appeared clean and safe for food preparation. LPA observed a two-day supply of perishable and a seven-day supply of non-perishable food. The sharps were observed locked in a kitchen cabinet. Chemicals were in a locked cabinet under the kitchen sink. LPA toured the resident bedrooms. The bedrooms had all required furnishings to include chair, dresser, and lighting. Beds were observed to have clean linen. LPA observed an adequate supply of clean linen in the hallway closet. The hot waster in the restrooms measured at 117 degrees F. Restrooms were observed to have grab bars and slip resistant mats in the showers. The facility has a washer and dryer. Detergent to wash is stored in a locked cabinet. Medication is stored in a locked hallway closet. A first aid kit with all required items was observed stored in the medication closet. Exterior tour conducted, all exits open and were free of obstructions. The back has a patio with seating available. Continued on LIC809-C. Continued from LIC809 A fire extinguisher was observed with a service date 05/23/2025. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. The last fire drill was conducted was 01/05/2026, per staff records. LPA reviewed client records, medications and staff records. The MARs reflected medication is being administered as prescribed. A deficiency was cited, see LIC 809-D. A plan of corrections was developed and a copy of this report and appeal rights were provided. Type of visit: Required - 1 Year

On 04/14/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct initial complaint investigation. LPA introduced self, stated the purpose of the visit, and met with caregiver Guadalupe Pena. Administrator Diana Diaz was called and arrived shortly. All three residents were present during visit. LPA toured facility with Administrator. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Fire extinguisher was observed with a service date: 06/10/24. Last fire drill completed on 03/07/25. An adequate supply of perishable and non-perishable food was observed. Refrigerator temperature is maintained at 39 degrees F and freezer at -5 degrees F. Washer and dryer observed operational and functional during visit. Cleaning supplies and chemicals stored and locked under kitchen sink. Knives stored and locked in kitchen drawer. Extra linens were observed in hall closet. Medications were observed locked in hall closet. MARs were reviewed and medications were checked. All bedrooms were observed to have the required furnishings and with adequate lighting. All bathrooms were toured and observed operational during inspection. Non-skid strips and grabbed bars were observed. Hot water temperature was tested at 118.7 degrees F in bathroom and 118. 6 degrees F in master bathroom. Outside of facility toured and observed to be free of debris. Adequate outdoor seatings available for residents. All residents’ and a sample of staff files were reviewed. Smoke detectors and carbon monoxide were observed operational during visit. A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 04/21/25. The following updated forms were requested: Lic 308, Lic 500, Lic 610E, current Administrator certificate, and current liability insurance. During the visit, LPA observed that Resident (R1) has been moved to this facility. R1 was admitted 7/26/24 to a private room with sliding doors which exit to the patio. LPA interviewed R1 and facility staff. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Licensee, Jason Johnson, Continual Administrator's Certification for Jaimy Johnson expires 11/16/2025. There are currently 4 residents who reside at this home and there is 1 resident on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested at 105 degrees. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. Resident 1's Pr - Admission appraisal is not signed or dated by Licensee. Resident 1's Physician Report does not have diagnosis. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Allegation investigated: Facility is refusing to accept a resident back after hospitalization. Facility did not effectively communicate with a resident's medical provider.

Licensing Program Analyst (LPA) Darius Williams conducted an unannounced follow up visit. LPA met with House Manager Diana Diaz, House Manager Liz Ramos, and discussed the purpose of the visit. LPA has conducted interviews and record reviews. Facility provided an order to the LPA fromResident 1’s (R1) physician stating a Skilled Nursing Facility (SNF) was needed. The Administrator and Staff 1(S1) reported they informed the hospital regarding the physicians order, but did not provide the note nor did the hospital request the note. On 12/4/2023, Hospital Worker 1 (HW1) reported they were going to place R1 into a Skilled Nursing Facility; at this the hospital was not in possession of the physician order for a SNF. LPA requested additional info which was not provided. *Continued on LIC 9099-C* Unsubstantiated On 12/12/2023, LPA Williams attempted to reach out to HW1 again for clarification via phone and e-mail with no success. In regards to the allegation, facility did not effectively communicate with a resident's medical provider,according to HW1, on 11/28/2023 a nurse contacted the facility and spoke to (S1). S1 verified speaking to a nurse from the hospital. HW1 reported attempting to contact the facility at the following number, 661-972-6468, which is a phone number to an individual not associated to the facility. HW 1 reported attempting to contact the facility through phone numbers found online with no success. The Administrator reported on 12/7/2023, that he reached out to the hospital and reported R1 is being placed into a SNF. Although the allegations, facility is refusing to accept a resident back after hospitalization and facility did not effectively communicate with a resident's medical provider. may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.
