On 08/15/2026, Licensing Program Analysts (LPAs) J. Duarte and M. Vega, arrived unannounced to conduct the required Annual inspection. LPAs introduced self, stated the purpose of the visit, and met with staff. Staff contacted Administrator (AD) Diana Diaz and she arrived shortly after. LPA toured the facility with staff and AD. Six residents were present during visit. The facility was observed to be at a temperature of 72 degrees F, clean, with no passageway obstructions. The living room has sufficient seating for residents. Living room has a fireplace with a cover. The kitchen was toured and LPA observed an adequate supply of perishable and non-perishable food. The facility stores sharps in a locked kitchen cabinet. A fire extinguisher was observed in the kitchen, with a service date of 05/28/26. All bedrooms were toured and observed to have the required furniture and adequate lighting. Bedroom are dual occupancy. Bathrooms were observed operational, with non-skid strips and grabbed bars observed for the showers and toilets. The hallway restroom hot water measured at 115 degrees F and the hot water to the restroom connected to the room measured at 118 degrees F. Extra linens and towels observed stored in a hall closet. Chemicals were observed stored and locked in hallway closet. The facility has a dryer and washer in the garage. The facility has an additional freezer and refrigerator in the garage with an additional food supply. Outside of the facility toured and observed to be free of debris with adequate seating available for residents. The carbon monoxide/smoke detector was observed operational during inspection. Medications observed kept locked in facility office. Medications were reviewed along with MARS and observed to be administered as prescribed. A first aid kit is also stored in the office. A sample of resident files and staff files were also reviewed and observed to have required documentation. Continued in LIC 809C. Continued from LIC 809. LPAs met with Administrator Diana Diaz and informed her the purpose of the visit. On 08/18/25, Licensing Program Analyst (LPA) J. Duarte arrived unannounced to conduct the required Annual inspection. LPA introduced self, stated the purpose of the visit, and met with staff Brenda Rodriguez. Staff contacted Administrator (AD) Diana Diaz and she arrived shortly after. LPA toured the facility with AD. The facility was observed to be at a temperature of 72 degrees F, clean, in good repair, and no passageway obstructions were observed. The living room has sufficient seating for residents. Living room has a fireplace with a cover. The kitchen was toured and LPA observed an adequate supply of perishable and non-perishable food. The facility stores kitchen knives in a locked hallway closet. A fire extinguisher was observed in the kitchen, with a service date of 05/23/25. The kitchen hot water measured at 120.7 degrees F. All bedrooms were toured and observed to have the required furniture and adequate lighting. Bathrooms were observed operational, with non-skid strips and grabbed bars observed for the showers and toilets. The hallway restroom hot water measured at 123.4 degrees F and the hot water to the restroom connected to the room measured at 121.6 degrees F. Extra linens and towels observed stored in a hall closet. Chemicals were observed stored and locked in the garage. The facility has a dryer and washer in the garage. The facility has an additional garage and freezer in the garage with an additional food supply. Outside of the facility toured and observed to be free of debris with adequate seating available for residents. The carbon monoxide/smoke detector was observed operational during inspection. Medications observed kept locked in facility office. Medications were reviewed along with MARS and observed to be administered as prescribed. A first aid kit is also stored in the office. The facility installed a door in one of the bedrooms and did not notify or submit proof to Community Care Licensing (CCL). Facility is to provide documentation and proof to CCL. Type of visit: Case Management - Health Checks

On 02/20/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct case management visit for the purpose of checking on the health and safety of the residents in care. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with caregiver Audie Green. Caregiver and LPA was unable to reach Administrator. Manager Elizabeth Ramos was called and arrived shortly and stated Administrator is unable to attend visit. LPA tour facility. R1’s Admission agreement, Needs and Services Plan, Physician Report (Lic 602A), Care plan, Charting, Hospital Discharge record, medical appointment notes, Progress notes, and Incident Reports is requested to be submitted to the department by 02/21/25. Allegation investigated: Questionable Death Staff did not have CPR training resulting in an improper medical intervention Staff did not seek timely medical assistance for resident

On 10/28/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver complaint findings on the above allegation. LPA introduced self, stated the purpose of the visit and met with Supervisor Diana Diaz. During the course of the investigation, the Department conducted interviews and reviewed records. Based on records reviewed and interviews conducted, there was insufficient evidence to prove or disprove that the death of R1 was questionable. CPR was performed and 911 was called when R1 was choking. S1 have current CPR training completed. Based on record reviewed and interviews which were conducted, the preponderance of evidence standard has not been met, therefore the above allegations are found to be UNSUBSTANTIATED. On 08/13/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct required Annual inspection. LPA introduce self, stated the purpose of the visit, and met with staff Adriana Hernandez. House supervisor Elizabeth Ramos and House Manager Diana Diaz was called and arrived shortly. House Supervisor called Administrator Jaimy Johnson who stated unable to attend meeting. LPA toured facility with House Supervisor. Four residents observed during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. Fire extinguisher was observed with a service date of: 6/10/24. Temperature maintained for refrigerator at 37 degrees F and freezer at 0 degrees F. All bedrooms were toured and observed to be required furniture and adequate lighting. Bathrooms were observed operational. Non-skid strips and grabbed bars observed in the bathroom. Toilet observed functional and operational. Extra linens and towels observed. Hot water temperature was tested 112.6 degrees F in bathroom 1 and 113.9 degrees F in shared bathroom. Chemicals were observed stored and locked in garage. Dryer and washer observed operating during inspection. Outside of facility toured and observed to be free of debris. Adequate outside seatings observed available for resident. Carbon monoxide and smoke detector observed operational during inspection. Medications observed kept locked in facility office.
