On 08/18/2026, Licensing Program Analysts (LPA) J. Duarte and M. Vega arrived unannounced to conduct the required Annual inspection. LPAs introduced selves, stated the purpose of the visit, and met with staff. Staff contacted Administrator (AD) Diana Diaz and she arrived shortly after. LPAs toured the facility with AD. Staff stated that there are three residents; one resident is in the facility, one resident is in the hospital, and one resident is at the clinic. The facility was observed to be at a temperature of 73 degrees F, clean, in good repair, and no passageway obstructions were observed. The living room had sufficient seating for residents. The kitchen was toured and LPA observed a two day supply of perishable and seven day supply non-perishable food. The facility stores sharps in a locked kitchen cabinet. Chemicals were observed stored and locked in the cabinet under the kitchen sink. A fire extinguisher was observed in the kitchen, with a service date of 05/28/26. The dining room has a fireplace with a cover. Bedrooms were toured and observed to have the required furniture and adequate lighting. Bathrooms were observed operational, with non-skid strips and grabbed bars were observed. The hallway restroom hot water measured at 110 degrees F and the hot water to the restroom connected to the room measured at 111 degrees F. Based on observation, three out of four oxygen tanks in a resident room were not secured in a stand or to the wall. The facility has a dryer and washer in the hallway. Extra linens and towels observed stored in a hall closet. The back patio was toured and observed to be free of debris with adequate seating available for residents. Medications observed kept locked in a hallway closet. A first aid kit is also stored in the medication closet. All resident files and a sample staff files were reviewed and observed to have required documentation. On 08/05/2026, per staff, R1 was sent to the hospital due to sustaining an injury. There is no record of the facility reporting the incident to CCLD. Medications and MAR log were reviewed. Based on observation, one resident had PRN medication transferred and pre-dispensed into a different container other than its originally received container that does not have the pharmacist label. Allegation investigated: Medications are not dispensed as prescribed. Residents are not changed or bathed with privacy.

On 10/27/2025, Licensing Program Analysts (LPAs) J. Duarte and M. Medina conducted an unannounced initial 10-day complaint visit. LPAs introduced self, stated purpose of visit, and allowed entrance by Administrator Elizabeth Ramos. Two residents were present at the facility and staff stated that one bed is on hold. LPAs toured facility, reviewed resident files, and conducted interviews during complaint visit. Two out of two residents stated that they are not given privacy when being changed or bathed, residents stated that door is often left open when staff are providing care to them. While conducting investigation and touring the facility, LPAs observed staff dressing R1 and preparing to transfer them to their wheelchair with bedroom door open. Regarding the allegation, medication are not dispensed as prescribed, R1 stated during interview that there have been several times that staff have provided R1 with medication to drink directly from the medication bottle and not administered as directed by the physician. The preponderance of evidence standard has been met; therefore, the above allegations are found to be Substantiated. LPA introduced self, stated the purpose of the visit, and met with staff Shirley Ralda. LPA contacted licensee Jason and he stated that he will send someone to conduct a tour with LPA. Administrator (AD) Diana Diaz arrived shortly later. LPA toured the facility with AD. The facility was observed to be at a temperature of 73 degrees F, clean, in good repair, and no passageway obstructions were observed. The living room had sufficient seating for residents. The kitchen was toured and LPA observed an adequate supply of perishable and non-perishable food. The facility stores kitchen knives in a locked kitchen cabinet. Chemicals were observed stored and locked in the cabinet under the kitchen sink. A fire extinguisher was observed in the kitchen, with a service date of 05/23/25. The dining room has a fireplace with a cover. Bedrooms were toured and observed to have the required furniture and adequate lighting. Bathrooms were observed operational, with non-skid strips and grabbed bars were observed. The hallway restroom hot water measured at 113.4 degrees F and the hot water to the restroom connected to the room measured at 110.9 degrees F. The facility installed a sliding glass door in one of the bedrooms and did not notify or submit proof to Community Care Licensing (CCL) of the alteration. LPA observed R1's bed rail to be extended to more than half the length of the bed. Licensee did not ensure a restricted health care plan for R3's restricted health condition. On 08/21/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct required Annual inspection. LPA introduce self, stated the purpose of the visit, and met with staff Catalina Aquino. Administrator Jason Johnson and House manager Diana Diaz was called and arrived shortly. LPA toured facility with Administrator and House Manager. All three residents observed during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. Fire extinguisher was observed with a service date of: 07/10/24. Last fire drill completed on 08/14/24. Chemicals were observed stored and unlocked under kitchen sink. Temperature maintained for refrigerator at 38 degrees F and freezer at -6 degrees F. All bedrooms were toured and observed to be required furniture and adequate lighting. Medications observed locked in hall closet. Dryer functioning and operating during inspection Bedroom was observed furnished and good lighting. Bathrooms were toured. Toilet observed functional and operational. Non-skid strips and grabbed bars was observed. Hot water temperature was tested 105.4 degrees F in bathroom hall bathroom and 105.1 degrees F in shared bathroom. Outside of facility toured and observed to be free of debris. Side gate self-latching and self-closing. Adequate outside seatings observed available for resident. Licensing Program Analyst (LPA) Darius Williams conducted an annual inspection visit. LPA met with Administrator, Jason Johnson and discussed the purpose of the visit. LPA Williams toured the facility with the Administrator. LPA observed the living room to have seating available for four residents and space to move around. Facility thermometer reflected 72 degrees Fahrenheit (F). LPA observed two bedrooms. Rooms had space for residents to move around. The rooms had a bed with required linens, chair, night stand, and dresser. Two bathrooms had grab bars and slip resistant strips placed in the bathroom/shower floor. The kitchen was clean and in good repair. LPA observed 2 days of perishable food and seven days of non perishable food. Kitchen refrigerator reflected approximately 34 degrees Fahrenheit (F) and the freezer 0 degress F. Chemicals and medications were observed locked and inaccessible to residents. LPA observed smoke detectors to be present and operational. First aid kit was present with all required items. *Continued on LIC 809C* The backyard had seating and space to accommodate residents. There is no pool on the premises. LPA reviewed 3 resident files and 3 employee files, which had all items requested. No deficiencies were cited during the visit.
