Allegation investigated: Resident sustained multiple bruises due to staff neglect.

On 03/09/2026 at 10:30am, Licensing Program Analyst (LPA) Zina Brown conducted an initial visit at this facility to conduct complaint investigation and deliver the findings for the allegations above. During today’s visit, LPA met with Gregg MacEllven (Administrator) & Tristan Nielsen (House Manager) and explained the purpose of the visit. The investigation consisted of the following: On 03/09/2026 between the hours of 10:30am - 11:00am, LPA interviewed Administrator (A1). Also, LPA requested and obtained copies of the resident roster (dated 01/21/2026). The investigation revealed the following: Allegation: Resident sustained multiple bruises due to staff neglect. It was alleged that staff at where the resident resides is starting to get "rough" with the resident. Resident also states "this is where all my bruises are coming from". Resident noted to have generalized bruising to arms, back and hip,” Unfounded On 03/09/2026 between the hours of 10:45am - 11:00am, LPA interviewed A1 regarding the allegation. A1 did not confirm nor deny the allegation and stated R1 does not reside at Ocean Breeze Care Home LLC (Facility #198602397), located at 911 S Weymouth Ave, San Pedro,CA 90732. A1 mentioned R1 actually resides at Ocean Breeze Care II (Facility# 198320059) located at 26509 Rolling Vista Drive, Lomita, CA 90717. A1 indicated R1's family member admitted R1 to the hospital on 02/27/2026 for a urinary tract infection (UTI). On 03/09/2026 between hours of 11:20am - 11:25am, LPA conducted a records review and observed the following: LIC 9020A Register of Facility Residents - Residential Care Facilities for the Elderly (dated 01/21/2026) list six (6) residents reside at this facility. Of the six (6) residents, R1 is not listed as a resident at Ocean Breeze Care Home (Facility #198602397), LLC located at 911 S Weymouth Ave, San Pedro, CA 90732. Unfounded: This agency has investigated the complaint alleging (for the allegation above). We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without reasonable basis. We have therefore dismissed the complaint. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced visit to the above facility. The purpose of today’s visit was to conduct the one-year inspection. LPA met with Gregg MacEllven , Administrator and the purpose of the visit was discussed. Facility is licensed to serve 6 non- ambulatory residents age 60 and over . The facility has an approved hospice waiver for 2 residents. One resident is diagnosed with dementia and 3 residents are receiving home health, There are No residents receiving hospice care services. The facility does not handle any of the residents’ money. This home is a single story home consisting of: (6) resident bedrooms, (5) bathrooms, outside den/covered patio, living room, kitchen with dining area, laundry room (located in the attached garage)and 1 staff room. LPA toured the Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place, water temperature measured between 115 F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards; doorways were free of obstructions. Kitchen was checked and observed to be within Title 22 regulations. Perishable and non-perishable food supply was checked. All cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. Smoke detectors were working properly and fire extinguisher was fully charged. Carbon monoxide detector was operational. First Aid kit was available. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. During todays visit LPA did not observe any deficiencies. Exit interview conducted with Gregg MacEllven Administrator. Type of visit: Required - 1 Year

On 2/21/2025, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Gregg MacEllven/Licensee. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) elderly adults ages 60 and above, of which (6) can be non-ambulatory. The facility has an approved hospice waiver for (2). Currently the facility has (6) residents. The facility is a split-level structure located in a residential neighborhood. It consists of five (5) client bedrooms, one (1) staff room, two (2) full bathrooms, three (3) half-bathrooms, shaded back yard, front yard, laundry room and attached 2 car garage. LPA Iniguez and the Administrator toured the physical plant. There were no bodies of water or obstructions on the premises. LPA inspected a total of (4) bedrooms and (3) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged from 105.0°F to 120°F, and the room temperature ranged from 76°F to 78°F. The evaluation Report continues on the next page, LIC 809-C, providing further details of the inspection findings. During the visit, LPA Iniguez observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored in a way that made them inaccessible to residents in care. The kitchen was inspected, and there was sufficient perishable and non-perishable food available, which was adequately maintained. All fire extinguishers were charged and operable. The last Fire/Disaster Drills were conducted on 12/5/24. A review of (3) residents' service files and (3) staff personnel files was maintained in order. LPA reviewed (3) Medication Administration Records (MARs) and found no discrepancies. LPA observed the facility's infection control practices. All mandated inspection control posters were displayed throughout the facility. A copy of liability insurance will be email to LPA. Facility Annual Fess current. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did not observe deficiencies during this visit; therefore, no citations were issued. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) residents ages 60 and above, (6) non-ambulatory. Approved hospice waiver for (2). LPA Iniguez toured the physical plant with an administrator. There were no bodies of water or obstructions on the premises. A total of (4) rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident’s personal belongings was observed. Bathrooms were found to be within Title 22 regulations and were operational. LPA inspected the carbon monoxide detectors combo were in operable condition. The water temperature properly measured between: 105°F-120°F: Kitchen 109.1°F, Bathroom #1:108.7°F, Bathroom #2:109.1°F. Evaluation Report Continues LIC 809-C LPA Iniguez observed the facility to be clean, sanitary, and appropriately furnished at the time of the visit. Storage areas for personal hygiene, sharps objects were stored and not accessible to residents. The kitchen was inspected and there is sufficient perishable and non-perishable food available at the properly. All fire extinguishers were charged and were operable. A review of (3) residents' service files, (3) staff personnel files were reviewed. LPA checked (3) Medication Administration Records (MAR) and no discrepancies were found. The first AID kit was checked. Last facility disaster drill was: 11/7/2023. LPA observed the facility's infection control practices. A copy of the liability insurance was given to LPA during the visit. Deficiency cited under California Code of Regulations, Title 22, Division 6, Chapter 8.
