Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct the required annual visit today at 09:45am. Upon arrival, the LPA was greeted by staff who then contacted the Administrator, Maaya Marri and informed them of the visit. The Administrator arrived at approximately 10:30am and the reason for the visit was explained. Entrance interview conducted. Beginning at 10:30am, the LPA along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards, and facility is in compliance with Title 22 Regulations. The following was observed: Kitchen: The LPA inspected the kitchen/food service area at approximately 10:30am. Knives and sharps were observed in a kitchen drawer locked and inaccessible at the time of the visit. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. Cleaning supplies were observed under the kitchen sink inaccessible to residents at the time of the visit. Common Areas: This includes the living room and dining room. Furniture in the common areas was observed to be in good condition at the time of the visit. The facility maintained a comfortable temperature. Garage: The garage was inaccessible to residents in care at the time of the visit. Washer and dryer were observed inside the garage. Detergents and toxins were observed locked at the time of the visit. The LPA observed an adequate amount of emergency food and water. Restrooms: There is one (1) restroom for residents’ use. Bathroom was clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathroom was sufficiently stocked with supplies and paper towels. Starting at 10:36am, the hot water temperature was measured in bathroom, and it measured within the required range of 105 – 120 degrees Fahrenheit. Bedrooms: There are five (5) bedrooms for residents’ use. Four (4) bedrooms are designated as private / single occupancy, and one (1) bedroom is designated as shared / double occupancy. Bedrooms were observed to be furnished appropriately with appropriate furnishings, and sufficient lighting. Additional clean linens and towels were observed in a closet by the main hallway. Staff bedroom observed on premises locked and inaccessible at the time of the visit. Backyard / Outdoors: The backyard has a covered patio area with patio furniture for residents’ use. The LPA observed two (2) locked sheds used for storage purposes. There is one (1) side gate with latching mechanisms. All passageways were observed to be clear of any obstructions. No bodies of water noted at the time of the visit. Records: Record review began at approximately 10:45am Six (6) resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, preplacement appraisal, consent for treatment form, and updated/current appraisal & needs and services plan. Four (4) personnel files including the current Administrator’s file were reviewed for, but not limited to: personnel records, health assessments with negative TB test results, criminal record clearances, first aid/CPR training, and the appropriate yearly training. All personnel files were complete. The Administrator’s Certificate is valid until 03/17/2028. Emergency Disaster Planning: During today’s visit, the LPA reviewed the facility's emergency disaster plan, which was observed to be complete and recently reviewed/updated. Emergency disaster drills conducted quarterly as per regulation; last fire drill conducted on 03/08/2026. Medications: Medications review began at approximately 12:30pm. Medications are centrally stored and kept in a locked cabinet adjacent to the living room. All medications including PRNs were labeled, stored, and locked inaccessible to residents in care. Medications appear to be administered as prescribed at the time of the visit. No citations issued. Exit interview conducted. Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct a post licensing visit. Upon arrival, the LPA was greeted by staff who then contacted the Administrator telephonically. The Administrator, Maaya Marri arrived at approx. 11:00 a.m., and the reason for the visit was explained. Entrance interview. Beginning at 11:05 a.m., LPA and Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. The kitchen/food service area was observed at approx. 11:25 a.m. Knives and sharps were observed in a locked drawer. Cleaning supplies and other chemicals are kept locked under the sink area. Kitchen appliances were in operable condition. The facility has a sufficient supply of two (2) day perishable and seven (7) day non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. There are five (5) bedrooms in the facility for resident use; four (4) bedrooms are designated as private, single occupancy rooms, and (1) bedroom is designated as a double, shared occupancy room. All passageways were observed to be clear of obstructions. All resident rooms were observed to be furnished appropriately with linens, appropriate furnishings, and sufficient lighting. Additional clean linens and towels are available for resident use in a hallway closet. There are two (2) restrooms in the facility. Restrooms were observed to be clean and in sanitary condition. Grab bars and slip resistant floor were observed. Restrooms were sufficiently stocked with supplies and paper towels. Report Continued on LIC 809C... Report Continued from LIC 809... The hot water temperature was measured starting at 11:18 a.m., and they measured outside of the required range of 105 – 120 degrees Fahrenheit. The Administrator adjusted the water temperature during today’s inspection. The LPA observed the garage which was inaccessible to residents at the time of the visit. There is a washer and dryer inside the garage. Toxins and detergents were observed locked and inaccessible at the time of the visit. A sufficient supply of emergency food and water was observed. The LPA observed the common area. This includes the living room and a dining room. At the time of the visit, furniture in the common areas was observed to be in good condition. The facility maintained a comfortable temperature. The fire extinguisher was observed to be new with a purchase date of 08/27/2025. LPA observed required postings throughout the common space. Activities were observed in the common areas. There is a functioning telephone on the premises. Fireplace was observed to be adequately covered at the time of the visit. The backyard has a covered patio area with shade, patio furniture including tables and chairs for resident use. All passageways were observed to be clear. LPA observed one (1) self-latching gate. No bodies of water noted at the time of the visit. Record review began at approx. 11:40 a.m. Six (6) resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, LIC627(c) Consent for Treatment form, PRN authorization letters, and current needs and services plan. Files were complete. Three (3) personnel files including the Administrator’s file were reviewed for, but not limited to: personnel records, health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. The Administrator’s Certificate is valid until March 2027. At approx. 12:40 p.m., LPA reviewed Guardian and observed that Staff #1 (S1) has started the criminal record clearance, but it is still in process and Staff #2 (S2) does not show a criminal record clearance on file. Interviews revealed that S1 and S2 have been working at the facility for one (1) day. S1 and S2 left during today’s inspection. Report Continued on LIC 809C... Report Continued from LIC 809C... During today’s visit, LPA reviewed the facility’s emergency disaster plan. Emergency disaster drill are being conducted quarterly as per regulation; last one conducted on 09/01/2025. Medications are centrally stored in a cabinet adjacent to the living room. Medications appear to be administered as prescribed at the time of the visit. Licensing Program Analyst (LPA), Martha Arroyo conducted a pre-licensing visit to this property. Upon arrival, the LPA met with Applicant Representative Maaya Marri as this is a change of ownership application from Manra Mansion LLC #565850331 to myMDcare Senior Living #565850596. The applicant has obtained fire clearance for a total capacity of six (6) residents, five (5) non-ambulatory residents and one (1) bedridden resident. The facility has a dementia program in place. The facility is one story. At approximately 10:10am, a physical plant tour was conducted inside and out. The LPA inspected facility for Fire Safety, Personal Accommodations and Services, and Food Service. The facility has four (4) private resident bedrooms and one (1) shared resident bedroom. Resident room #2 is approved for single bedridden resident and has a direct exit to the outside. There are no fire sprinklers in the facility. Main hallway leading to resident bedrooms from kitchen was observed to be equipped with a fire door. The smoke alarms and carbon monoxide detectors are wired and function properly when tested. The LPA observed one (1) fire extinguisher to be new with a purchase date of 08/06/2024. Each bedroom is equipped with a mattresses, pillows, and bedding. Bedrooms have sufficient lighting. There is a closet in the hallway with a sufficient supply of clean linens and towels. The facility has two (2) bathrooms for resident use. Resident bathrooms contained appropriate non-skid mats and grab bars. Bathrooms have sufficient paper products. Report Continued on LIC 809C... Report Continued from LIC 809C... Medications are stored and locked in a file cabinet by the office adjacent to the dining room. Facility records and First aid kit are stored by the office. First aid kit was observed to have bandages, thermometer, scissors, tweezers and a current first aid manual. Facility has an adequate supply of Personal Protection Equipment (PPE). Hot water temperature in both bathrooms measured between 105- and 120 degrees Fahrenheit. The LPA observed trash cans with tight fitting lids at the time of the visit. The facility has at least seven (7) day supply of non-perishable food and two (2) days perishable food. The kitchen has a sufficient supply of plates, cups, cookware, and utensils. There are no pesticides (poisons) or toxins stored in any food storage area or preparation area with utensils. Kitchen knives and sharps were observed in a locked drawer inaccessible to residents in care. Appliances in the kitchen were clean and all appeared functional at the time of the visit. Trash cans had tight fitting lids. No flies or other vermin were observed. The common areas were appropriately furnished, and the lighting was adequate. All window screens and coverings are in good repair. Enough seating for six (6) residents at the same time in the dining room table. A working telephone is present. There is a television and other entertainment equipment, games and/or activity supplies in the living room. There was sufficient space to accommodate both indoor and outdoor activities. Night lights were maintained in hallways and bathrooms. There is a fireplace in the living room that is non-operable at this time. It is adequately screened and there are no tools. All doors have functioning auditory alarms at the time of the visit. The facility has required postings, including emergency exit plan, Licensing Complaint Poster, Resident Personal Rights, Theft and Loss Policy, and Resident Council Rights. Report Continued on LIC 809C... Report Continued from LIC 809C... The garage is attached to the house and inaccessible to residents in care. The washer and dryer was observed inside the garage. Resident’s personal hygiene items were observed locked and inaccessible. There will be no firearms/ammunition stored on the property. There is a sufficient supply of emergency food and water. The exterior passageways were clean and clear of any obstructions. There is one (1) self-latching gate for emergency use. The LPA observed the backyard, which has a covered outdoor area and appropriate furniture for outdoor use. The outside area was observed to be large enough to conduct outdoor activities. There are no bodies of water noted at the time of the visit. The physical plant of this facility location is in compliance with Title 22 regulations at this time. During today's visit, the LPA completed Component III with the Applicant Representative. Component II was completed on 03/18/2025. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect approval of your license. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
