On September 11, 2026, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced to conduct the Required Annual Inspection and met with Administrator, Helen Ramirez. Facility file review was conducted at the Regional Office, and additional records were requested and reviewed on site. The facility is licensed for six Adults and is currently operating at a capacity of four adults. For a facility type (740). Licensed for age 60 and over, and six hospice. LPA Mixson toured the facility along with Helen Ramirez and made observations pertaining to the annual visit. LPA inspected the facility inside and outside there were no obstructions or debris to the indoor or outdoor passageways at the time of this visit. Additionally, there were no bodies of water on the premises. The facility is comprised of a single story cottage type facility. Located at 29533 Mactan Road Valley Center, CA 92082. Facility phone number is (442) 278-1004, and it is operable. There is a gate code 9531#. LPA conducted a tour of the interior and exterior areas of the facility. Facility has three shared resident bedrooms, two full bathrooms, and a laundry room. LPA observed the dining area, kitchen space, and family room. The backyard, and front of the facility were free of clutter and debris at the time of this visit. LPA Mixson observed the facility to be clean and free of unpleasant odors and debris. Kitchen was observed to be clean, the dishes and eating utensils were in good condition and stored safely at the time of this visit. LPA observed that sharps, chemicals, and other hazardous items were locked and inaccessible at the time of this visit. Meals are prepared and served on site and seven day supply were kept in shed in back of the facility. LPA Mixson observed that the food supply was sufficient and met the two day supply of perishable food items, along with the seven day supply of nonperishable food items. LPA saw that there were a variety of food selections for the resident at the time of this visit. Hot water tested within regulations at the time of this visit. LPA observed fire extinguisher was in the green and serviced. Smoke and carbon monoxide detectors were observed and operable currently. The facility is conducting emergency disaster drills quarterly; those reviewed via documentation met the Department standards. Medications were locked and stored in their original packaging (There were no observable medication mistakes at the time of this visit. LPA reviewed staff and resident records, and files reviewed were observed to have required records at the time of this visit. On 09/12/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit annual inspection. LPA was greeted and granted entry by Caregiver Diosdado Delena where LPA explained the purpose of the visit. The administrator Jose Ramirez arrived shortly after LPAs arrival. At the time of the visit there was (1) staff and (4) residents present. LPA conducted a tour of the interior and exterior and observed the following: The governing body is in good standing and the facility annual fees have been paid. The facility was observed to be free of any obstructions. LPA conducted a review of contact information and verified, and that no changes are needed at this time. LPA obtained a copy of updated liability insurance for the facility file at the regional office. The facility food supply was observed to be adequate, as there was a 2 day supply of perishable and 7 day supply of non perishable food items. LPA observed for there to be expired mustard and barbecue sauce in the storage pantry, that were discarded during LPAs visit. Therefore no deficiency was cited. The medications are stored in a locked cabinet, that is stored in the hallway heading to the kitchen. There are no known guns or ammunition, or pools or bodies of water on the premises. LPA observed for there to be three (3) medications requiring refrigeration inside the refrigerator. The administrator purchased a medication lock box during today's visit, it will be delivered on 09/15/25. The facility is conducting drills on a quarterly basis, the last drill was conducted on 08/07/25. The water temperature was unable to be checked during today's visit as both restrooms were occupied continuously throughout LPAs visit. LPA conducted a file review on both staff and resident files. The administrator Raymond Ramirez, Administrator certification was observed to be valid and expires on 10/21/25. Staff were observed to have have criminal record clearance and to be associated to the facility. The resident files were observed to have medical assessments and appraisals. The smoke and carbon monoxide detectors were tested and were found to be operable. On 09/09/24 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit annual inspection. LPA was greeted and granted entry by Caregiver Diosdado Delena where LPA explained the purpose of the visit. At the time of the visit there was (1) staff and (4) residents present. LPA conducted a tour of the interior and exterior and observed the following: The facility is in need of overall cleaning as there were trails of ants crawling in the dining room area, cob webs with spiders both alive an unalived on the walls and floor, multiple flies flying around and sitting on the kitchen table, the smell of urine, in addition there was dried puddles of urine on Resident #1 (R1) bedroom floor. Deficiency cited. In the kitchen LPA observed for the refrigerator to have a red magnetic lock. The facility does not have an approved waiver on file to have a lock on the refrigerator. Per the administrator the purpose is to minimize the residents from increasing their sugar intake. Per the file review there was no Doctor that had prescribed any of the residents with a specialized diet. Deficiency cited. LPA conducted a review of contact information and observed for the facility to not have a telephone on the premises. Deficiency cited. The medications are stored in a locked cabinet, that is stored in the hallway heading to the kitchen. LPA observed for the medications being stored in weekly increments, rather than in the original packaging. Deficiency cited. There are no known guns or ammunition, or pools or bodies of water on the premises. LPA conducted a file review on both staff and resident files. The administrator Raymond Ramirez, Administrator certification expired on 07/29/24, however proof of renewal was provided. The resident files were observed to have medical assessments and appraisals. The smoke and carbon monoxide detectors were tested and were found to be operable. The facility is conducting drills on a quarterly basis, the last drill was conducted on 7/31/24.Based on today's inspection citation's will be issued on the attached 809D, in accordance with the California Code of Regulations (Title 22, Division 6, Chapter 8).
