Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Graham Gunby arrived at the facility unannounced to conduct a Required-1 Year Inspection utilizing the inspection tool. LPA met with Administrator, Geta Loredana Pop, and explained the purpose of the visit. LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured include but not limited to resident bedrooms, bathrooms, kitchen, common areas and backyard. LPA observed required furniture, and lighting throughout the residents' bedrooms and facility. LPA observed residents' bathrooms to be clean, sanitary, and in good repair. LPA observed food supplies of non-perishables for a minimum of seven (7) days and perishable foods for a minimum of two (2) days. Toxins and cleaning supplies are locked and inaccessible to residents in care. The hot water temperature was measured in the kitchen sink at 120.3 degrees Fahrenheit. LPA observed fire detectors and carbon monoxide alarms to be operable. The fire extinguishers are located in the kitchen and hallway and were last serviced on 01/19/26. LPA observed required Licensing posters posted in the entry of the facility. LPA observed medications to be locked and inaccessible to residents in care. LPA reviewed five (5) resident files. Resident files contained all required documentation. LPA reviewed two (2) staff records. Staff have all required documentation and first aid training. No deficiencies are being cited during today's inspection. Type of visit: Case Management - Incident

Licensing Program Analysts (LPAs) Lavinia Muscan and Lavinia Talwinder Bains arrived at the facility unannounced on 5/28/25 to do a case management visit. LPAs met with Administrator, Loredana Pop and explained the purpose of the visit. Incident: Department conducted a case management visit to follow up on an incident which happened on 5/15/25 as reported by the facility. Incident report stated that: on May 15, 2025, at approximately 5:30pm, the caregiver initiated a self-cleaning function on the oven. Shortly after, staff noticed smoke coming from the oven. Staff immediately began evacuating residents and 911 was called. All residents were assessed by paramedics and found to be in good health. Residents were relocated to nearby facilities for the night. Fire Department advised that once the house was properly ventilated and kitchen was cleaned from debris and new oven installed, it would be safe for residents to return to facility. Residents returned 5/17/25. Facility notified all residents, families, responsible parties, and all other required agencies regarding this incident. Facility followed emergency protocol to handle this incident and conducted staff training. All residents are back at the facility. No immediate health and safety concerns were identified related to residents during today’s visit. No citations were issued per Title 22 Regulations. On 1/28/25, Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to conduct a Required-1 Year inspection utilizing the CARE tool. LPA met with Administrator Geta Loredana Pop and explained the purpose of the visit. LPA and Administrator toured the interior and exterior of the facility to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, residents' bedrooms, bathrooms, kitchen, garage and backyard. LPA observed required furniture, and lighting throughout the residents' bedrooms and facility. LPA observed residents' bathrooms to be clean, sanitary, and in good repair. LPA observed food supplies of non-perishables for a minimum of one (1) week and perishable foods for a minimum of two (2) days. Toxic and cleaning supplies locked and is inaccessible to residents in care. The hot water temperature was measured within compliance. First aid kit was completed. LPA observed fire detectors and carbon monoxide alarms to be operable. Fire extinguisher was serviced. LPA observed medications to be locked and inaccessible to residents in care. LPA observed required Licensing posters posted throughout the facility. During today's annual inspection, LPA reviewed resident (3) and staff file (1). All resident files contained the required paperwork. Staff file contained the required paperwork. Staff has current first aid and CPR training. Staff training contained the required initial training. Administrator certificate is current. Facility was clean and well organized. Facility is current on fire drills. LPA informed Administrator that all staff files should be available, at the facility, for review. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by 2/14/25. No deficiencies being cited during today's inspection. Type of visit: Case Management - Incident

Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced on 9/18/24 to do a case management visit. LPA met with Administrator Loredana Pop and explained the purpose of the visit. Department followed up on an Incident report that happened on 9/16/24. Incident report and SOC341 was sent to the department on 9/16/24. R1 claimed that a resident (R2) in the facility attempted to rape R1. R1 stated that R2 did rape R1 and that R2 was married with another resident and that they were expecting a baby boy. LPA observed that R2 is a two person assist and that R2 is not able to transfer unassisted. R1 also claimed that someone named “Richard” (no staff member, resident or resident family member or visitor is named Richard) raped R1. When facility notified R1’s responsible party, the responsible party stated that R1 made the same claim against R1’s occupational therapist. All requested documents relating to this incident were submitted to the department timely. Interviews conducted. At this time, this case in under review and department will follow up as needed. On 12/6/2023 at 11:26 AM, Licensing Program Analyst (LPA) Sarena Keosavang arrived at the facility unannounced to conduct a Required- 1 Year inspection utilizing the CARE tool. LPA met with staff and explained the purpose of the visit. LPA requested for staff to call administrator, Geta Loredana Pop, and notify administrator of LPA's presence at the facility. Administrator arrived at a later time. At 11:35 AM, LPA and staff toured the interior and exterior of the facility to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, residents' bedrooms, bathrooms, kitchen, garage and backyard. LPA observed required furniture, and lighting throughout the residents' bedrooms and facility. LPA observed residents' bathrooms to be clean, sanitary, and in good repair. LPA observed food supplies of non-perishables for a minimum of one (1) week and perishable foods for a minimum of two (2) days. Toxic and cleaning supplies locked and is inaccessible to residents in care. The hot water temperature was measured in the bathroom at 107 degrees Fahrenheit. First aid kit was completed. LPA observed fire detectors and carbon monoxide alarms to be operable. Fire extinguisher was last serviced on 2/9/2023. Fire drill was last conducted on 072023. LPA observed medications to be locked and inaccessible to residents in care. LPA observed required Licensing posters posted throughout the facility. At 11:50 AM, LPA reviewed a total of three (3) resident files. Resident files contain signed admission agreements, physician's reports, appraisals, identification sheets, releases, and resident's rights. Medications are centrally stored, locked, and appear to be given per doctor order. LPA compared medications to those being given for three (3) residents and found no discrepancies. Administrator will submit staff files for review by the end of business today, 12/6/2023. No deficiencies being cited during today's inspection.
