Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to complete annual inspection. LPA met with Maria Manghiuc during today's inspection. Currently there are 6 residents residing within the facility. LPA toured the facility with Administrator. LPA observed 5 resident rooms and 4 bathrooms. LPA toured the common living spaces, staff room, kitchen, and outdoor area. LPA observed all outdoor exits were clear and free from obstruction. All emergency exits were unlocked and accessible. There is a locked storage for medications and toxins. Food supply is adequate for 2-day perishable and 7-day nonperishable. LPA observed an adequate amount of linens and found the first aid kit to be complete. Water temperature was measured at 114 degrees. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA reviewed 3 of 6 resident files and 2 staff files. A review of staff records indicates that all facility staff has received criminal record clearances and/or are associated to this facility. Staff records reviewed indicated current first aid certificates and training completed. LPA observed a copy of current liability insurance. Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced and met with Administrator Maria Manghiuc to conduct an annual inspection utilizing the inspection tool. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. LPA observed five (5) resident rooms and four (4) common area bathrooms. LPA observed rooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 106.5 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two day perishable and (7) seven day non-perishable food supply on hand. Smoke detectors and carbon monoxide detectors are operational in the care home. Fire extinguishers and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed four (4) resident files, two (2) staff files and resident medications. Facility has a current copy of certificate of liability insurance and LPA obtained a copy. As a result of this visit, no deficiencies were cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. Type of visit: Case Management - Incident

Licensing Program Analyst (LPA) Cassandra Mikkelson arrived at the facility and met with Administrator Maria Manghiuc to follow up on an LIC624 Incident Report that was sent to the Department on March 15, 2025. LPA and Administrator discussed regulations in regards to medication management for all residents. LPA conducted a medication review, staff training and resident files. As a result of this visit, a deficiency was cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. The deficiency is listed on 809-D page. Allegation investigated: Facility staff are providing treatments without physician orders

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to deliver complaint findings. LPA met with Maria Manghiuc during today’s facility inspection. LPA investigated allegation, “Facility staff are providing treatments without physician orders.” LPA interviewed resident, staff, healthcare providers, and reviewed documentation. LPA interviewed relevant party in which they stated Administrator was not following physician orders for R1’s wound care. Relevant party stated that Administrator was using Medihoney instead of Betadine which was ordered by the physician. LPA interviewed physician in which they stated that she has received multiple phone calls from Administrator suggesting certain orders. Continuation on 9099-C. Unsubstantiated The physician stated during the last visit with R1, R1 had the proper wound care in place. The physician stated they had heard the administrator was using Medihoney prior to order being given 8/31/23 but they had not seen it first hand. LPA interviewed administrator (registered nurse) in which she stated she has always followed physician orders and never used medihoney until ordered on 8/31/23. LPA interviewed resident in which they stated they could not remember when medihoney was used. Due to the conflicting information LPA finds the information to be UNSUBSTANTIATED.
