Type of visit: POC
License Program Analyst (LPA) Elena Mallett conducted an unannounced POC visit. LPA was met by staff in charge, Carolina. The purpose of today’s visit was explained. LPA Mallett spoke with House Manager Peter Ventura and Administrator Sarah Levante on the phone. Peter Ventura arrived shortly after. The purpose of today’s visit was to follow up on the Plan of Correction citations issued during the 01/15/2026 Case Management visit. From the 01/15/26 inspections of the facility following citation was issued: CCR 87217(g)(1) Records of residents' cash resources maintained...account shall include a ledger accounting...for each resident, and supporting receipts filed in chronological order. Each accounting shall be kept current. Per record review , Residents(R) 1-4, do not have current PNI ledgers available for review. This poses a potential health and safety risk to 4 out of 4 clients in care Licensee agrees to provide updated PNI records for R1-R4 to reconcile the cash balance for each resident and fax these records to Office Fax by POC due date. During today's visit LPA did not observe that Residents #1-#4 had a current PNI ledger available for inspection.Civil Penalties will be issued on today's visit in the amount of $1000 for the period of 01/23/26-02/01/26 and will be ongoing until the deficiency is corrected. Type of visit: Case Management - Other
Licensing Program Analysts (LPAs)Elena Mallett and Bonnie Tao conducted an unannounced Case Management visit to the facility to check PNI accounts of residents in care. The LPA was met by Caregiver Staff 1(S1) and the purpose of the visit was discussed.House Manager Peter Ventura was contacted by phone shortly after beginning of visit, The facility is licensed to serve 4 Non-Ambulatory Residents ages 60 and above, with a hospice waiver for (1) one. 2 residents were present in the facility today and 2 were at Day Program. The facility is a single-story home located in a residential area in Norwalk, CA. LPA toured the common areas and observed no health and safety risks. LPA asked and obtained current resident and staff rosters along with resident PNI ledgers for 4 clients in care. LPAs observed PNI ledgers for Residents (R) 1-4 not be up to date. Per phone conversation with House Manager Peter Ventura the PNI Ledgers for all residents were not current and not reconciled. Upon review of the PNI ledgers LPA found that resident accounts were not being accurately recorded each month. A deficiency was observed. ( See LIC 809-D) Per Title 22 Regulations deficiencies were cited today. An exit interview was conducted with Staff 1 and explained to House Manager Ventura over the phone. Type of visit: Case Management - Annual Continuation
Licensing Program Analyst (LPA) Elena Mallett conducted an unannounced, Annual continuation visit on 11/14/2025. LPA met with Administrator Sarah Levante and explained the purpose of the visit. Common areas were toured and no health and safety risks were observed. The CARE tool was utilized to conduct visit. The following domains were completed: Staffing: There appears to be sufficient staffing at all times in the facility. With night staff that is trained and able to assist in care and supervision of the residents in the case of an emergency. Personnel Records-Training: Staff files were reviewed for criminal record clearance, current First-Aid/CPR/AED training along with training in postural supports, medication assistance, and other ongoing training. LPA reviewed 4 staff files. Deficiencies were cited. ( See LIC 809-D) Administrator Sarah Levante's Administrator certificate expires on 07/23/2026. Licensee to submit Change of Administrator request to CCLD for Sarah Levante. Incidental Medical & Dental: Medication is properly labeled and are centrally stored in a locked cabinet and are in their original containers. Facility uses a MAR ( Medication Administration Record) Log. Four client medication logs were reviewed with no issues. Administrator Levante had to leave prior to conclusion of the visit. House manager Peter Ventura joined the visit to receive the findings for the facility. Licensee to Fax bank statements for General Facility Fund and Clients' PNI accounts for September and October to CCLD Fax by close of business 11/17/2025. House Manager provided physical copy of clients' PNI bank statements for August, September and October to LPA Mallett at end of visit. Licensee was advised that Annual fees were late and PIN was provided. House Manager Peter Ventura stated the fees have been paid earlier this morning. A deficiency was cited per Health and Safety Code during today's visit. An exit interview was conducted with House manager, Peter Ventura and a copy of this Licensing report along with appeal rights were provided. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA)Elena Mallett and Licensing Program Manager (LPM) Fernando Fierros conducted the required annual inspection. LPA and LPM arrived unannounced and met with House Manager Peter Ventura and explained the purpose for today’s visit, shortly after Administrator Sarah Levante arrived to assist with the visit. The facility is licensed to serve 4 Non-Ambulatory Residents ages 60 and above, with a hospice waiver for (1) one. The facility is a single-story home located in a residential area in Norwalk, CA. A tour of the facility includes: 4 resident bedrooms, 1 resident bathroom, 1 staff bedroom, 1 staff bathroom, kitchen, dining area, living room, front yard and back yard. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit todays visit and the initial visit and observed the following: Infection Control: Staff are cleaning and disinfecting throughout the day. Facility has sufficient PPE supplies and has an Infection Control Plan maintained at the facility. Operational Requirements: The facility has an approved fire clearance, there is a plan of operation with required Infection Control Plan and facility maintains the required liability insurance and Surety Bond. (Continued on LIC809-C) Physical Plant & Environment Safety: LPA toured facility, residents’ bedrooms were checked and closet/drawer space to accommodate each resident comfortably was available. The front yard is free of debris/hazards and the outdoor and passageways are free of obstruction. No bodies of water were observed at the facility. There are no security bars or weapons on the premises. Hygiene products are readily available for clients. The hot water temperature was tested throughout the facility and measured within the required range of 105-120 degrees F. All storage areas for cleaning solutions, toxins, knives, and hazardous items are kept in a locked and are inaccessible to residents. Smoke detectors and carbon monoxide detectors are operable and in compliance. There fire extinguisher was observed and is fully charged. Resident Records-Incident Reports: Resident files are kept in a secure location and have the following documents in their files - Pre-admission appraisal/Appraisal Needs & Services Plan, Admission Agreements, Identification & Emergency Information and current Physician's Report. LPA reviewed 4 Resident Files with no issues observed. Residents Rights-Information: Residents are provided with telephone and internet at the facility. The facility has the following posters posted: Residents Rights, Complaint Poster, and Ombudsman. Planned Activities: Facility provides scheduled activities and have a variety of activities to choose from within the facility. There is an outdoor activity area available for the residents. Food Service: The kitchen was observed for the ability to prepare and serve food. LPA observed an appropriate food supply of two (2) days of perishables and one week (7 days) of non-perishables. Disaster Preparedness: The facility has an Emergency Disaster Plan with contact numbers and at least 2 relocation sites. Residents with Special Health Needs: There are no bedridden or residents using hospice or home health services at this time. Per California Code of Regulations, Title 22, and California Health and Safety Code, deficiencies observed during the visit cited per Title 22, Division 6 Chapter 8, refer to LIC 809D page. Appeal Rights were discussed and provided along with a copy of the licensing report. Due to time constraints LPA to return to complete Annual Inspection. Licensing Program Analyst (LPA) Tena Herrera conducted the required annual inspection. LPA arrived unannounced and met with Annie Gardner and explained the purpose for today’s visit, shortly after Peter John Ventura arrived to assist with the visit. The facility is licensed to serve 4 Non-Ambulatory Residents ages 60 and above, with a hospice waiver for (1) one. The facility is a single-story home located in a residential area in Norwalk, Ca. A tour of the facility includes: 4 resident bedrooms, 1 resident bathroom, 1 staff bedroom, 1 staff bathroom, kitchen, dining area, living room, front yard and back yard. Disaster Preparedness: The facility has an Emergency Disaster Plan with contact numbers and at least 2 relocation sites. Residents with Special Health Needs: There are no bedridden or residents using hospice or home health services at this time. Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during the visit. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Nicol Wesley conducted an unannounced Required 1 year inspection at the facility and met with Lilia Ordonez and explained the purpose for todays visit. The facility phone number is 562 219 0158. The facility consist of 5 bedrooms(1 staff room), 2 bathrooms, living room, dining room, kitchen, laundry area in the garage and an indoor/outdoor activity area. The facility had all postings at the front entrance, bathrooms, and throughout the facility. A Pre screening area with PPE supplies was observed upon entry into the facility. LPA conducted a complete tour of the facility, and observe the supply of food. Resident medications, and medication logs were reviewed. The smoke detectors/carbon monoxide detector are operable. LPA observed one fire extinguisher on a wall next to the kitchen. The water temperature was tested and measured at 106.3-108.9 degrees F. Administrators Certificate for Peter John E Ventura #600438740 has been renewed and it is pending/awaiting the certificate. The following deficiency was cited in accordance to the California Code of regulations title 22, division 6, chapter 8. Appeal rights given.
