Licensing Program Analyst (LPA) Gabriela Castro and Licensing Program Manager (LPM) David Sicairos conducted an unannounced required annual visit using the Compliance and Regulatory Enforcement (CARE) Tool. LPA was greeted by Jesusa Enriquez and explained the reason for the visit. Administrator Euphrosyne Dimaano arrived shortly thereafter. This home is licensed to serve 6 residents ages range 60 and over, six (6) non-ambulatory of which one (1) may be bedridden. Approved hospice waiver for six (2). Facility Tour & Observations: Personal Rights postings (LIC 613C and Ombudsman) were observed in a common area. Residents had access to personal space, privacy, and adequate storage. No firearms/weapons were present. Residents appeared comfortable. Physical Plant The facility is located in a residential area and is a one-story home consisting of four (4) resident bedrooms, two (2) bathrooms, living room, kitchen, dining area, attached garage, front yard, and backyard. Each resident bedroom contained the required furniture (bed, mattress, linens, dresser, chair, and lighting). Cleaning supplies and toxic substances are inaccessible to residents in a locked storage in garage as well as within other locked cupboards in kitchen. Bathrooms were clean and equipped with required grab bars in showers and near toilets, as well as non-skid mats; hot water measured in bathroom (1) 114.8F and bathroom (2) 108.9F which is within the required 105F-120F .Extra linens and towels were available in the hallway closet. Smoke/carbon monoxide detectors were functional, fire extinguisher was available, and no bodies of water were present. Backyard provided shaded seating. Passageways and exits were observed to be clear and unobstructed. Food Service: Refrigerators/freezers were maintained at proper temperatures (refrigerators maximum of 40 degrees F and freezer 0 degree F) with sufficient supply of 2-day perishable and 7 days non-perishable food. Fresh produce, proteins, and dry goods were stocked. Knives and cleaning supplies were observed in a locked kitchen drawer. (Continued on LIC809C) Health-Related Services & Records: Five (5) residents files were reviewed and contained current required documents (Admissions Agreements, Pre-Placement Appraisals, Consents, Needs/Service Plans, Physician’s Reports with TB/ambulatory status, Rights acknowledgments. Five (5) residents medications were reviewed; medications were observed to be centrally stored in a locked living room closet. MAR logs were current. Disaster Preparedness: Last fire/earthquake drill was conducted in October 1, 2025 with logs available. LIC 610D Emergency Disaster Plan was posted in kitchen bulletin board. Emergency supplies (water, food, flashlights, batteries, first aid) were observed. Infection Control Plan was updated. Personnel Records & Training: Five (5) staff files were reviewed and included criminal record clearances, CPR/First Aid,required training and TB screenings. Administrator Certificate for Euphrosyne Dimaano was valid through February 12, 2026. Insurance: Liability insurance was in compliance with an expiration date of December 11, 2025. An exit interview was conducted with the Administrator Euphrosyne Dimaano. During the inspection, the facility was observed to be following Title 22, Division 6 regulations. Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met Care Giver Jesusa Enriquez at approximately 8:35 AM and explained reason for visit. Administrator Peter Nora and Euphrosyne Nora arrived shortly. This home is licensed to serve age range 60 and over, six (6) non ambulatory of which one (1) may be bedridden. Approved hospice waiver for six (6). There were four (4) residents in care during the time of this visit. The facility is located in a residential area. A tour of the single-story facility included: 4 resident bedrooms, 2 bathrooms, living room, kitchen, dining area, attached garage, front yard, and backyard. LPA toured the facility and observed the following: Each resident’s bedroom has the required furniture and bedding. There is extra clean linen and towels in hallway closet. Smoke detectors/carbon monoxide were observed in each room and throughout the facility and are properly operating. The facility has one (1) fully charged fire extinguishers which is kept in kitchen. Cleaning supplies and toxic substances are inaccessible to clients in a locked storage in garage as well as within other locked cupboards in kitchen. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 40 degrees F. Sufficient supply of 2 days perishable & 7 days non-perishable foods was observed in the kitchen. There is an extra refrigerator in garage with more food. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms were measured between the required range of 105-120 degrees F. The bathrooms are clean and have the required grab bars in the shower and near the toilet for non-ambulatory residents. The shower has non-skid mat The facility does not have a swimming pool or bodies of water on the premises There is a shaded seating area for the residents located in the backyard. Passageways and exits are free of obstruction. Four (4) staff files were reviewed and included Criminal clearance record, CPR/training, and health screening with TB. Four (4) residents files were reviewed and included physicians report, TB clearance, and appraisal needs and service plan. Last fire/earthquake drill was conducted in October of 2024. Infectious control plan was reviewed. Four (4) residents’ medications were reviewed. Medications are centrally stored and locked MAR log is used. No deficiency was observed during today’s visit. On 10/19/23 at 11:15 a.m., Licensing Program Analyst (LPA) Jewel Baptiste conducted an unannounced Annual/Required inspection to Cerritos Care. Upon arrival LPA was greeted by Direct Support Professional (DSP) Florante Peralta who contacted the Administrator. The Administrator’s Peter Nora and Euphrosyne Nora arrived at 11:30 and LPA explained the reason for the visit. This home is licensed to serve age range 60 and over, six (6) non ambulatory of which one (1) may be bedridden. Approved hospice waiver for six (6). There were (5) clients in care during the time of this visit. The last emergency disaster/fire drill was conducted on 9/14/2023. The Administrator Certificate expires on 12/20/2024 #6031894740. During today's visit LPA inspected the physical plant inside and outside, reviewed the food supply, tested the smoke/carbon monoxide detectors, reviewed three (3) staff files, (5) resident files, medications, and medication administration records for (5) residents. This home contains 4 bedrooms, 2 bathrooms, living room, kitchen, dining room and an attached garage. LPA toured the physical plant with the Administrator, and observed all (4) client bedrooms, contained required furniture, lamps, dresser, chair, and closet space. The bathrooms contain a working toilet, basin, and water faucet, walk in shower with grab bar, shower chair, and bath strips. The temperature measured at 117.8*F-119.3*F respectively. The smoke detectors were battery operated and individually tested and observed to be working properly. The carbon monoxide detector was located throughout the facility, tested, and functioning properly. There were (1) fire extinguishers located in kitchen fully charged and up to date. The kitchen was toured and contained working appliances; refrigerator, stove, oven and contained dishware, cups, plates, utensils, pots, and pans. The knives, cleaning agents and toxins was locked and secured underneath kitchen sink. The pantry was well stocked with canned goods, pasta, cereals, and the food supply contained a sufficient supply with a two-day supply of perishables and a seven-day supply of non-perishables that met title 22 guidelines. Walls and floors, cabinets and counters were clean and sanitary throughout the home. (Report continued on LIC809C.) The outdoor grounds were toured and inspected, and the patio was well maintained with a shaded seating area accessible for client use. The garage contained a working washer and dryer, with cabinetry that contained emergency supply kits, bottled water, toiletries, personal care supplies, and toxins and cleaning agents stored locked and inaccessible to the clients.
