Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced annual visit today. LPA met with Emma Peteza, Caregiver and the reason for the visit was explained. Runette Catibog, Assistant Administrator, arrived thereafter and continued to facilitate the visit. The facility is licensed to serve adults age range 60 and over; approved for (6) non-ambulatory, of which (1) may be bed-ridden and approved Hospice waiver for (2). The facility is operating within the scope of its license. The facility is in a residential area of Cerritos. The single-story home consists of living room, dining area, kitchen, (3) shared resident bedrooms, (1) staff room, (2) full bathrooms, front and backyard/patio area and attached garage/laundry area. During today’s visit, LPA toured the home and observed the following: Facility was observed clean inside and out. Walkways, passages and exists are free of debris and obstructions. Living room and dining area have sufficient seating for residents and furniture is in good repair. Kitchen appliances were observed clean and operable. Facility has a sufficient 2-day perishable and 7-day non-perishable supply of food which is kept labeled and properly stored. Sharps/knives and cleaning supplies are kept locked in a kitchen cabinet under the sink and in are inaccessible to residents. Bedrooms were observed clean; however, Resident 1’s bed has a half bed rail which has not been ordered by physician. Both bathrooms were also observed clean and sanitary. Water temperature was tested in both bathrooms and measured at 118.3 and 118.7 degrees F, which is within compliance range. ***Continues on LIC 809-C The front and backyard are well maintained, and backyard has a shaded patio area and patio furniture is in good repair. Garage is kept clean and detergents and other toxins are kept locked. Laundry appliances are in good repair and were observed to be working properly. No pools or bodies of water observed. During visit, (6) resident and (4) staff files were reviewed. Resident files contain admission agreements, Physician Reports, medical/functional assessments, Appraisals/Needs and Services Plans, TB clearance, personal rights and consent forms. Staff files contain up to date First Aid/CPR certification, health screenings, criminal background clearances and training documentation. Resident medication was reviewed and found to be administered according to physicians’ orders and documented accordingly. Medication is centrally stored, locked and inaccessible to clients. Facility keeps PPE supplies in the garage. Emergency and Disaster Plan (LIC 610-D) was reviewed and is up to date. Facility conducts safety/fire drills quarterly. Last drill was conducted on 10/13/25 with staff and resident participation. Facility has a fire extinguisher in the dining room and was observed charged and operable. Fire extinguisher was last serviced on 6/24/25. Combination smoke and carbon monoxide detectors were tested and were working properly. During today’s visit, a deficiency is noted and citation issued, per California Code of Regulations, Title 22, and California Health and Safety Code. Type of visit: Case Management - Health Checks

Licensing Program Analyst (LPA) Noemi Galarza made unannounced healthy and safety case management visit to check on new relocated resident whom was relocated on 3/7/2025 from Alain Crest # 197607462. LPA met with Abigail Despi caregiver staff and explained the purpose of the visit. Assistant Administrator Runette Catibog arrived shortly after. Administrator Osvaldo Santa Ana arrived later. A physical plant tour of the facility was completed. The following observations were made: Resident (R1) is in a shared room. The room has required furniture. Medication Administration Records (MARs), medications, resident file documents were reviewed. Sufficient staff was observed. However, caregiver staff (S1) is not associated to the facility. A civil penalty is being assessed. Assistant Administrator associated S1 during the visit. LIC 500 Personnel Report needs to be updated. The facility has sufficient 2-day perishable and 7-day non perishable food supplies. T The last fire drill was conducted on 3/7/2025. Two (2) half rails were observed in R2's bed. However, the resident is not enrolled in hospice services. Postural Supports. Bed rails that extend the entire length of the bed are prohibited except for residents who are currently receiving hospice care and have a hospice care plan that specifies the need for full bed rails. Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met lead Caregiver Gino Puzalan at approximately 11:10 AM and explained reason for visit. Licensee Osvaldo Santa Ana arrived shortly. This home is licensed to serve age range 60 and over. Approved for 6 non-ambulatory of which one (1) may be bedridden. Approved hospice waiver for two (2). This home contains 3 resident bedrooms, 1 staff bedroom, 2 bathrooms, living room, kitchen, dining room and an attached garage. LPA toured the facility and observed the following: Each resident’s bedroom has the required furniture and bedding. There is extra clean linen and towels in a hallway closet. Smoke detectors were observed in each room and throughout the facility and are properly operating. There is 1 carbon monoxide in the hallway and is properly operating. The facility has one (1) fully charged fire extinguishers which is kept in the kitchen. Cleaning supplies and toxic substances were observed to be inaccessible to residents locked under kitchen sink. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 40 degrees F. Sufficient supply of 2 days perishable & 7 days non-perishable foods was observed in the kitchen. Sharps are locked and placed in cabinet in kitchen. There are no firearms or weapons stored at the facility. The two bathrooms contain a working toilet, basin, and water faucet, walk in shower with grab bar, shower chair, and bathmat. The hot water temperature in the bathrooms were measured between the required range of 105-120 degrees F. The facility does not have a swimming pool or bodies of water on the premises There is a shaded seating area for the residents located in the backyard. Passageways and exits are free of obstruction. The garage is clean and has extra supplies and freezer. SEE LIC 809C Four (4) Staff file reviewed and included Criminal clearance record, CPR/training, and health screening. Six (6) residents files were reviewed and included physicians report, TB clearance and apparel needs and service plans. Fire/earthquake drill was conducted in October of 2024.Infectious control plan was reviewed. The medications are centrally stored and locked in a cabinet in kitchen The facility uses the Medication Administration Record (MAR) log to document medications given. LPA reviewed medications for residents, and they are being administered as prescribed by the physician. On 12/11/23 at 1:20 p.m., Licensing Program Analyst (LPA) Jewel Baptiste conducted an unannounced Annual/Required inspection to Cerritos Assisted Living. Upon arrival LPA was greeted by the Administrator, Runette Catibog, and LPA explained the reason for the visit. This home is licensed to serve age range 60 and over. Approved for 6 Non- Ambulatory, of which 1 may be bedridden. Approved hospice waiver for 2. There were (4) clients in care during the time of this visit. The last emergency disaster/fire drill was conducted on 12/6/2023.The Administrator Certificate expired on 10/26/2023 #6011129740. LPA observed the Administrators named listed on the pending list on the CDSS website. During today's visit LPA inspected the physical plant inside and outside, reviewed the food supply, tested the smoke/carbon monoxide detectors, reviewed (3) staff files, (4) resident files, medications, and medication administration records for (4) resident. This home contains 3 resident bedrooms, 1 staff bedroom, 2 bathrooms, living room, kitchen, dining room and an attached garage. LPA toured the physical plant with the Administrator. and observed all (3) resident bedrooms, contained required furniture, lamps, dresser, chair, and closet space. The two bathrooms contain a working toilet, basin, and water faucet, walk in shower with grab bar, shower chair, and bathmat. The temperature measured at 111.3*F-117.1*F. The smoke detectors were battery operated and individually tested and observed to be working properly. The carbon monoxide detector was located, tested, and functioning properly. There were (1) fire extinguishers located in the dining room fully charged and up to date. The kitchen was toured and contained working appliances; refrigerator, stove, oven and contained dishware, cups, plates, utensils, pots, and pans with knives secured and locked underneath kitchen sink with cleaning agents and toxins. The pantry was well stocked with canned goods, pasta, cereals, and the food supply contained a sufficient supply with a two-day supply of perishables and a seven-day supply of non-perishables that met title 22 guidelines. Walls and floors, cabinets and counters were clean and sanitary throughout the home. (Report continued on LIC809C.) The outdoor grounds were toured and inspected, and the patio was well maintained with chairs. The back yard was also well maintained with two shaded seating area accessible for resident use. The garage contained a working washer and dryer, with cabinetry that contained emergency supply kits, bottled water, toiletries, personal care supplies, and toxins and cleaning agents stored locked and inaccessible to the residents. The dining room contained a fireplace that was locked from beneath the fireplace. The facility contained notifications and postings: California Labor Laws, Emergency Disaster Plan, personal rights, facility license, business license, medical emergency information, let-us-know licensing contact information, consumer grievance, support services, community resources and client hygiene schedule.
