Licensing Program Analyst (LPA) Lavinia Muscan arrived unannounced on October 20, 2025 to conduct the annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA and Administrator Corazon Susbilla toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. All exits were unobstructed. The administrator's certificate is current. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked . LPA observed cleaning products and other toxins to be locked away. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguishers are ready for emergency use. Water temperature is within compliance. In the areas toured, there were no health or safety violations observed. LPA reviewed resident (3) and staff files (2). All resident files contained the required paperwork. All staff files contained the required paperwork. All staff have current first aid and CPR training. Facility was clean and well organized. Facility is current on fire drills. All required posting were observed. Staff training contained the required initial training. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by end of the month. Exit interview conducted. Allegation investigated: Staff are financially abusing resident in care

On 7/3/24, Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Administrator Corazon Susbilla. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. The results of the investigation are as follows: **Report continued on 9099-C** Unfounded Staff are financially abusing resident in care Throughout the course of the investigation the department conducted interviews and reviewed documentation relevant to the allegation: Staff are financially abusing resident in care. The department reviewed resident documentation, licensee documentation, and bank records. The department reviewed R1’s bank statements and observed multiple withdrawals each month for much of their stay at the facility. Interviews with licensee indicated they did not manage resident money or keep track of R1 spending. Interview conducted with resident (R1) revealed that no financial abuse occurred and/or that financial maleficence has occurred therefore the allegation above is UNFOUNDED. This agency has investigated this complaint. We have found the complaint was UNFOUNDED, meaning that the allegation is false, could not have happened, and/or is without a reasonable basis. Type of visit: Case Management - Deficiencies

During the complaint (#59-AS-20240207154557) investigation visit on 02/14/24, Licensing Program Analysts (LPAs) Talwinder Bains and Lavinia Muscan, met with Administrator Corazon Susbilla. It was discovered, from record review for Resident, R1 that Licensee/Administrator Corazon Susbilla, was the Power of Attorney (POA) for R1, which is a violation of CCR regulation under Title 22 ,87217 (d)(2) which state that no licensee or employee of a facility shall accept any general or special power of attorney for any residents. Based on record review and interviews with R1 and staff, S1, it has been revealed that S1 was the appointed POA for R1 which was a violation of above regulation. Based on this information, per California Code of Regulations, Title 22 Division 6, Chapter 8, a deficiency is cited on the attached 809-D page. Exit interview conducted with administrator. Plan of correction: Licensee/administrator shall submit proof to CCLD that they are no longer the POA for R1 including documentation who will be the POA for R1. POC documents are due by 02/28/24. Civil Penalties shall be assessed if POC requirement is not met by due date, 02/28/24.

Licensing Program Analyst (LPA) Talwinder Bains arrived on 12/04/23 to conduct the annual inspection. LPA met with administrator,Susbilla Corazon who assisted LPA during today's inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed residents (2) and staff files (1). All residents files contained the required paperwork. Staff file contained the required paperwork. Staff have current first aid and CPR training. Facility was clean. All required postings were observed. LPA reviewed medications of 2 residents comparing with physician orders and find no errors. LPA and Administrator toured the facility together to ensure the health and safety of residents in care. The areas toured included residents rooms, bathrooms, kitchen, common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. All exits were unobstructed. There is a gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked . LPA observed cleaning products and other toxins to be locked away. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguisher is ready for emergency use . Water temperature is within compliance (110 degree F) .Inside temperature was observed to be 73 degree F. In the areas toured, there were no health or safety violations were observed. Facility has a pool which found be to be fully fenced/secured and locked during today's visit. Administrator has completed all required components regarding administrator's certificate and now waiting for the current certificate. Administrator will update LPA once they will get administrator's certificate. LPA requested a copy of the LIC500, LIC610E and current liability insurance to be sent to the Department by 12/31/23.
