On 7/30/2026, Licensing Program Analyst (LPA) K. Kaur arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility by Administrator Jacklyn Becerra. LPA conducted facility tour with Administrator Jacklyn Becerra. All pathways, entrances and exits were clear from obstructions. The tour started in kitchen, continued to dining room, living room and resident rooms. LPA observed a 7-day supply of non-perishable foods and a 2-day supply of perishable foods. Sharps are locked in kitchen cabinet. Facility Laundry room is connected to Kitchen. Laundry room observed locked with cleaning supplies and chemicals. Office area observed with back up supplies and cleaning supplies; Office is kept locked when no one is present. Dining room and Living room observed with sufficient seating and activities for residents. Facility has five residents at this time. LPA toured several bedrooms which were observed to be furnished with required furniture and adequate lighting. Four residents observed with full bed rails. Bathrooms were properly equipped with non-slip mats and grab bars. Fire extinguishers were last serviced on 6/1/2026 and was fully charged. Linen supply is kept in the hallway closet. Carbon monoxide and smoke alarm detectors installed and operational. LPA observed sufficient seating under covered patio area in the back of the facility. Side gate was self-closing and self-latching. Medications are kept in locked cabinets in the hallway. Last fire drill was conducted on 5/06/2024. Resident's records contained signed Admission Agreement, Personal Rights, and Emergency Identifications forms. 3 out of the 5 residents had current Physician's Reports. During medication audit LPA observed 2 out of 5 residents that had a missed dose of medication. 2 out of 5 residents PRN medication was not logged or was logged without the required information. Facility failed to have appraisal needs and service plan for residents and Hospice Care Plan did not have facility duties outlined for providing care to residents. Staff files were reviewed for good health. It was verified that current staff on duty is CPR certified. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. LPA is requesting the following documents be submitted to the Fresno CCL office by 8/06/2026: Current copy. of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Emergency and Disaster Plan, Personnel Report (LIC500), Register of Facility Clients/Residents LIC9020. Exit interview conducted, plan of corrections were reviewed and developed with Administrator Jacklyn Becerra. On 9/03/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection. PA introduced self, stated the purpose of the visit, and was granted entry to the facility by direct care staff. Administrator Jacklyn Becerra contacted by telephone and arrived a short time later to conduct facility inspection with LPA. LPA conducted facility tour with Administrator. Facility observed to be clean, odor free, and a comfortable temperature. Facility tour began in resident bedrooms. All bedrooms observed to have all required furnishings. All common areas of facilities observed to have adequate seating available for residents in care. Kitchen toured, LPA observed a 7-day supply of non-perishable foods and a 2-day supply of perishable foods. All knives observed to locked and secured. Facility laundry room is located within office area, laundry room was observed to be locked and secured. Facility also has chemicals stored in laundry room. Bathrooms observed to have shower chairs, skid resistant mats and grab bars available. Medications are locked and secured in medication cart. Fire extinguishers present in hallway near bedrooms, in kitchen, and near back exit door near office, all fire extinguishers have a service date of 6/06/2025. Facility is equipped with sprinkler system. LPA observed adequate supply of linens available in hallway cabinet. LPA observed sufficient seating under covered patio area in the back of the facility. Side gate was self-closing and self-latching. LPA requested the following documents to be submitted to Fresno Regional Office no later than 9/12/2025: Copy of Liability Insurance, Personnel Roster (LIC 500), Register of Facility Residents (LIC 9020), Designation of Administrative Responsibility (LIC 308). LPA conducted facility tour with Administrator Jacklyn Becerr. All pathways, entrances and exits were clear from obstructions. The tour started in kitchen, continued to dining room, living room and resident rooms. LPA observed a 7-day supply of non-perishable foods and a 2-day supply of perishable foods. Carbon monoxide and smoke alarm detectors installed and operational. Linen supply is kept in the hallway closet. Medications are kept in locked cabinets in the hallway. Last fire drill conducted on 5/17/2024. Resident's records contained signed Admission Agreement, Personal Rights, and current Physician's Report. Medication Audit conducted. Staff files were reviewed for good health. 2 out of 3 staff were not associated with the facility. It was verified that current staff on duty is CPR certified. LPA observed sufficient seating under covered patio area in the back of the facility. Side gate was self-closing and self-latching. Deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Immediate Civil Penalty assessed for caregiver background check. Continued to LIC809C LPA is requesting the following documents be submitted to the Fresno CCL office by 7/17/2024: Current copy. of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Emergency and Disaster Plan, Personnel Report (LIC500), Register of Facility Clients/Residents LIC9020.
