Type of visit: Required - 1 Year
On 10/25/25, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to conduct the required annual inspection. LPA was greeted by Administrator, stated the purpose of the visit and was allowed entry into the facility. Administrator on record is Leticia Villela, Certificate #7003015740, expires 10/26/25, renewal application was submitted. LPA toured the facility inside and out. 4 out of 4 residents in care receive Regional Center services. Facility is a three bedroom two bathroom home. Resident bedrooms were observed to have the required lighting and furnishings and were free from odor and free from any passageway obstruction / fire hazards. Facility temperature was 74 degrees F. Bathrooms were toured and observed to have operational lights, grab bars, running water, and non- slip floors. Hot water temperature tested at 110 degrees F. Trash can with lid and hand washing postings were observed. Medications were observed to be locked in cabinet(medication station) located in the hallway. Cleaning supplies were observed to be locked in a cabinet in the hallway. LPA toured the kitchen observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. Outside of facility toured. All exits open and free of obstruction. No hazards observed. Facility observed to have several shaded areas with seating available outdoors. Carbon monoxide and smoke detectors were tested and observed to be operational. Night lights were observed in the hallways. Fire Extinguisher was observed with a service date of 04/08/25. First aid kit was observed and contained all required items. Required postings were observed. Quarterly Emergency Disaster Drill logs were observed for staff. No residents are receiving Hospice services residents or receiving Home Health care service. A sample of resident and staff files were reviewed and observed to have the required forms and training records. (Continued on LIC 809-C) (Continued from LIC 809) An exit interview was conducted with Administrator. A copy of this report was discussed and provided at the time of visit. No deficiencies cited on today's visit. Type of visit: Required - 1 Year
On 10/25/2024, Licensing Program Analyst (LPA), M. Medina conducted an unannounced Annual Required visit. LPA arrived, introduced self, stated purpose of visit, and allowed entrance by Licensee, Leticia Villela. Leticia also serves as facility Administrator, certificate #7003015740, expires 10/26/25. Facility observed to be clean, odor free, and a comfortable temperature. Currently, four (4) residents in care, 1 (one) resident was present at time of inspection. Facility is a 3 bedroom, 1 bathroom home. There is one (1) shared room and two (2) private shared room. Living room area and dining room area observed to have adequate seating for residents. Kitchen toured, facility observed to have a 2-day of perishable and a 7-day of non-perishable food available. Additional freezer and refrigerator/freezer are stored in the garage for facility. Knives observed to be locked and secured, stove knobs observed to be off and secured. Resident bedrooms toured and observed to have all required furnishings, resident bathroom toured and observed to have grab bars. Shower area observed to have grab bars, non-skid mat, and shower chair available. Water temperature measured at 115 degrees F. Carbon monoxide and smoke detectors present and observed operational during inspection. Fire extinguisher present with a service date of 3/27/24. All chemicals observed to be locked and secured under kitchen sink and additional supplies secured in laundry area. Last fire drill conducted 9/08/24 according to facility records. Outside of facility toured. All exits open free of obstruction. No hazards observed. Facility observed to have several shaded areas with seating available outdoors. No deficiencies cited during inspection. To improve the quality and value of the inspection process, a survey will be sent to the email address provided. Type of visit: Required - 1 Year
On 10/22/23 at 12:26 PM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to conduct an Annual inspection. LPA explained reason for inspection and was granted entry by Licensee/Administrator (LIC) Leticia Villela. LPA toured inside and outside the facility. Facility set at comfortable temperature. Smoke and carbon monoxide detectors tested and operational. All bedrooms observed with sufficient furnishings and lighting. Non-skid mats observed in showers. Grab bars observed for each toilet and shower. Sharps observed locked. Food supply observed sufficient. Centrally stored medication observed locked. Staff and resident records reviewed. Administrator certificate valid. No deficiencies cited during this inspection. An exit interview was conducted. A copy of this report was left with Licensee, whose signature on this form confirms receipt of these documents.
