Allegation investigated: Staff confined resident to a wheelchair

On 5/5/2026, Licensing Program Analyst (LPA) M Vega conducted 10 day complaint investigation visit to the facility. LPA arrived at the facility and was granted access by staff and met with Administrator (AD) - Melinda Aguilar. During the course of this complaint investigation LPA obtained and/or reviewed facility records and conducted a Health and Safety check on the residents in care. It was determined based on the record review that the above allegation is UNFOUNDED. Allegation: “Staff confined resident to a wheelchair” - per review of resident files there is a hospice order for postural supports for R1. Order is signed by Physician and Registered Nurse. All residents that need postural support have proper documentation. This agency has investigated the above allegation and has determined them to be UNFOUNDED. This means that the allegation was false, could not have happened or is without a reasonable basis. We have found that the complaint was unfounded, therefore we have dismissed the allegations. No deficiencies issued during this inspection. An exit interview was conducted with the Administrator - Melinda Aguilar. Mar 18, 2026Facility evaluation reportReport on file

Type of visit: Required - 1 Year

On 3/18/2026, Licensing Program Analyst (LPA) J. Leffall arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility by Administrator (A1) Melinda Aguilar. 5 residents were present during the inspection LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Samples of resident’s medications were observed locked in cabinet. Clients’ MARS was reviewed with samples of resident’s medications. Fire extinguisher reviewed with a purchase date of: 9/23/25. Fire drill completed on 2/3/26. Clients' bedrooms were toured and reviewed. Cleaning chemicals were observed stored and locked in closet. Residents bedrooms observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 105.6 to 118.5 degrees in 3 bathrooms. Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature observed at -1 degrees F and refrigerator temperature maintained at 36 degrees F. Smoke detectors and carbon monoxide were tested and observed to be operational. All clients’ files and samples of staff’s files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. LPA is requesting the following documents be submitted to the Fresno CCL office by 4/1/26: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E ), Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020A) A copy of this report was provided to Administrator, whose signature on this form confirms receipt of this report.the state’s words, verbatim · CDSS document, Mar 18, 2026

An unannounced Annual visit was conducted on the date & times above by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with & greeted Administrator (Admin) Physical plant toured. Dining, living, & parlor rooms sufficiently furnished with adequate lighting. Kitchen toured. Appliances at appropriate temperatures. 7 day supply of non-perishable & 2 supply of perishable food on the premises. Sufficient supply of utensils, plates, drink wear. Knives inaccessible. Kitchen appeared to be clean with no unpleasant odors. Resident bedrooms toured. Bedrooms appeared to be sufficiently furnished with adequate lighting & to appear & smell to be clean with no unpleasant odors. Physician's orders for 1/2 rails on file. Resident's receiving hospice care services with full rail have physician's order on file. Required linens on bed. Sufficient supply of linens on the premises. Resident bathrooms appeared to be clean with no unpleasant odors. Fixtures operational with grab bars in all toilet, tub/shower areas. Non-skid surfaces in tub/shower. Sufficient supply of grooming items on the premises. Facility, resident & staff records reviewed. Medications organized & appropriately labeled. Medication documents, MARs, CSMDRs maintained. Designated location for cleansers & other items may be hazardous, maintained in a locked area inaccessible to residents.Interior & exterior passageways free of obstructions. Ramps at front & back doors. Property maintained & appeared neat/tidy with no miscellaneous debris or trash. Smoke & carbon monoxide detectors operational. Fire extinguisher service date: 7/10/24.
