Type of visit: Required - 1 Year
On 8/20/2026 Licensing Program Analyst (LPA) K.Kaur arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Licensee Jose Pira. Facility has no residents in care. LPA toured facility with Licensee. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. Some food was observed in refrigerator. Freezer temperature was maintained at -2 degrees F and refrigerator temperature was maintained at 38 degrees F. Cleaning chemicals were observed stored and locked in cabinet and laundry room. Fire extinguisher was observed with a service date of: 01/19/2026. resident's bedrooms were toured and observed to be adequately furnished with bed, dresser, chair and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 110 in bathroom. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seating available for residents. Medications are not present in facility because of vacancy. Facility does have a Centrally Stored Medication Record designation in locked cabinet. Carbon monoxide and smoke detectors were tested and observed to be operational. No resident files were reviewed due to vacancy in the facility. No staff files were reviewed as facility does not currently have staff. No deficiencies issued during this inspection. LPA requested the following documents to be submitted to CCL by 8/27/26: current copy of Administrator’s Certificate, Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-D), Affidavit regarding Resident Cash Resources (LIC 400), Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020) in order to update the facility file. Exit interview conducted Administrator/ Licensee Jose Pira. On 7/10/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Licensee (L1) Jose Pira and Licensee (L2) Melanie Rafanan. LPA was granted entry. 0 clients are currently not placed in facility. LPA toured facility with L1 and L2. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. No food was observed in refrigerator or freezer because of no residents in facility. Freezer temperature was maintained at -2 degrees F and refrigerator temperature was maintained at 38 degrees F. Cleaning chemicals was observed stored and locked in cabinet. Fire extinguisher was observed with a service date of: 3/26/24. Fire drill currently not conducted because of vacancy of residents in facility. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 105.4 in 1 bathroom. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Medications not present in facility because of vacancy. Facility does have a Centrally Stored Medication Record designation in locked cabinet. Carbon monoxide and smoke detectors were tested and observed to be operational. No clients’ file reviewed because of vacancy in the facility. No staff files were reviewed as facility does not currently have staff. Licensee and Administrator was advised that staff files will need to be present and ready for review in facility. No deficiencies issued during this inspection. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 7/24/25: Lic 308, Lic 500, Lic 610D, Lic 9020 and Administrator Certificate. Type of visit: Prelicensing
Licensing Program Analyst (LPAs) K.Kaur and J. Leffall conducted Pre-licensing follow up Inspection on this date. LPA met with Licensee Melanie Rafanan and Jose Pira. A tour of the facility was conducted together. This is a new facility with no residents in care. The facility was observed to be at a comfortable temperature, and in good repair. No passageway obstructions or fire hazards were observed inside or outside. Common areas were properly furnished and well-lit throughout. The facility is a 3-bedroom, 1-bathroom home with a fire clearance for 6 non-Ambulatory. Licensee has made all corrections. Water temperature tested at 110 F. LPAs observed Liability insurance paperwork. Non-discrimination notice has been posted. All debris was cleared from back-yard and tools were moved to a locked location. Night stands were added to the bedrooms. Facility telephone number is 559-802-3166. Component III was conducted during today's pre-licensing visit. Applicant has met all pre-licensing requirements. Type of visit: Prelicensing
Licensing Program Analyst (LPA's) K.Kaur and J. Leffall conducted a Pre-licensing Inspection on this date. LPA's met with Licensee/Administrator Melanie Rafanan and Jose Pira. A tour of the facility was conducted together. This is a new facility with no residents in care. The facility was observed to be at a comfortable temperature, and in good repair. LPA’s did not observe operational telephone service. No passageway obstructions or fire hazards were observed inside or outside. Common areas were properly furnished and well-lit throughout. LPA’s observed Dining table located in kitchen. Table is not sufficient for 6 residents. Knives and sharps are locked in cabinet in kitchen. Fire extinguisher observed in the kitchen with a service date of 3/26/2024. Medications will be locked in cabinet located in kitchen area. Scissors not observed in First Aid Kit. Living room is equipped with adequate futon and chairs for residents. Cleaning and Chemical supplies observed locked in laundry room next to the washing machine. Resident’s bedrooms were observed to be adequately furnished with bed, dresser, chair and adequate lighting. LPAs did not observe nightstands in resident rooms. Mattresses and linen were in good condition. Extra linen and towels are available in the hallway cabinets. Carbon monoxide and smoke alarm detectors installed and operational. Grab bars installed in shower and by toilet, non-skid mats in place, hand soap and paper towels available for use. Trash cans with tight fitting lids are in place. Complaint poster posted, resident council info posted, residents' rights posted, emergency disaster plan posted. LPA’s did not observe nondiscrimination and theft policy. Adequate outside space for rest and recreational under a patio with sufficient seating. LPA’s observed excess broken items. LPA’s observed unlocked garden tools and ax in backyard. Gate is self-closing and self-latching. The following issues will need to be corrected prior to pre-licensing visit and Licensure of facility: 1. Liability Insurance 2. Facility Telephone Service Continued to LIC 809C 3. Post nondiscrimination and theft policy 4. Licensee to lock garden tools, ax, sharps. 5. Remove broken items in backyard not in use. 6. Missing scissors from First Aid Kit 7. Night-stand in each resident room Pre-licensing requirements were not met. An exit interview was conducted with Licensee/Administrator. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of the following areas: 1.
