Type of visit: Required - 1 Year
Licensing Program Analyst's (LPA) Edith Conchas made an unannounced visit to the facility to conduct an annual inspection. LPA met with Licensee Rose Wright-Ilori and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6) and a current census of (5) residents in care. The facility has four (4) hospice residents. LPA conducted an overall inspection of the facility The indoor and outdoor passageways are free of obstruction. The outdoor shaded area is sufficient for resident activities and enclosed by self-latching gates. LPA inspected client bedrooms are equipped with required furniture. The bathrooms were operating in safe and sanitary conditions, and appliances operated appropriately. The facility has sufficient lighting and is maintained at a comfortable temperature of 76 degrees Fahrenheit (F). Residents’ bathrooms were operating in safe and sanitary conditions. The hot water temperature in residents' bathrooms measured between 112-115 degrees F. Residents’ bedrooms have sufficient lighting and furniture in good repair. Facility has operating carbon monoxide alarms and telephone service. The facility has sufficient linen, towels, and personal hygiene items for residents. The facility has posters in a common area, facility license, Community Care Licensing complaint poster, Ombudsman poster, disaster evacuation plan and emergency telephone numbers and current liability insurance. The facility has sufficient non-perishable and perishable food supply for residents in care. The refrigerator and freezer are operating in a healthful manner. Pesticides and other cleaning solutions were kept locked and stored away from food areas. Continue to LIC809-C The facility has 24-hour/7-days a week care staff. The staff files were reviewed and observed. The staff working have criminal record clearances or exemptions through the Department. The residents’ files were reviewed and observed. LPA audited the residents’ medications and are centrally stored and kept in a locked cabinet. Based on LPA observations and record review, the following technical were issued LPA observed temperatures to run between 95-98 degrees. License adjusted the water. Water was re-measured to be at 112-115 degrees F. LPA observed complaint poster but was not at the required measurement. LPA observed plan was not annually reviewed. However, license reviewed and updated. No changes were made. LPA observed emergency kit is sufficient for 4, census is 5. Licensing Program Analyst's (LPA) Edith Conchas and Eldin Serrano made an unannounced visit to the facility to conduct a required annual inspection. LPA's met with Licensee Rose Wright-Ilori and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6) and a current census of (5) residents in care. The facility has four (4) hospice residents. LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant: Indoor and outdoor passageways are free of obstruction. The facility has no bodies of water accessible to residents in care. Outdoor shaded area is sufficient for resident activities and enclosed by self-latching gates. LPAs inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were operating in safe and sanitary conditions, and appliances were operating appropriately. The facility has sufficient lighting and is maintained at a comfortable temperature. The facility temperature was at room temperature of 76 degrees Fahrenheit (F). Resident’s bathrooms were operating in safe and sanitary conditions. The hot water temperature in residents' bathrooms measured 111.2 degrees F. Resident’s bedrooms have sufficient lighting and furniture in good repair. Facility has operating carbon monoxide alarms and telephone service. The facility has sufficient linen, towels, and personal hygiene items for residents. The facility has posters in a common area, facility license, Community Care Licensing complaint poster, Ombudsman poster, disaster evacuation plan and emergency telephone numbers. ****Continue 809-C**** Food Service: Facility has sufficient non-perishable and perishable food supply for residents in care. The refrigerator and freezer are operating in a healthful manner. Pesticides and other cleaning solutions were kept locked and stored away from food areas. Care & Supervision: Facility has 24-hour/7-days a week care staff. Staff working have criminal record clearances or exemptions through the Department. Record Review: Two (2) staff files were reviewed and observed to be complete. Three (3) out of five (5) resident files were observed to be complete. Facility maintained a completed physician's report, medical assessment, including tuberculosis, preplacement appraisal including physician's signature and date. Medical Related Services: LPA's observed medications is centrally stored and kept in a locked cabinet. Based on LPA observations and record review no deficiencies are being cited per Title 22, Division 6 of The California Code of Regulations. An exit interview was conducted where reports (LIC809/ LIC 809-C) were discussed with the licensee Rose Wright-Ilori. Type of visit: Required - 1 Year
Licensing Program Analysts (LPAs) Magda Malcore and Becky Mann made an unannounced visit to the facility to conduct a required annual inspection. LPAs met with Jonie Gatus, Assisting Administrator, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6) and a current census of (6) residents in care. LPAs conducted an overall inspection of the facility, which included, but was not limited to, the following: Operation/Physical Plant: The facility has an infection control plan and emergency disaster plan on file. Indoor and outdoor passageways are free of obstruction. The facility has no swimming pools or similar bodies of water. The facility has sufficient indoor and outdoor activity space. Outdoor activity area is enclosed with a self-latching gate. The facility has sufficient lighting and is maintained at a comfortable temperature. Resident bathroom equipment were operating in a safe and sanitary condition. The hot water temperature in residents' bathrooms measured 105 degrees F. Resident’s bedrooms had beds, bed linen, chairs, sufficient storage space and lighting. Facility has operating carbon monoxide alarms, telephone service and a signal system. The facility has sufficient linen, towels, and personal hygiene items for residents. The facility has posted in a common area, Community Care Licensing complaint poster, Ombudsman poster, Personal Rights, disaster evacuation plan and emergency telephone numbers. Sharps, disinfectants, and cleaning solutions were kept locked and inaccessible to residents in care. Food Service: Facility kitchen and dining areas are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents in care. The facility's refrigerator and freezer were operating in a healthful manner. Care & Supervision: Facility has 24-hour, 7 days a week care staff. Staff working have criminal record clearances through the Department. Record Review: Staff files reviewed were observed to be complete. Resident files reviewed were observed to be complete. Administrator’s certification and liability insurance were current. Medical Related Services: Medications were centrally stored in a locked cabinet. LPAs review of resident medications reveals staff did not maintain a complete and current record of resident medications for review. LPAs were provided several lists to find all current medications. Discontinued medications were also listed as current medications. Resident#1 (R1) had a medication on file which was labeled for daily use; however, the Assisting Administrator stated that the medication is to be provided as needed and will have R1's physician update the medication label. Based on observations and record review, deficiencies are being cited per Title 22, of The California Code of Regulations.
