On 2/19/2026, Licensing Program Analysts (LPA), Ahliah Sharp and Tremayne Barra, conducted an unannounced required annual visit. The LPAs were allowed entrance into the facility and met with Caregiver, Miguel DeLa Cruz. The LPAs informed Miguel of the purpose for the visit. Administrator, Diana Valdez later arrived to assist with the inspection of the grounds. The inspection included the following: Food Service: The LPA inspected the facility's kitchen areas and food supply. The LPA observed all food to be of good quality. The facility maintained the required (2) two day supply of perishable foods and (7) seven day supply of non-perishable foods. LPA observed emergency food and water to be stored in the garage. Knives and other sharp items were stored in a locked cabinet. Physical Plant: The facility consists of four (4) resident bedrooms, three (3) bathrooms, a kitchen and two (2) dinning areas, a living room area, a sitting room area, a garage and storage spaces, and a yard with sufficient seating and space for activities. There are no bodies of water located on the property. According to Caregiver Miguel, no weapons are stored in the home. The facility is being maintained at a comfortable temperature 77 degrees. All outdoor and indoor passageways were kept free of obstruction and are free of debris and other trash. There are grab bars for toilets and showers used by residents. One smoke detector and one carbon monoxide device was tested by staff and were observed to be in operating condition. The facility was kept clean, organized and free of any odors. Record Review: Staff were observed to have appropriate fingerprint clearances. Training on dementia care, postural supports, restricted health conditions, hospice and medication administration was observed on file. Resident files had admission agreements, medical assessments, and other required records on file. . The facility currently has an approved Hospice Waiver for two (2) residents; and there is currently one (1) resident in care, receiving hospice services. There is an emergency disaster plan in place. The licensee (HONORS WAY CARE HOME LLC) is a current and active corporation. The LPA observed current liability insurance on file, which expires 03/04/2026. Medication Review: The LPA inspected resident medications. Medications were observed to be well organized and inaccessible to unauthorized individuals. Centrally stored medication destruction records were observed on file. An exit interview was conducted with Administrator Valdez, in which this report was reviewed, and a copy was provided. Licensing Program Analyst (LPA), Stephanie Martinez, conducted a required annual inspection at the home. The LPA was allowed entrance into the facility and met with Administrator, Diana Valdez. The LPA informed Valdez of the purpose for the visit. The inspection included the following: Food Service: The LPA inspected the facility's kitchen areas and food supply. The LPA observed all food to be of good quality. All readily perishable foods and beverages capable of supporting rapid and progressive growth of micro-organisms were stored in covered containers at appropriate temperatures. Soaps, detergents, cleaning compounds and similar substances were stored in areas separate from food supplies. All kitchen areas were kept clean and free of litter, rodents, vermin, and insects. Physical Plant: The facility consists of four (4) resident bedrooms, three (3) bathrooms, a kitchen and two (2) dinning areas, a living room area, a sitting room area, a garage and storage spaces, and a yard with sufficient seating and space for activities. The LPA observed current liability insurance on file, which expires 03/04/2025. Medication Review: The LPA inspected resident medications. Medications were observed to be well organized and inaccessible to unauthorized individuals. Centrally stored medication destruction records were observed on file. Administrator Valdez agreed to provide the LPA with a copy of the current liability insurance, staff schedule, and resident roster. An exit interview was conducted with Administrator Valdez, in which this report was reviewed, and a copy was provided. Licensing Program Analyst (LPA), Stephanie Martinez, conducted a required annual inspection at the above named facility. The LPA was allowed entrance into the facility by Administrator, Diana Valdez. The LPA informed the Administrator of the purpose for the visit. The facility currently has an approved Hospice Waiver for two (2) residents. The inspection included the following: Physical Plant: The facility consists of four (4) resident bedrooms, two dinning areas, two living spaces, an open kitchen, a laundry room, a garage space, and a patio with sufficient seating and space for activities. There are no bodies of water located on the property. According to Administrator Valdez, no weapons are stored in the home. The facility is being maintained at a comfortable temperature. All outdoor and indoor passageways are kept free of obstruction and are free of debris and other trash. There are grab bars for each toilet, bathtub and shower used by residents. Resident showers have non-skid mats or strips present. The carbon monoxide and smoke detectors were tested by facility staff and were observed to be in operating condition. The home was kept clean and free of any odors. Food Service: There is a minimum of 2 days of perishable foods and 1 week's supply of non-perishable foods available. The kitchen was observed to be maintained in a clean state. Record Review: All staff were observed to have appropriate fingerprint clearances. LPA did not observe any excluded individuals on the premises at time of visit. Staff responsible for direct care and supervision have current first aid and CPR training. Dementia care training was observed to be available. No documented training for Staff Two (S2) was observed on file relating to Restricted Health Conditions, Hospice Care, or Postural Supports. All records were well organized. Administration: The facility was operating beyond the conditions specified on the license. The LPA was informed by Administrator Valdez there are currently three residents in care who are receiving hospice services. The facility only has an approved hospice waiver for two residents at any time. A citation will be issued. There is a disaster and mass casualty plan in place. All services requiring specialized skill are being performed by personnel qualified as appropriately skilled professionals. Medical Related Services: There are no persons who have a prohibited health condition admitted at the facility. The LPA conducted a medication audit and observed no violations.
