Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to Harbor View Estate. The purpose of today’s visit was to conduct the Annual Required inspection. LPA was allowed entry into the facility and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents and there are 4 residents present during today's visit. Facility has an approved hospice waiver for 6 residents and the home currently has 1 resident on hospice. Administrator Vilma Manuel has an administrator certificate valid until 03/07/2028. Upon entry, facility appears clean and sanitary. LPA Lyman along with Administrator Manuel toured the facility at 8:25 AM. LPA toured the physical plant, checked food service, first aid kit and reviewed records. The home consists of four resident bedrooms, 1 common restroom, 1 staff restroom, living room, dining room, 2 staff rooms, laundry room and kitchen. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident rooms are single and double occupancy. There are two residents with half bed rails. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between at 105.4 and 107 degrees F in facility restrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards. First aid kit had all the elements including thermometer, tweezers and scissors as well as a first aid manual. LPA observed toxins are secured during today's visit. Auditory exit alarms are operational during today's visit. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances are operational during today's visit. LPA observed protective mechanisms on the cook top knobs. Smoke detectors and carbon monoxide detectors tested operational during today's visit. Fire extinguisher is fully charged. LPA toured the outside grounds and there is ample shaded seating for residents. Exit gates are unlocked and operational. CONT ON LIC809-C DATED 05/26/2026 LPA observed ample emergency food and water supply. LPA reviewed the emergency disaster plan and infection control and plans are thorough and complete. Facility provides activities in the form of exercise and music therapy. LPA reviewed four resident files and three staff files. Resident files contained required documents including admission agreements, physician reports, physician orders for bed rails as indicated and resident appraisals. Staff files reviewed contained required documentation of medical clearance/ TB, CPR training and criminal record clearance as well as required training. LPA reviewed medication storage and administration. Medications are stored in a locked closet. Medications appear to be administered per physician order and facility audits medications weekly. During the visit, LPA consulted with Administrator regarding the procedures for submitting an exception and provided the following email for submittal: CCLASCPORANGECOUNTYRO@DSS.CA.GOV Based on the observations made during today's visit, NO deficiencies are being issued. Licensing Program Analyst (LPA) Kimberly Lyman made an unannounced visit to the facility for the purpose of a Plan of Correction (POC) visit, based upon the deficiencies cited in LIC form 809D on 06/03/2025. LPA was greeted and granted entry into the facility and explained the reason for the visit. *Deficiency cited under Title 22 Regulation 87303(a) pertaining to Maintenance and Operation has been cleared. Licensee corrected noted items. Licensee has complied with the terms of the POC. LPA observed noted items on advisories have been addressed. Licensee has been advised to maintain all items especially those that were previously deficient in the facility in accordance with Title 22 Regulations. LPA toured the physical plant, checked food service, first aid kit and reviewed records. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident rooms are single and double occupancy. At 8:10 AM, LPA observed facility has put up a divider in staff room. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured at 108.6 degrees F in facility restroom. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards. First aid kit had all the elements including thermometer, tweezers and scissors. LPA observed toxins are secured during today's visit. Auditory exit alarms are operational during today's visit. Facility provides activities in the form of exercise, and music therapy. LPA reviewed five resident files and three staff files. Resident files contained required documents including admission agreements, physician reports, physician orders for bed rails as indicated and resident appraisals. Staff files reviewed contained required documentation of medical clearance/ TB, CPR training and criminal record clearance as well as required training. LPA reviewed medication storage and administration. Medications are stored in a locked closet. Medications are being administered per physician order and facility audits medications weekly. Based on the observations made during today's visit, the following violation is being cited per California Code of Regulations, Title 22, Division 6, Chapter 8. Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Vilma Manuel, Administrator. The facility is a single story building with an approved fire clearance of six non-ambulatory residents of which six are approved for hospice. The facility currently has a census of five residents in care with two on hospice. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in two of two resident bathrooms, and testing auditory devices on all exits. The hot water temperature measured between 111.0 and 111.3 degrees Fahrenheit and all smoke detectors were operational. The fire extinguisher is charged and was serviced on April 24, 2024. The facility’s last fire drill was conducted on April 30, 2024. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed medication storage and reviewed the centrally stored medications. Per review medication appear to be being given as prescribed. LPA reviewed three of three staff training and fingerprint records. LPA conducted a complete review of resident records. LPA interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPA confirmed that administrator has a pending renewal administrator certificate which will expire on March 9, 2026. Administrator has proof of certified mail and check was cashed. Based on the observations made during today’s visit, the facility appears to be in compliance with Title 22 Division 6 of the California Code of Regulations, no deficiencies cited on this date.
