Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Hope Gianan and explained the reason for the visit. Hope Gianan's Administrator's Certificate expires on March 12, 2028. Facility is a single story home with 7 bedrooms, 5 bathrooms, living room, activity room, dining room, kitchen and a 2 car garage. Facility is licensed for 6 non-ambulatory residents of which 1 can be bedridden and a hospice waiver for 5. LPA and Administrator toured the facility. LPA observed the See Something, Say Something poster posted in the main entry way of the facility. The fireplace in the living room is screened. The Administrator reported the fireplace is never used. LPA observed all resident rooms had the required furnishings. LPA observed all bathrooms are clean and operational. Hot water measured 113.5 degrees Fahrenheit in the shared bathroom in the hallway. The fire extinguisher in the kitchen is fully charged. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. Knives are kept locked in a kitchen drawer. Soaps and cleaning supplies are kept locked under the kitchen sink. LPA observed the 5 burner gas stove lights unassisted. Medications are kept locked in the hall closet. LPA observed clean linens are stored in the pantry in the hallway. Smoke detectors/carbon monoxide detectors tested operational. The last emergency drill took place on April 15, 2026. LPA and staff toured the backyard. No bodies of water observed. There is a shaded seating area in the backyard. Both exit gates are operational. No obstacles or hazards observed in the backyard. LPA and the Administrator toured the garage. The garage is kept locked and used for storage. LPA observed extra supplies and food stored in the garage. LPA observed a 3 day supply of emergency food and water stored in the garage. LPA inspected the first aid kit. The first aid kit has all the required elements. LPA reviewed 6 resident records and medications, no discrepancies observed. LPA reviewed 4 staff files, no discrepancies observed. All 4 staff have the required training and CPR/First Aid Training. All staff present at the facility are background cleared and associated to the facility. No deficiencies observed during the visit. No deficiencies are being cited as a result of this visit. LPA and the Administrator toured the facility. LPA observed all resident rooms had the required furnishings. Extra linens and bedding are stored in the hall pantry. All resident bathrooms are clean and operational. Hot water measured 111.0 degrees Fahrenheit. No obstacles or hazards observed inside of the facility. The smoke detectors/carbon monoxide detectors tested operational. LPA inspected the kitchen. LPA observed the kitchen is clean and organized. LPA observed a two day perishable and a seven day non-perishable food supply on hand in the kitchen. The stove lights unassisted. LPA observed the fire extinguisher in the kitchen is fully charged. The knives and sharp objects are kept locked in a kitchen drawer. Cleaning supplies are kept locked under the kitchen sink. LPA toured the backyard. The exit gate is operational. No bodies of observed. LPA observed there is a shaded seating area for the residents to sit outside. LPA toured the garage. The garage is used for storage and off limits to the residents. LPA reviewed resident medications, no discrepancies observed. LPA reviewed 4 out of 4 resident records, no discrepancies observed. LPA reviewed 4 out of 4 staff files. LPA observed that Staff 1's and Staff 2's, 20 hours of annual training could not be verified. No other discrepancies observed. Deficiencies are being cited per Title 22 division 6 of the California Code of Regulations.
