An office meeting was held in the San Diego Adult and Senior Care Regional office. The following people were present at the meeting: RM Jerry Romero, LPM Simon Jacob, LPM Lizzette Tellez, LPA Angelica Boyles, Chuck Thomas and Suolang “Julie” Quzchen. The purpose of this meeting was to gather information regarding the operations of of Gate Manor, Gate Manor II, and Gate Manor III. Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced visit to obtain signatures on an amended report. During today’s visit, LPA was greeted by Administrator Ambalada, identified herself, and discussed the purpose of the visit. During today’s visit, LPA obtained signature on an amended version of a report originally delivered on April 28, 2026. Type of visit: Case Management - Annual Continuation

icensing Program Analyst (LPA) Debbie Correia made an unannounced visit to the facility to conduct a continuation of the annual licensing inspection. LPA identified herself to Administrator Ambalada, was granted entry, and explained the purpose of the visit. Gate Manor II is licensed to serve six (6) residents, all of whom may be non-ambulatory. The facility may serve up to four (4) residents receiving hospice services and one (1) bedridden resident. During today’s visit, the current census was five (5) residents: three (3) non‑ambulatory and two (2) receiving hospice services. The facility was operating within its licensed capacity and conditions. LPA, accompanied by Administrator Ambalada, conducted a tour of the facility, inspecting both the interior and exterior. The facility was observed to be clean, sanitary, and in good repair. Pathways were free of obstructions and slip hazards. Required postings were visible. Resident bedrooms contained the required furnishings. Resident showers were equipped with non‑skid flooring and grab bars and were in working order. Doors, windows, and screens were in good condition. Extra linens, hygiene supplies, and Personal Protective Equipment (PPE) were available. LPA observed a landline telephone; operational smoke and carbon monoxide detectors; a current fire extinguisher; and a complete First Aid kit and manual. The facility includes an adequately sized common area and a shaded outdoor space for activities and visitation. [Continued on LIC 9099C} [Continuation of LIC 9099] There were no bodies of water observed on the premises. Per Administrator Ambalada, there are no firearms or ammunition stored at the facility. Medications were stored in a locked medication cart. Toxic substances and hazardous materials were secured in locked cabinets. Based on today’s inspection, no deficiencies were cited. An exit interview was conducted, and a copy of this report, along with Licensee Rights (LIC 9058, rev. 01/16), was provided to Administrator Ambalada. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced visit to the facility to complete an annual licensing inspection. LPA identified herself to Administrator Maniza Ambalada of Gate Manor II, was granted entry, and explained the purpose of the visit. Administrator Charles Thomas of Gate Manor I arrived shortly thereafter to participate in the inspection. Gate Manor II is licensed to serve six (6) residents, all of whom may be non-ambulatory. The facility may serve up to four (4) residents receiving Hospice services and one (1) bedridden resident. At the time of today’s visit, the current census consisted of five (5) non-ambulatory residents and four (4) receiving hospice services. The facility was operating within its licensed capacity and conditions. LPA Correia conducted a review of resident records and completed a partial tour of the facility. Based on today’s review, resident records were complete and up to date. During the facility tour, LPA observed that smoke detectors, carbon monoxide detectors, and fire extinguishers were present and operable. Required postings were properly displayed, and the facility had a functional landline telephone. Sharps, toxins, and medications were locked and inaccessible to residents in care. [Continued on LIC 809C] [Continuation of LIC 809] LPA also verified that the facility’s lease agreement, liability insurance, and administrator certification were current. Staff records showed all employees were fingerprint-cleared through Live Scan and had completed the required ongoing training. An overall facility inspection was initiated; however, due to time constraints, LPA was unable to complete the visit. LPA will return at a later date to conclude the remaining portion of the inspection. No deficiencies were cited during today’s visit. This report was reviewed with Administrator Ambalada. A copy of the report and Licensee Rights (01/2016) will be provided at the conclusion of the visit. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) David Roman conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA D. Roman was welcomed by, Director, Maniza Ambalada. LPA D. Roman provided a visual of CA State ID, identified himself as the LPA and discussed the purpose of the visit with Director, Maniza Ambalada. According to the facility’s license, the facility has a maximum capacity of six clients, of whom all may be non-ambulatory and one of which may be bedridden in bedroom 5. The facility is equipped with locked perimeters. LPA D. Roman along with Director, Maniza Ambalada toured the interior and exterior of the facility and inspected each room. The facility was sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the adequate furnishings. Doors, windows, toilets, and showers were in working order. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and resident activities. Cooking/dining equipment and utensils were present. There were no toxic chemicals/poisons accessible to clients. Medications were labeled, as required, and stored in locked areas. Water temperature was measured at 120 degrees F. No pools or bodies of water on the premises. LPA inquired if firearms or ammunition are kept at the facility, to which the Director denied. Fire Alarms and Carbon Monoxide detectors were tested at 10:00am, emergency lighting, and facility telephone were all working. Fire extinguishers were present. First aid kits were complete and readily accessible. Resident records reviewed had required documentation, Staff records reviewed CPR and First Aid certificates valid until 2027. An exit interview was conducted with Director, Maniza Ambalada, to whom a copy of this report and the Licensee/Appeal Rights were provided during the visit. Licensing Program Analyst (LPA) Tiffany Holmes conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was greeted and allowed entry into the facility by Ma.Niza Ambalada, Administrator to whom LPA discussed the purpose of the visit. According to the facility’s license, the facility has a maximum capacity of six (6) elderly residents. During today’s inspection, five (5) were at the facility. LPA, accompanied by the Administrator, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility’s ambient internal temperature was comfortable. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, or open-faced heaters accessible to clients. Medications were labeled, as required, and stored in locked areas. [CONTINUED ON LIC 809-C] [CONTINUED FROM LIC 809] No pools or bodies of water on the premises. Per Ma.Niza Ambalada, Administrator, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and facility telephone were all working. Fire extinguisher has been serviced. First aid kits were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. LPA interviewed staff present. LPA reviewed multiple staff and client records/files. Files reviewed contained required documents. Confidential records were stored in locked areas. Administrator presented proof of current/active business liability insurance and surety bond. No deficiencies were observed or cited during today's annual inspection. Allegation investigated: Facility is in disrepair Staff did not keep the facility free from cockroaches

Licensing Program Analyst (LPA) Amy Rodgers and Licensig Program Manders (LPM) Denise Powell made an unannounced visit to the facility to conduct a Complaint Visit . LPA Rodgers and LPM Powell were met by Maniza Ambalada, Administrator, and were granted entry into facility. LPA met with Ms. Ambalanda and discussed the purpose of the visit. At 11:05 am LPA and LPM toured facility and interviewed staff. LPA inspected the kitchen cabinets and drawers including food pantry areas. LPA did not observe any dead or alive cockroaches. Record review confirmed facility has ongoing pest control services and has added interior roach control. Facility interior appeared clean and in overall good condition. LPA and LPM inspected water leak area located inside the staff office. Repairs were made timely and no evidence of current leak, dry rot, or mildew was observed. Unsubstantiated continued 9099 Based on observations and record reviews there is insufficient evidence to support the allegations. The allegations are determined as unsubstantiated meaning they could have happened or be true, there is not a preponderance of evidence to meet the standard. An exit interview was conducted with Maniza Ambalada, Administrator, and a copy of this report along with the licensee rights was provided.
