On June 13, 2026, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Rose Matute administrator. LPA explained the purpose of today’s visit. The facility is licensed to serve (5) non-ambulatory and (1) maybe bedridden ages 60 and above. The facility is approved for (4) hospice waivers. Currently, there is (1) hospice resident in care. The facility is a single-story structure located in a residential neighborhood. The facility consists of (4) bedrooms, (2) full bathrooms, a patio area, a dining area, a living room, a kitchen, a laundry room, and (2) storage closets. LPA toured the physical plant. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of the visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. A water temperature of 113.9 degrees F. A comfortable temperature of 72 degrees F. was maintained in the facility. LPA observed the facility to be sanitary and appropriately furnished during the visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there are sufficient perishable and non-perishable food available and maintained adequately. The fire extinguisher was charged, and smoke detectors and carbon monoxide were operable. The facility conducted a Fire Drill 04/10/26. Evaluation Report Continues on LIC 809-C A review of Medication Records Administration (MAR) was observed to be maintained in order and accurate. The facility has a working landline telephone. The staff have all current CPR/First Aid Training on file. During the visit, LPA observed the facility's infection control practices. LPA observed staff following screening protocols for visitors, staff, and residents, sanitizing stations in common areas and restrooms. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). Mandate posters for inspection control were posted along with Facility Food Menu. An audit of residents #1-#6 (R1-R6) service records and staff #1-#6 (S1-S6) personnel records were revealed to be complete. The facility is current on CCL annual dues. The facility has a current administrator's certificate for Rosita Matute #7006130740 effective 10/16/2024 through 10/15/2026 and Jun Fausto Figueroa #7001877740 effective 11/25/2025 through 11/24/2027. The facility has a current liability insurance coverage policy #0100324656-1 effective 09/20/2025 through 09/20/26 No deficiencies cited during this inspection visit. On May 15, 2025, Licensing Program Analyst (LPA), Deborah Lee conducted an unannounced annual required visit using the CARE Inspection Tool. LPA Lee met with Jun Fausto Figueroa and the purpose of today’s visit was explained. The facility is licensed to serve 6 elderly adults ages 60 and above, of which 6 can be non-ambulatory. The facility has an approved hospice waiver for (4). A total of 6 residents are currently residing in this facility. Physical Plant/Structure : The facility is a one-story residential home consists of 6 resident bedrooms, (2) resident bathrooms, living rooms, dining room, kitchen, staff room, office area, storage area, attached garage washer and dryer room, backyard with table and chairs. LPA Lee and the Administrator toured the physical plant. There were no bodies of water or obstructions on the premises. Bedrooms: LPA inspected all 6 bedrooms; the mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked. Page 1 of 3 Bathrooms: Toilets and water faucets worked properly, grab bars were secure, and a non-skid mat was in place. Adequate lighting and toiletries accessible to residents. LPA tested hot water temperature, and it measured between 105 and 120 degrees Fahrenheit. This facility Common Rooms: All common areas are clean and free of obstructions. There is adequate seating for the residents in care. Each common area is equipped with furniture that is in good repair. Required Postings: LPA observed all required documents posted throughout the facility. The facility has a working landline telephone. Kitchen LPA observed a 2-day supply of perishable foods, and a 7-day supply of non-perishable foods properly stored, packaged, and labeled. Safety LPA observed 2 fully charged fire extinguishers last serviced on 02/26/25 The last emergency drill was conducted on 4/18/25. Smoke detectors were test and operational. File Review: LPA reviewed 6 resident files and found that ( 6 ) out of ( 6 ) had the required documents. LPA reviewed 4 staff files and found that ( 4 ) out ( 4 ) had the required documents, training, and certifications. LPA reviewed a copy of the facility’s Liability Insurance with expiration date of 9/20/25 Infection Control LPA observed required infection control signs posted throughout the facility. On 5/24/2024, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Jun Figueroa /Administrator. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) elderly adults ages 60 and above, of which (6) can be non-ambulatory. The facility has an approved hospice waiver for (4). The facility is a one-story residential home consists of (6) resident bedrooms, (2) resident bathrooms, living rooms, dining room, kitchen, staff room, office area, storage area, attached garage washer and dryer room, backyard with table and chairs. LPA Iniguez and the Administrator toured the physical plant. There were no bodies of water or obstructions on the premises. LPA inspected a total of (6) bedrooms and (2) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged from 113.5°F to 115.2°F, and the room temperature ranged from 76°F to 78°F. Storage areas for personal hygiene were in place. Sharp objects were stored in a way that made them inaccessible to residents in care. The kitchen was inspected, and there was sufficient perishable and non-perishable food available, which was adequately maintained. All fire extinguishers were charged and operable. The last Fire/Disaster Drills were conducted on 5/24/24. A review of (4) residents' service files and (4) staff personnel files was maintained in order. LPA reviewed (4) Medication Administration Records (MARs) and found no discrepancies. LPA observed the facility's infection control practices. All mandated inspection control posters were displayed throughout the facility. A copy of liability insurance was emailed to LPA. Facility Annual Fess current.
