Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced annual visit today and met with Harley Almira and Josephina Facultad, Caregivers, and the reason for the visit was explained. Joy Alipio, Administrator, was informed telephonically regarding today’s visit and arrived thereafter. The facility is licensed to serve (6) adults age 60 and over; (2) ambulatory and (2) non-ambulatory (room #2 and #3 cleared for 2 non-ambulatory and room #4 for ambulatory). Facility also has an approved Hospice waiver for (1) resident. There are currently (4) residents living in the facility. The facility is operating within the scope of its license. The facility is a single-story home located in a residential neighborhood of Long Beach and consists of the following: four (4) resident bedrooms, (3) bathrooms, living room with workspace for staff, kitchen, dining area, laundry area, an outside shaded seating area, and a detached garage. The following was observed during today’s visit: · Facility is clean and walkways, ramps and exits are free of debris and obstructions. Furniture throughout the facility is in good repair. Sufficient seating is available in the living room and dining area. Bedrooms have the required furniture such as beds, dressers, lamps, and chairs. The rooms have ample closet space. Beds have mattress pads and clean bedding. · Kitchen appliances are clean and were observed to be working properly. The home has sufficient 2-day perishable and 7-day non-perishable supply of food. Food is properly stored and kept within expiration limits. Knives and other sharps are locked and inaccessible to residents in care. Cleaning supplies and other toxins are also kept locked. ***Continues on LIC 809-C*** Bathrooms were inspected and observed clean and sanitary. Showers have anti-slip surfaces and safety grab bars. Water temperature was tested and measured within the required 105 - 120 degrees F. Sufficient personal hygiene supplies were observed. · The front and backyard are well maintained and there are no pools or large bodies of water. Garden is well maintained and outdoor passageways and ramps are kept free of debris and obstructions. Laundry area is clean and washer and dryer were observed to be operable during visit. · Smoke and carbon monoxide detectors were tested and are working properly. Fire extinguisher is charged and was last inspected on 7/9/2025. Fire/safety drills are conducted quarterly. Last fire/safety drill was conducted on 6/1/2026 with staff and resident participation. Emergency supplies are readily available for use, and the First Aid kit has the required tools and current edition of the First Aid manual. · Four (4) client and (3) staff files were reviewed and contained required documentation. Medication is kept centrally stored, locked and inaccessible to residents. Medication was observed to be administered and documented accordingly. Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies noted during today's visit. On 06/09/25, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with staff, Jimbo Paug and explained the purpose of today’s visit. LPA was granted access to the facility. Administrator Lodervine Alipio joined LPA for the inspection shortly after. The facility is licensed to serve two (2) ambulatory and (4) non-ambulatory residents aged 60 and above. Hospice waiver approved for one (1) resident. Currently there are four (4) residents in the facility. The facility is a single-story home located in a residential neighborhood. It consists of the following: four (4) resident bedrooms, three (3) bathrooms, of which two (2) are private, and one (1) is shared, a living room with workspace for staff, kitchen, dining area, laundry area, an outside shaded seating area, and a detached garage. LPA Gonzalez and Lodervine Alipio toured the entire physical plant. There were no bodies of water or obstructions on the premises. Exits/ Walkways around the home were free of debris and hazards. All residents’ bedrooms were inspected. The bedrooms had the required furniture, beds, mattresses, box springs and bedding were in good condition, adequate lighting was provided, and storage for resident’s personal belongings was observed. LPA observed the walls and floors were in good repair. Bathrooms were clean and operational and found to be within Title 22 regulations. The hot water temperature measured between 105.F and 120.F degrees. A comfortable temperature is maintained in the facility. LPA observed the facility to be sanitary and appropriately furnished at the time of visit. The storage area for personal hygiene, cleaning supplies, toxins, and sharp objects were stored and inaccessible to residents. Continued on LIC 809-C The kitchen was inspected, and sufficient perishable and non-perishable food was available and maintained properly. Fire extinguishers were charged, and smoke detectors and carbon monoxide were tested and operable. A working landline phone is available. A first aid kit along with manual was fully stocked and available. LPA noted that all required postings were on the walls. LPA conducted a review of four (4) resident records, and two (2) staff files, and observed them to be in order and complete. The Medication Administration Records (MAR) were reviewed and found to be maintained in order. Centrally stored medications are kept locked and inaccessible to residents. The facility is current with Community Care Licensing (CCL) annual fees. Liability insurance is active with expiration date of 07/01/25. No deficiencies were cited during this inspection visit. On 05/31/24 Licensing Program Analyst (LPA) Lizeth Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Director Lodervine Alipio as the purpose of the visit was explained. The facility is licensed to operate for (4) non-ambulatory and (2) ambulatory elderly adults ages 60 and above. The facility is approved for (1) hospice client, there are currently no hospice clients at the facility. Current facility census is 4, clients are Harbor Regional and South-Central Regional consumers. Facility fees are current, liability insurance is active with expiration date of 07/01/24. The facility is a single-story structure located in a residential neighborhood. It consists of the following: (4) clients' rooms, (3) bathrooms of which (2) are private and (1) is shared, a living area with a work space for staff, a dining area, a kitchen, a laundry area, an outside seating area, and a garage that houses emergency supplies and an additional freezer. Client bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F.. A supply of perishable and non-perishable food was observed, toxins and knifes were stored and inaccessible to clients, no weapons nor bodies of water on the premises, exits and walkways are free of debris/hazards. LPA conducted a records review of 2 staff records, 2 client records, and 2 medication administration records, no discrepancies observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last drill was conducted on 03/23/24, 1 fire extinguisher fully charged, carbon monoxide and smoke detectors are interconnected and operational, land line observed.
