On July 11, 2026, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced annual required visit with a primary focus on Infection Control measures using the new CARE Inspection Tool. LPA met with assistant administrator Anchie Reyes and explained the purpose of today’s visit. The facility is licensed to operate for six (6) non-ambulatory elderly residents ages 60 and above. The facility is approved for two (2) hospice residents. Currently, there are no hospice residents in care. The facility is a single-story structure located in a residential neighborhood. It consists of the following: five (5) resident's rooms, two (2) common bathrooms, living area, dining area, kitchen, and outside covered patio area. LPA toured the physical plant. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for resident personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured 105.9 F. A comfortable temperature of 72 degrees was maintained in the facility. LPA observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene were stored and not accessible to residents. The kitchen was inspected and there is sufficient perishable and non-perishable food available and maintained properly. The facility has two (2) fire extinguishers that were charged. A working landline telephone remains available. Evaluation Report Continues on LIC 809-C A review of Medication Records Administration (MAR) and Fire Drills were observed to be maintained in order and accurate. The facility conducted Fire/Safety Drill on 05/01/26. During the visit, LPA observed the facility's infection control practices. LPA observed staff followed screening protocols for visitors, staff, and residents, sanitizing stations in common areas and restrooms. Posters mandated for inspection control were posted. An audit of residents #1-#6 (R1-R6) service records and staff #1-#6 (S1-S6) personnel records revealed to be complete. The facility has current liability Insurance policy # 0100230231-3 Effective 03/10/26 - 03/10/27 The facility is current on CCL annual dues. The facility has a current administrator's certificate for Anchie Reyes #7010807740 valid through 01/11/28. DEFICIENCIES: During a health and safety inspection, LPA observed at 11:30 am the exterior side gate with a padlock. LPA observed all (5) residents rooms with surveillance cameras and the facility had no waiver request or updated plan of operation to include the use of cameras in private rooms. On July 16, 2025, Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced annual required visit using the CARE Inspection Tool. LPA was met by Anchie Reyes Administrator and the purpose of today's visit was explained. The facility is licensed to operate for six (6) non-ambulatory elderly residents ages 60 and above. The facility is approved for two (2) hospice residents Physical Plant/Structure: The facility is a single-story structure located in a residential neighborhood. It consists of the following: 5 resident bedrooms, 2 common bathrooms, living room, dining room, kitchen, detached garage and outside shaded patio area with table and chairs. LPA inspected all 5 bedrooms; the mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked. Page 1 of 3 Bathrooms: Toilets and water faucets worked properly, grab bars were secure, and a non-skid mat was in place. Adequate lighting and toiletries accessible to residents. LPA tested hot water temperature, and it measured 107.6 degrees Fahrenheit. Common Rooms: All common areas are clean and free of obstructions. There is adequate seating for the residents in care. Each common area is equipped with furniture that is in good repair. Required Postings: LPA observed all required documents posted throughout the facility. The facility has a working landline telephone. Kitchen: LPA observed a 2-day supply of perishable foods, and a 7-day supply of non-perishable foods properly stored, packaged, and labeled. Sharps were locked away and inaccessible to the residents. Safety LPA observed 2 fully charged fire extinguishers last serviced on 8/30/24 The last emergency drill was conducted on 5/14/25. First aid kid had all required items. File Review: LPA reviewed 3 resident files and found that ( 3 ) out of ( 3 ) had the required documents. LPA reviewed 4 staff files and found that (4 ) out ( 4 ) had the required documents, training, and certifications. Page 2 of 3 Medications LPA observed all centrally stored medications were secured and locked. All medications were observed in their original packaging. Infection Control LPA observed required infection control signs posted throughout the facility. LPA observed a sanitizing stations in entryway of facility. LPA observed an ample supply of PPE supplies. Facility maintains General Liability Insurance which expires 3/10/26. There were no deficiencies cited during today's visit. On 07/02/2024, Licensing Program Analyst (LPA) Mario Leon conducted an unannounced annual required visit using the CARES Inspection Tool. LPA met with Anchie Reyes, Administrator (S1) and explained the purpose of today’s visit. The facility is licensed to operate for six (6) non-ambulatory elderly residents ages 60 and above. The facility is approved for two (2) hospice residents The facility is a single-story structure located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, two (2) common bathrooms, living room, dining room, kitchen, detached garage and outside shaded patio area with table and chairs. LPA toured the physical plant. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for resident personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured 110.5 degrees F. and a comfortable temperature of 69.8 degrees F was maintained in the facility. LPA observed the facility to be sanitary and appropriately furnished at the time of visit. Medications were centrally stored, locked and inaccessible to residents. Storage areas for sharps and personal hygiene were observed and not accessible to residents. The kitchen was inspected and there is sufficient perishable and non-perishable food available and maintained properly, with additional freezer stored in detached garage. The facility has two (2) fire extinguishers that were both fully charged, as of 07/17/2023. A working landline telephone remains available. LPA observed First Aid Kit was maintained. The last emergency disaster drill was conducted on 05/30/2024. The facility had operational smoke and carbon monoxide in bedrooms and common areas. The facility has current liability insurance on file, effective 03/10/2024 through 03/10/2025. An audit of residents #1-#6 (R1-R6) service files and medication administration record (MAR) and staff #1-#3 (S1-S3) personnel files revealed to be complete. Report continues, see LIC809C. During today's visit, there were no deficiencies cited.
