On 12/3/2025, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Anchie Reyes /Administrator. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) elderly adults aged 60 and above, of which (2) can be non-ambulatory and (1) bedridden. The facility has an approved hospice waiver for (3). Currently the facility has (6) non-ambulatory residents. The facility is a one-story residential home consists of (5) resident bedrooms, (2) resident 1 staff office and 1 staff room, living room, kitchen, dining area and laundry room and backyard with shaded area. LPA Iniguez and the Administrator toured the physical plant. There were no bodies of water or obstructions on the premises. LPA inspected a total of (5) bedrooms and (2) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged from 113.5°F to 115.2°F. The evaluation Report continues on the next page, LIC 809-C, providing further details of the inspection findings. During the visit, LPA Iniguez observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored in a way that made them inaccessible to residents in care. The kitchen was inspected, and there was sufficient perishable and non-perishable food available, which was adequately maintained. All fire extinguishers were charged and operable. The last Fire/Disaster Drills were conducted on 8/11/25. A review of (4) residents' service files and (4) staff personnel files was maintained in order. LPA reviewed (4) Medication Administration Records (MARs) and found no discrepancies. LPA observed the facility's infection control practices. All mandated inspection control posters were displayed throughout the facility. A copy of liability insurance was emailed to LPA. Facility Annual Fess current. Deficiency cited under California Code of Regulations, Title 22, Division 6, Chapter 8. See details below: -Facility exceeded fire clearance for non-ambulatory, current clearance is (2) number of non-ambulatory at the moment are (6). Immediate Civil penalty rendered for $500.00. -Facility doing construction inside. Note: *Citations not cleared by the due date will be a $100 fine assessed for each citation until it is cleared. On November 8, 2024, Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced required annual visit using the CARE Inspection Tools. LPAs met with Anchie Reyes and explained the purpose of this visit. The facility is licensed for (6) 60-year-old or older non-ambulatory adult clients; 1 may be bedridden. Currently, there are (5) non-ambulatory residents and (1) ambulatory residing in the facility. Structure The facility is a one story residential home consists of (5) resident bedrooms, (2) resident 1 staff office and 1 staff room, living room, kitchen, dining area and laundry room. No weapons are stored in the premises. Physical Plant LPA and Administrator Anchie Reyes toured the facility inside and outside. LPA observed There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the resident's personal belongings was observed. LPA observed that facility had required postings: Facility license, personal rights, ombudsman information poster, facility sketch, exit signs, infectious disease postings, Administrator certificates, an emergency disaster plan. Bedrooms LPA inspected all (5) bedrooms--1 shared, and 4 private. All bedrooms were observed to have the required furniture including beds, dressers, night stands with lamps, chairs, and ample storage space for personal belongings. All bedrooms were observed to be clean, in good repair, and have ample lighting. page 1 of 3 Bathrooms LPA inspected the facility bathrooms. In the resident’s bathroom the toilet, faucets, and shower were fully operational. All safety handrails were securely fastened. LPA observed the showers to be clean and free of mold or mildew. The shower had a nonskid material in bottom and shower chair. Resident’s toiletries and incontinent supplies are secured in hall closet. The water temperature measured 112.2 degrees Fahrenheit. All bathrooms were observed to be clean, in good repair and within Title 22 regulations. Linens & Hygiene LPA observed all beds to have the required linens including mattress cover, fitted sheets, blanket, comforter, and pillow. LPA observed an ample supply of linens, towels, and blankets in hall closet. Kitchen LPA inspected the kitchen and observed all appliances to be in good working repair, including stove/oven, microwave, dishwasher, refrigerator. LPA observed an ample supply of cutlery, pots, pans, and bowls to be in good repair. LPA observed knives and additional sharps to be secured in locked cabinet in the kitchen and are inaccessible to residents. LPA observed a 3-day supply of perishable foods and a 7-day supply of nonperishable foods. There is a laundry room where the washer and dryer are located. The detergent and other cleaning supplies are locked in a cabinet inaccessible to residents in care. Common Rooms In the living room, LPA observed 5 recliners and a mounted TV. The formal dinning area dining contained a large 8 seated dining table, with extra chairs. Page 2 of 3 Safety LPA observed and tested smoke/carbon monoxide combo detectors to be fully operable. LPA observed ( 3 ) fully charged fire extinguishers that was last serviced on 08/30/2024. The last emergency drill was conducted on 08/03/2024. LPA inspected the First Aid kit and found it contained an ample supply of required items: Scissors, tweezers, gauze, disinfectant wipes, band aids. LPA observed all exits to be clear and easily accessible. All toxins locked and inaccessible to residents in care. Medications LPA observed all centrally stored medications in their original packaging and are secured in a locked cabinet that is inaccessible to Residents in care. Files LPA reviewed ( 6 ) resident files and found that (6) out of (6) contained all the necessary documentation. LPA reviewed (4) staff files and found that (4) out of (4)contained the required documentation, certification, and training. Liability Insurance expires on 03/10/2025. Infection Control During the visit, LPA observed the facility’s infection control practices. LPA observed a sanitizing station at the facility entrance. PPE supplies are readily available to staff. Outside area: During visit LPA observed the outside grounds (front and back) to be free of clutter, debris, and passage ways was free of obstruction. There were no deficiencies cited during today’s visit. An exit interview was conducted with Anchie Reyes and a copy of this report was provided.
