Type of visit: Required - 1 Year

On March 3, 2026, Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced required visit using the CARE Inspection Tool. LPA was greeted by staff and granted entry after stating the purpose of the visit. Direct Care Staff Juliet Gines arrived shortly after to assist with the facility inspection. Staff contacted Assistant Administrator Maria Tavares who arrived later to facility. The facility is licensed for six (6) non-ambulatory residents of which one (1) may be bedridden with approved hospice waiver for four (4) residents. Currently, there are three (3) Hospice residents present during today’s visit. This is a single story with a detached garage facility. The facility has five (5) resident bedrooms (four private and one shared) and three full bathrooms. LPA conducted a tour of the physical plant accompanied by Juliet Gines and Maria Tavares, and the following was observed: There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, storage and closet space was available for the resident’s personal belongings was observed. Bed linens, comforters, and bath towels were available during the visit. Bathrooms were operational with water temperature measured at 116.6 degrees F. A comfortable temperature of 75 degrees F. was maintained in the facility. LPA observed the facility to be furnished at the time of the visit. Storage areas for cleaning supplies and sharps objects were stored and not accessible to residents. The kitchen was inspected, and facility has sufficient two day perishables and seven day non-perishable food was maintained adequately. Facility has supply of emergency food and water. CONTINUED ON 809C Facility has one fire extinguisher which was mounted and fully charged. During the visit, LPA observed screening protocols for visitors, staff, and residents. LPA observed the facility has a supply of Personal Protective Equipment (PPE). Mandated inspection control posters were posted. LPA observed First Aid Kit was maintained. The last fire drill was conducted on February 8, 2026. The facility had operational smoke and carbon monoxide detectors in bedrooms and common areas. The facility has current liability insurance on file effective 4/25/25 – 4/25/26. The facility is current on Community Care Licensing annual dues. A review of six residents (R1-R6) service files and three staff (S1-S3) personnel files revealed to be complete. The facility has the current administrator's certification on file for Maria Jesusa Gonzales # 6075311740 - Expiration 02/13/2027. No deficiencies during this inspection visit. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA was greeted and granted entry by Staff Juliet Gines and the purpose of the inspection was discussed. Administrator (AD) Ricardo Banos and Assistant Administrator (AAD) Maria Tavarez were contacted by phone and arrived at approximately 9:30 a.m. During the inspection, LPA and Staff Gines conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, garage and observed the following: This is a one-story home with five resident bedrooms, one staff bedroom, three bathrooms, living room, dining room, kitchen, and detached two-car garage. All resident bedrooms had the required furnishings. LPA observed all resident beds had linens and blankets. LPA observed all windows were screened. The backyard has a shaded sitting area. LPA observed residents watching television in the living room and resting in their respective bedrooms. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested at 110.8 degrees Fahrenheit. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged with service tag dated March 5, 2025. Electric stove, microwave, washer, and dryer were all inspected and observed to be operable. Toxic chemicals, cleaning solutions, and disinfectants were observed to be inaccessible to residents. Medication was observed to be centrally stored and locked. LPA reviewed four of four resident files and three staff files. LPA interviewed two residents and two staff. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: Required - 1 Year

On June 20, 2024, at 1:05pm, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim met with Assistant Administrator (AA) Maria Tavarez and explained the purpose of the visit. The facility is licensed to operate for six (6) non-ambulatory of which one may be bedridden and have a hospice waiver for four (4) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, one (1) staff bedroom, three (3) bathrooms, living room, dining room, kitchen, detached 2-car garage, and an outside covered patio area. LPA Kim toured indoor and outdoor of the physical plant with AA Tavarez. There are no obstructions or bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. All bedrooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, Resident Room 5, and Staff Room 1. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 111.3 and 114.0 degrees F. A comfortable temperature of degrees 71 degrees F was maintained in the facility. LPA Kim observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available and maintained properly. Emergency food, water, supplies are stored in the family room. The facility has one (1) fire extinguisher that is charged and was serviced on March 4, 2024, and smoke/carbon monoxide detectors were operable. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). The facility conducts Fire/Safety Drill quarterly and was last conducted on May 21, 2024. A working telephone (562-296-8871) remains available. First Aid Kit contained all the necessary elements. LPA conducted an audit of resident files (R1-R6), staff files (S1-S7) and medication and medication administration review that were all in order and complete. LPA Kim conducted 4 resident interviews and 4 staff interviews. A technical violation was assessed during this inspection visit according to the California Code of Regulations (Title 22, Division 6, Chapter 8).
