Type of visit: Required - 1 Year
On July 2, 2026, Licensing Program Analyst (LPA) Kevin Mknelly arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with designee and explained the purpose of the visit. Administrator was unavailable to attend. LPA recommended designees coordinate with Admin for detailed plan with caregivers in the event Admin advice is needed while Admin is away. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. The home is very clean and residents stated they are happy with care. Overnight awake ON caregiver appears to be needed for current residents LPA reviewed 4 resident files. Files are complete and well organized. deficencies noted. LPA reviewed staff files. Files are complete. increased training advised LPA requested the following documents to update the facility file: Liability Ins certificate, LIC 500, As a result of this inspection, the following deficiencies were cited on 809-D, per Title 22 Regulations, Division 6. (A)This poses an immediate Health and Safety risk to clients/residents in care. Allegation investigated: Staff lock resident in their room for extended periods of time while in care. Staff do not provide adequate care and supervision to resident in care.
On 4/22/26 , Licensing Program Analyst (LPA) Kevin Mknelly LPA Mknelly arrived and met with Administrator to deliver investigation findings. LPA reviewed resident records, and conducted interviews. LPA finds that facility met Tittle 22 requirements. Three of 5 residents present were observed to have care needs met. 2 residents are in hospital. LPA intended to interview R1. R1 has voluntarily moved to a private residence. LPA spoke by phone with a housing navigator for R1. R1's location and contact could not be disclosed. Navigator, however, provided information of R1's status and that allegations were fabricated in order to be removed from the home. Navigator expressed utmost praise for the care R1 received at the home. This agency has investigated the above complaint allegations. We have found that the complaint is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint. Type of visit: Case Management - Annual Continuation
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management annual continuation visit. LPA met with staff and explained the purpose of the visit. Today's visit, LPA provided facility with a new license for licensure of four non-ambulatory and two bedridden. At this time, facility has an additional pending fire clearance. Once approved and/or denied, LPA will submit a notification letter to the facility along with new license. LPA and staff conducted a tour of the interior of the facility to ensure the health and safety of residents in care. LPA observed clients in their private rooms. LPA observed facility to have at least two days of perishable and seven days of nonperishable foods. LPA observed sharps, medications, and toxins to be locked and inaccessible to residents in care. LPA observed facility to be at a comfortable temperature of 75* F. LPA observed facility to be clean, free of malodor, and in good repair. LPA conducted a file review of staff records. Additionally, LPA reviewed Emergency Disaster Plan LIC610E revealed it was reviewed by Administrator for the year of 2025. Fire Drill records revealed it was recently conducted on February 15, 2025 and July 21, 2025. CARE tool was completed with staff. As a result of today's visit, no deficiencies cited. Type of visit: Required - 1 Year
On July 15, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with co-Administrator and explained the purpose of the visit. Today's census is six residents in care with zero residents on hospice. Facility is licensed for six non-ambulatory with hospice waiver for four. During today's visit, LPA and co-Administrator conducted a file review for six residents in care. LPA observed R1 to be recently admitted to the facility but did not observe a medical assessment on file. LPA was informed admission agreement and medical assessments may be with Administrator. LPA observed R2's LIC 602 is on file which indicated R2 is bedridden. LPA did not observe R3's LIC 602 on file as facility had provided the original copy to emergency medical services, but co-Administrator has an electronic copy. Electronic copy indicated R3 is bedridden. LPA was unable to complete today's inspection due to lack of time. LPA will return at a later day to complete inspection and inspection tool. Allegation investigated: Resident sustained unexplained injury in care Facility staff handled resident in a rough manner.
On xx/xx/xx, Licensing Program Analyst (LPA) Kevin Mknelly conducted an unannounced complaint investigation visit to deliver the findings for the above allegations and met with Caregivers/ designees. Administrator was informed of the visit. LPA conducted records review and interviews. LPA is unable to find and or meet the preponderance, per policy. On 7/26/24, LPA conducted an investigation visit, observed residents in care and conducted interviews. R1 was interviewed and provided an explanation of an incident where they sustained a black eye that was different than her previous accounts. R1 was onserved to be treated respectfully by caregivers. R2 was unable to participate in an interview as they were asleep. At todays visit, R1 is currently hospitalized for observation and treatment of frequent behavioral incidents. R2 has relocated to another of the licensee's care homes. Unsubstantiated Observations, interviews and records showed that R1 and R2 have issues frequently addressed by caregivers according to resident care plans. As a result of this investigation, LPA finds allegation to be (US)Unsubstantiated - A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Melissa Parks arrived on Tuesday July 16, 2024 to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA Parks reviewed resident (6) and staff files (2). All resident files contained the required paperwork. Staff files contained the required paperwork and training. LPA Parks and staff Sheree toured the facility together to ensure the health and safety of residents in care. The areas toured included staff room, resident rooms, bathrooms, living room, kitchen, garage, and backyard. In the areas toured, there were no health or safety violations observed. Facility was clean and well organized. All required posting were observed. All knives/sharps are kept locked and inaccessible to residents. Chemicals were kept locked in the laundry room. First aid kit was fully stocked. Fire extinguisher had a current inspection tag. No deficiencies cited. Exit interview conducted. Allegation investigated: Personal rights violations Staff denied residents' food. Staff did not ensure that a resident's hygiene needs were being met.
On 5/1/24 , Licensing Program Analyst (LPA) Kevin Mknelly LPA Mknelly arrived and met with caregivers. The Administrator arrived for LPA to investigate and deliver investigation findings. LPA reviewed facility records, and conducted and extensive interview with R1. LPA finds that facility met Tittle 22 requirements. LPA reviewed R1's LIC 602 and interviewed R1 10-11 AM, privately in R1's room. LPA and R1 discussed a number of R1's medical conditions and things that R1 wishes to resolve. R1 was questioned about the allegations and stated that his care is good but he would like to receive additional therapy for what he perceives are issues effecting his mobility and independence. R1 demonstrated a lack of insight to his current diagnosis and abilities. R1 stated they are happy with the care they recieve in this home. This agency has investigated the above complaint allegations. We have found that the complaint is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint.
