Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Melissa Parks and Investigator Vincent Moleski arrived on Tuesday December 30, 2025, to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA and Investigator Moleski reviewed resident (6) and staff (3) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training. LPA, Investigator Moleski and Administrator Virginia toured the facility together to ensure the health and safety of residents in care. The areas toured included the following areas in resident bedrooms, bathrooms, kitchen, laundry room/bathroom, and backyard. Facility had current inspection tag on fire extinguisher. Water temperatures were within the required range. Facility had a fully stocked first aid kit. All required postings were observed. Facility was current on fire drills. See 809 D for deficiencies cited during todays visit. Exit interview conducted. Appeal rights were given. Allegation investigated: Staff hit a resident in care. Staff verbally abused a resident in care.
On Aug. 28, 2025 , Licensing Program Analyst (LPA) Kevin Mknelly LPA Mknelly arrived and met with Administrator to deliver investigation findings. LPA reviewed staff records, facility records, and conducted interviews. LPA finds that facility met Tittle 22 requirements. LPA interviewed facility staff and residents identified in the complaint. R1 recanted their statements of verbal abuse. R1 stated that they were confused and upset at the time of past statements. R1 stated that they get good care and have good relationship with staff. R2 denied staff have touched then in an aggressive way, R2 likes the care they recive and staff are respectful. This agency has investigated the above complaint allegations. We have found that the complaint is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 1/14/25 to conduct a Annual Inspection utilizing the CARE inspection tool. LPA met with Administrator and Licensee arrived to assist. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. Licensee agrees to repair weathered gate, a recently cracked front window and has adjusted hot water temperature to be between 105 and 120' F. Licensee will post See Something/ Say something poster measuring 20x26 in. LPA reviewed staff and resident files and they were complete and up to date. LPA greeted residents who stated satisfaction with care. LPA received LIC 500 and Admin cert. Updated plan of operations to be emailed to LPA. Allegation investigated: Staff does not allow resident to go outside. Staff refused to provide resident's records
Licensing Program Analyst (LPA) Cassie Yang arrived uannounced at the facility to open a complaint the Department received regarding the allegations cited above. LPA met with Caregiver and explained the purpose of the visit. Caregiver then contacted Administartor via telephone, who arrived to the facility shortly afterwards. During today's investigation, LPA conducted extensive interviews and file review. The results of the allegation are as follow on LIC 9099-C. Unfounded **LIC 9099-C** Allegation: Staff does not allow resident to go outside. The Department conducted interviews. Based on interview with R1, it revealed staff does assist R1 to go outside if needed. R1 stated R1 has frequent bladder impairment so they do not go outside much but R1 likes to sit at the porch occasionally. Interview with Administrator, it revealed right now it is too hot to go outside but if requested facility complies. It further revealed that if R1 has doctors appointment, facility does assist with transportation. Administrator stated all residents in care are free to leave the facility to stay outside if desired. Based on information obtained through interviews, the allegation listed above is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. Allegation: Staff refused to provide resident's records The Department conducted interviews. Based on interview with R1, it revealed R1 is unsure of this allegation. Interview conducted with Administrator revealed that another assisted living facility had inquired for R1's facility documents through a caregiver who did not understand the request. Administrator explained that fax number was then provided to R1 to provide to the assisted living facility for the request to be faxed to the facility. Administrator stated the request included R1's LIC602A which Administrator then inquired a new one from R1's primary care physician. Administrator explained that records were not refused but it is still in progress. Records reviewed revealed Administartor had submitted a request for updated LIC602A on July 26, 2024 Based on information obtained through interviews and file review, the allegation listed above is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. Type of visit: Case Management - Deficiencies
On 8/1/2024, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding a deficiency LPA observed. LPA met with Caregiver and explained the purpose of the visit Caregiver then contacted Administrator via telephone. Today's census is five residents in care with zero of hospice services, facility is licensed for six non-ambulatory hospice waiver of three. During today's visit, LPA conducted a tour of R1's bedroom and observed the exterior door pathway to be obstructed with a mattress and a wheelchair. LPA took a photo of the following observation. When discussed with Administrator, she explained the items had been placed there for approximately two weeks now but it will be removed immediately. LPA and Administrator additionally discussed that fire doors are regulated to be closed at all times. LPA informed Administrator if wished to be open, an automatic door holder will need to be installed connecting to the fire system. As a result of today's observations, deficiencies were cited for violation of CCR Title 22, Chapter 8. Type of visit: Required - 1 Year
On 1/24/2024, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the full care tool. LPA met with caregiver who contacted Administrator who arrived to the facility shortly afterwards. During today's inspection, LPA observed the license posted to be inaccurate to the license on FAS. LPA printed, signed and provided facility with the update license. Facility is licensed six non ambulatory residents, hospice waiver of three. Today's census is four with no residents on hospice services. LPA and Administrator conducted a tour of the facility. Areas toured included but not limited to: kitchen, residents room, staff room, bathroom, and the common areas. LPA observed facility to have the required posters posted in the common area. LPA observed medications, sharps, toxins to be locked and secured. LPA and Administrator discussed ensuring to lock laundry room at all times. LPA observed facility to be at a comfortable temperature of 71*. LPA observed facility to have 2+ days of perishable and 7+ days of nonperishable foods. Administrator Certificate posted for #6016228740 is active with expiration date of 06/07/2024. LPA obtained a copy of facility liability insurance, effective date of 3/26/2023 to 3/26/2024. LPA conducted a file review of R1, R2, R3 and R4. LPA and Administrator completed the inspection tool and found the facility to be in compliance. No deficiencies cited. Type of visit: Case Management - Legal/Non-compliance
On 10/27/2023, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced to conduct a required quarterly case management visit in accordance with the Stipulation and Waiver; And Order, effective date 02/07/2022 to 02/07/2025. LPA met with Administrator, Virginia Bonite, and explained the purpose of the visit. LPA and Administrator conducted a tour of the facility to ensure the health and safety of residents in care. LPA observed five residents to be present in their rooms. LPA and Administrator discuss facility's previously submitted request for probational term reduction. LPA informed Licensee the request is still under review by the Department. Administrator was informed LPA will update Licensing Program Manager Anthony Perez of today's visit. LPA observed the stipulation to be posted in a conspicuous space. LPA conducted a file review of facility's Stipulation binder. LPA observed monthly training documentation for August 2023, September 2023 and October 2023. LPA observed staff certificate of completion for the following training topics: - August 2023: Wandering Within and Outside of the Facility - September 2023: Memory Behavior - October 2023: Special Needs of Persons With Alzheimer's Disease and Dementia. At this time, LPA found facility to be in compliance with the Stipulations and Waiver; And Order. No deficiencies observed.
