Licensing Program Analyst (LPA) Graham Gunby arrived at the facility unannounced on 08/26/2026 to conduct a Required-1 Year Inspection utilizing the inspection tool. LPA met with Administrator, Mihail Mehedinti, and explained the purpose of the visit. LPA reviewed a total of two (2) resident files. Resident files contain signed admission agreements, physician's reports, identification sheets, releases, and resident rights. Medications are centrally stored, locked, and appear to be given per doctor order. R1's medical assessment specified that the resident is currently bedridden, though the facility does not have bedridden fire clearance. LPA reviewed a total of two (2) staff records. Staff is CPR certified. S2 has not completed annual training. LPA and administrator toured the interior and exterior of the facility to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathrooms, kitchen, and backyard. LPA observed required furniture, and lighting throughout the residents' bedrooms and facility. LPA observed food supplies of non-perishables for a minimum of one week and perishable foods for a minimum of two days. Toxic and cleaning supplies were locked and is inaccessible to residents in care. The hot water temperature was measured in the kitchen sink at 114.4 degrees Fahrenheit. First aid kit is complete. LPA observed fire extinguisher/carbon monoxide alarms to be in working order and the fire extinguisher to be last service 05/07/2026. Scissors were found in an unlocked bathroom drawer. Licensing Program Analyst (LPA) Angela Hood arrived at the facility unannounced and met with the Administrator, Mihail Mehedinti, to conduct a Required-1 Year Inspection. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. There are three (3) bedrooms and one (1) bathroom for resident use. LPA observed bedrooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 111.2 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two-day perishable and (7) seven-day non-perishable food supply on hand. LPA observed knives, cleaning products and other toxins to be locked away and inaccessible to residents. LPA observed the outdoor area and perimeter of the care home to be free of clutter and debris and there appeared to be no potential safety hazards to the residents in care. Smoke detectors and carbon monoxide detector are operational. Fire extinguisher and first aid kits are maintained and ready for emergency use. LPA checked medication storage and found medication to be locked away and inaccessible to the residents. LPA reviewed three (3) resident files and also reviewed two (2) staff files. Licensee provided a copy of certificate of liability insurance. As a result of this visit, no deficiencies were cited per California Code of Regulations, Title 22. Licensing Program Analyst (LPA) Melissa Parks arrived on Monday August 12, 2024 to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA Parks reviewed resident (3) and staff files (2). All resident files contained the required paperwork. Staff files contained the required paperwork and training. LPA Parks and Administrator Mihail toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathroom, living room, kitchen, and backyard. In the areas toured, there were no health or safety violations observed. Facility was clean and well organized. Facility was current on fire drills. First aid kit was fully stocked. All required posting were observed. All knives/sharps were kept locked and inaccessible to residents. Fire extinguisher had a current inspection tag. No deficiencies cited. Exit interview conducted. Allegation investigated: Staff slapped resident.

On 3/19/24, Licensing Program Analyst (LPA) Kevin Mknelly conducted an unannounced complaint investigation visit to deliver the findings for the above allegations and met with the licensee. LPA conducted records review and extensive interviews. LPA is unable to find and or meet the preponderance, per policy. Information from a treating hospital for R1 was that when R1 was examined on 12/28/24, “No new signs of trauma were appreciated on my examination.” Family of R1 had been told by R1 that a staff at the facility had hit R1. R1 did not provide details of who allegedly hit them or when. The family member for R1 had also been contacted by S3. LPA interviewed S3. S3 and S1 made counter allegations about each others behavior. S3 did not observe S1 slapping R1. LPA Mknelly conducted a collateral visit to the new home that R1 was moved to. Unsubstantiated The Licensee at R1’s new home stated that R1 is demonstrating behaviors at that home too and has made vague allegations of staff hitting R1. The licensee has investigated the allegations and found them to not be credible. While this allegation is found to be unsubstantiated, there were some deficiencies observed to have occurred. Those deficiencies are noted in a separate report. As a result of this investigation, LPA finds allegation to be (US)Unsubstantiated - A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Type of visit: Case Management - Deficiencies

On 3/19/24, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with licensee. On 1/2/24, the department received a complaint regarding R1. While the complaint was unsubstantiated, additional deficiencies were found in the course of the investigation. In December 2023, statements by licensee and staff showed that S1 was employed for approximately 2 weeks and S2 was employed for 1 day. Department records showed that neither staff was associated to the home through the background checks system. Furthermore, the licensee did not have personnel files for either S1 of S2. It was also found that the licensee issued an improper eviction notice to R1’s, responsible party. The issuance of the notice failed to comply with regulation requirements. As a result of this inspection, the following deficiencies were cited on 809-D, per Title 22 Regulations, Division 6. (A)This poses an immediate Health and Safety risk to clients/residents in care.
