Allegation investigated: Facility shower is in disrepair
Licensing Program Analyst Hiratsuka, conducted this unannounced complaint visit. LPA toured the facility and interviewed staff. LPA observed the shower in the main shower room and it worked. Two staff members showed LPA the shower head worked and both stated there hasn't been any issues with it. LPA didn't observe any leaks or defects in the shower. There is a second shower in a private bathroom that is not in use at the moment because the resident who occupies the room is unable to use it. LPA toured the facility and did not observe any physical plant issues. Based on the above, the allegaion is unfounded. A finding that an allegation is UNFOUNDED means that the allegation is false, could not have happened, and/or is without a reasonable basis. Type of visit: POC
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a Plan of Correction visit regarding a deficiency cited on November 13, 2025. LPA met with staff and explained the purpose of the visit. During LPA's annual inspection conducted on November 13, 2025, file review of documents revealed R1 was bedridden but was residing in a non-ambulatory room. Facility is licensed for six non-ambulatory residents. Facility was cited the following Type A deficiency: 87202 Fire Clearance (a) All facilities shall maintain a fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. Prior to accepting or retaining any of the following types of persons, the applicant or licensee shall notify the licensing agency and obtain an appropriate fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. (2) Bedridden persons Plan of Correction was Licensee is to submit LIC 200 and updated facility sketch to LPA for a new fire inspection. The following is due November 14, 2025. As of today's visit, LIC 200 and facility sketch to request for one bedridden clearance has not been received. Today's visit, a failure to correct civil penalty was assessed. Type of visit: Required - 1 Year
On November 13, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with staff and explained the purpose of the visit. There are six residents in care with one resident on hospice services. Today's visit, LPA and staff conducted a tour together to ensure the health and safety of residents in care. Areas toured included but not limited to: resident private bedrooms, bathrooms, laundry room, garage, medication room, kitchen and the common areas. LPA observed resident rooms to be clean and free of dangerous items. LPA observed medication room to be locked and secured. LPA observed pantry to have seven days of nonperishable foods. LPA observed garage to have separate storage for toxins and foods. LPA observed at least two days of perishable foods stored in the garage refrigerator. LPA observed fire extinguisher present with recent service date. LPA and staff did not inspect the upstairs area as residents are prohibited from entering. LPA and staff discussed ensuring the fire doors remain closed at all times. File review was conducted for resident records and personnel records. Based on file review of resident records, it revealed resident (R1) has been living at the facility since 2022 but there are documentation of R1 being bedridden which is out of facility's licensure. CARE tool was completed and deficiencies was cited. Please see LIC 809-D. Type of visit: Case Management - Deficiencies
Licensing Program Analyst(LPA) Cassie Yang arrived at the facility to conduct a case management visit regarding deficiencies observed. LPA met with caregiver, who contacted newly appointed Administrator, who LPA explained the purpose of the visit to over the phone. The purpose of the visit was to issue citations for violations found in the meeting of 9/29/25. This license is in the process of change of ownership. During the process the licensee failed to follow the following regulations: 87355( c) Criminal Record Clearance (c) A licensee or applicant for a license may request a transfer of a criminal record clearance. By transferring ownership and installing Angela Chen as administrator, the licensee failed to transfer their clearance. 87155(a)(7) Application for License (7) Name and address of owner of facility premises if applicant is leasing or renting. This requirement was not met based on statements that the licensee did not maintain a title or lease for their licensed property. 87211(g) Reporting Requirements (g) The licensee shall notify the Department, in writing, within thirty (30) days of the hiring of a new administrator. This requirement was not met based on statements that a new administrator has been in place for greater than 30 days without notice to CCLD. Type of visit: Office
On September 29, 2025, a virtual office meeting was conducted via Teams, with Licensing Program Manager, Maribeth Senty and Licensing Program Analysts, Kevin Mknelly and Cassie Yang, was conducted with Administrator, Kevin Lee. Also in attendance was Maria Cucicea. The purpose of the meeting is to discuss the pending change of ownership. Items discussed and agreed to include: · The licensee shall provide written notice to the department and to each resident or his or her legal representative of the licensee's intent to change the use of the facility to a new licensee. · If the property is already sold and the transfer of the property has already been completed the licensee shall submit a lease agreement between the property owner and the current licensee (which may have a termination date of when/if a new license is granted). · Licensee will submit a designation of new administrator if applicable as well as a copy of the new administrator’s administrator certificate. · A new administrator or any new staff shall be on the current licensee’s staff roster. This can/ must be done by submitting a transfer request form of through Guardian. · Licensee will submit LIC 500s for the three homes under changes of ownership which include expected hours that the administrator will be present at each facility. · As the licensee you will need to provide CCL with copies of the 60 days' notices that were served to the residents and their responsible parties. Continue LIC 809-C LIC 809-C · The licensee agrees licenses are not transferable; therefore, the buyer of a facility must apply for and obtain a new license from the Department before the ownership of the facility can be transferred. · The licensee is still accountable for the facility until the licensure has been approved. · As a reminder, licenses are not transferable; therefore, the buyer of a facility must apply for and obtain a new license from the Department before the ownership of the facility can be transferred. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Angela Hood arrived at the facility unannounced on 11/15/24 and met with the caregivers to conduct a Required-1 Year Inspection. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. There are six (6) bedrooms and seven (7) bathrooms for resident use. LPA observed bedrooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 115.3 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two-day perishable and (7) seven-day non-perishable food supply on hand. LPA observed knives to be locked away and inaccessible to residents. LPA observed the outdoor area and perimeter of the care home to be free of clutter and debris and there appeared to be no potential safety hazards to the residents in care. Smoke and carbon monoxide detectors are operational. Fire extinguishers and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed three (3) resident files and also reviewed two (2) staff files. As a result of this visit, no deficiencies were cited per California Code of Regulations, Title 22. Allegation investigated: Staff did not issue a refund
On 7/23/24, Licensing Program Analyst (LPA) Kevin Mknelly spoke with Maria Cucicea by phone to open the complaint and to deliver complaint findings for the above allegation. LPA reviewed resident records and conducted and interview . LPA finds that the allegations cited above are substantiated. R1 movied into the facility on 5/7/23 and the contract was terminated on 5/23/24. LPA and Maria discussed fees charged and fees to be refunded. It was found that, through human error, the refund, paid today, exceeded the 15 days from the time of move out. As a result of this investigation, LPA finds allegation to be (S) Substantiated - A finding that the complaint is Substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. Admission agreement. (c) A refund of any fees paid in advance covering the time after the resident’s personal property has been removed from the facility shall be issued to the individual, individuals, or entity contractually responsible for the fees or, if the deceased resident paid the fees, to the resident’s estate, within 15 days after the personal property is removed. Plan of correction: Assistant administrator informed LPA that they are resolving the issue today. Licensee agrees that LPA will rceive confirmation for R1's POA that fees have been reimbursed by the POC date of 7/30/24.

Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Angela Hood arrived at the facility unannounced on 10/11/23 to conduct a Required-1 Year Inspection. LPA spoke with the Administrator, Maria Cucicea, by phone and was informed that the caregiver can conduct the tour and provide any necessary documentation for review. Caregiver will also sign the report. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. There are six (6) bedrooms and seven (7) bathrooms for resident use. LPA observed bedrooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 108.5 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two-day perishable and (7) seven-day non-perishable food supply on hand. LPA observed knives, cleaning products and other toxins to be locked away and inaccessible to residents. LPA observed the outdoor area and perimeter of the care home to be free of clutter and debris and there appeared to be no potential safety hazards to the residents in care. Smoke detectors and carbon monoxide detector are hard wired and operational. Fire extinguishers and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed two (2) resident files and also reviewed two (2) staff files. Facility provided LPA a copy of the certificate of liability insurance and LIC500. As a result of this visit, no deficiencies were cited per California Code of Regulations, Title 22.
