At approximately 8:30 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year visit and was greeted by Administrator Chin Razon. Bright Horizons Care Home - Santa Rosa is Licensed as a Residential Care Facility for the Elderly (RCFE). The facility is a single story ranch house. The facility has an approved fire clearance for four (4) non-ambulatory residents. The facility has a Hospice Waiver for two (2) residents. The facility has a dementia program on file. Upon arrival, LPA was informed that there were four (4) residents in care and two (2) staff members on-site. Four (4) residents were away at day programs during the inspection At approximately 8:45 AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 8:50 AM, LPA toured the facility with Administrator Razon. All exits were clear and unobstructed. The facility's one (1) fire extinguisher was last serviced and tagged on 3/20/2026. The facility's sprinkler system were inspected on 3/20/2026. The facility was sufficiently lighted. LPA inspected four (4) resident bedrooms and found all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Toxins were observed to be stored inaccessible to residents. The facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility is conducting fire and emergency drills monthly. The last disaster drill was conducted on 6/1/2026. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Continued on 809-C... ...Continued from 809 Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors were observed to be operational. At approximately 9:30 AM, LPA reviewed four (4) resident files. Four (4) of four (4) resident files were observed to be well organized and thorough with all required documentation. LPA reviewed five (5) staff files. Five (5) of five (5) staff files were observed to be well organized with all required documentation including First Aid and CPR certification and proper training documentation. LPA audited Medication for three (3) residents. LPA observed all medications to be centrally stored, secure and with proper documentation. Client’s monies for Personal and Incidental (P&I) items were audited and observed to balance to P&I logs. Chin Razon’s Administrator Certification 6075868740 is current with an expiration date of 2/26/2027. LPA requested the following documents be submitted to Community Care Licensing by 7/16/2026: LIC 500 Personnel Report LIC 610E Emergency Disaster Plan Proof of Liability Insurance The facility is in compliance. Type of visit: Case Management - Annual Continuation

At approximately 12:50 PM, Licensing Program Analyst (LPA) Frank arrived unannounced to continue a Required 1 Year visit and met with Facility Administrator Razon. LPA was informed that there are currently four (4) residents in care. During the 7/11/2025 initial visit, LPA reviewed four (4) resident files. Four (4) of four (4) resident files were observed to be well organized and thorough with all required documentation. LPA observed that a resident (R1) was diagnosed with a prohibited health condition. The facility did not request an exemption to keep R1 at the facility from Community Care Licensing (CCL). This deficiency will be cited. LPA reviewed three (3) staff files. All staff files were observed to be well organized with all required documentation including First Aid and CPR certification and proper training documentation. LPA spot checked Medication for two (2) residents. LPA observed all medications to be centrally stored, secure and with proper documentation. The facility does not handle resident’s monies for personal and incidental items. Chin Razon’s Administrator Certification 496804244 is current with an expiration date of 2/26/2027. Type of visit: Prelicensing

At approximately 8:20AM, Licensing Program Analyst (LPA) Felias arrived announced to conduct a Pre-Licensing Inspection and met with Applicants, Justine Silverio and Arnel Silverio. Upon arrival, LPA was informed that there were currently no residents in care. Facility received an approved fire clearance dated 04/02/2024 that allows for a total capacity of 4 non-ambulatory residents. Facility also has a hospice waiver for 2 individuals. Facility plans to care for older adults with intellectual and developmental disabilities. LPA conducted a walk-though of facility with Applicants and observed the following: Per facility sketch, facility is a one story residence with 4 resident bedrooms, two bathrooms, 1 staff room, and common areas. Facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Facility had emergency lighting. Facility has an Infection Control plan on file. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. Facility's hot water temperatures for all sinks were within Title 22 Regulations of 105F to 120F. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Mattress pads were in place or available for Resident use. Toxins were observed to be stored inaccessible to residents. Bathrooms were equipped with necessary grab bars, and non-slip floors/mats were present. All resident rooms were furnished per regulation with a bed, lamp, dresser, chair and bedside table. Facility has sufficient items for cooking and eating. Facility has a locked cabinet located in the kitchen for centrally stored medications. Facility files will be in a cabinet located in the front room. Facility has indoor/outdoor areas for visiting and activities. Facility's fire extinguishers were inspected May 2024. Facility's combination smoke and carbon monoxide detectors were tested and operational. LPA confirmed that contents of the facility's First Aid Kit were sufficient. Component III was reviewed with Applicants. No Deficiencies or Advisories given during visit. Pre-Licensing completed. Facility is ready to be Licensed as a Residential Care Facility for the Elderly. LPA will submit Pre-Licensing Application Report to the Application Unit Analyst in Sacramento. Application Unit Analyst will notify Applicant of Status. Exit interview conducted.
