Type of visit: Office

An office meeting was conducted today in the Santa Rosa Regional Office. Present in the meeting were: Licensing Program Manager (LPM) Victoria Bertozzi, Licensing Program Analysts (LPAs) Dina Alviso, Robert Frank, Marisol Cuadra, Licensee Sarah Lawrence, facility representatives Megan Lawrence-Ferreira, Lamon Lawrence and Jacqueline Lawrence via phone. The purpose of this office meeting is to have a conversation regarding the possibility of development a change of ownership plan for this facility and the Licensee's other facilities Sarah's Adult Residential Care #496800668, Sarah's Bancroft Drive Adult Home #496800869, Sarah’s Country Home #490108916, Sarah's Dutton Avenue Manor #496801432, Sarah's Group Home for Young Adults #490107871, and Sarah's Young Adult Home #496801313 in which Sarah Lawrence is the identified Licensee. Currently, there are no other areas of concern regarding care and supervision provided to residents in care. However, The Department learned about licensee's health incidents that could result in the licensee not been able to operate the facilities in the event of their absence. Based on records review, LIC309 Administrative Organization form indicates that Sarah Lawrence is the licensee for this facility and other facilities above mentioned where it is listed as a sole proprietorship. During today’s meeting it was discussed with licensee the possible option that they can choose to continue the operation of all the facilities. The current option discussed is the following: -Submit a change of ownership application to the Centralized Application Bureau (CAB) for each of the facilities indicating the percentage of ownership of each member. CAB contact information was given. -Provide an updated LIC500 reflecting who will be the designated administrator responsible for the management of the facility including sufficient number of hours to permit adequate attention to the management and administration of the facility by not later than 10/02/2026. Type of visit: Required - 1 Year

At approximately 8:30 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year visit. House Manager (HM) Kyra Perez arrived at 8:45 AM. Sarah's Retirement Home for DD Seniors is Licensed as a Residential Care Facility for the Elderly (RCFE). The facility is a single story ranch house. The facility has an approved fire clearance for six (6) non-ambulatory residents. The facility has a Hospice Waiver for two (2) residents. Upon arrival, LPA was informed that there was four (4) residents in care and two (2) staff members on-site. At approximately 8:50 AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 9:00 AM, LPA toured the facility with HM Perez. All exits were clear and unobstructed. The facility's two (2) fire extinguishers were last serviced and tagged on 1/21/2026. The facility was sufficiently lighted. LPA inspected four (4) resident bedrooms and found all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Toxins were observed to be stored inaccessible to residents. Facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility is conducting fire and emergency drills quarterly with the last disaster drill having been conducted on 12/12/2025. The facility is not keeping a required disaster drill log for drills prior to August 2025. A Technical Violation will be issued. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors were tested and observed to be operational. Continued on 809-C... ...Continued from 809 At approximately 9:50 AM, LPA reviewed four (4) resident files. Four (4) of four (4) resident files were observed with all required documentation. LPA reviewed four (4) staff files. One (1) of four (4) staff files (for staff member S2) was observed not to have proof of required annual training. This deficiency will be cited. One (1) of four (4) staff files (for staff member S1) was observed not to have a required LIC 503 Health Screening and proof of a negative Tuberculosis test. This deficiency will be cited. Two (2) of four (4) staff files were observed with all required documentation including First Aid and CPR certification and proper training documentation. LPA spot checked Medication for two (2) residents. LPA observed all medications to be centrally stored, secure and with proper documentation. Resident's monies for personal and incidental (P&I) items were audited and observed to match the P&I logs. Licensee Sarah Lawrence’s Administrator Certification 7036265740 is current with an expiration date of 10/31/2026. LPA requested the following documents be submitted to Community Care Licensing by 2/23/2026: LIC 500 Personnel Report LIC 610E Emergency Disaster Plan Proof of Liability Insurance Documents required to change Administrator on file (copy of documents required left with HM Perez) Request to change Email Address on file Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency, on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. Exit interview conducted. Copy of report, LIC-809Ds, LIC 9102, Plan of Corrections, 811 Confidential Names and Appeal Rights discussed and provided to HM Perez. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Alviso arrived unannounced to conduct a Required -1 Year inspection, on 1/23/25 at approximately 9:20am, and met with Sarah Araya, Administrator. LPA observed two caregivers on duty during the inspection. Currently there are four (4) clients in care. The facility has a fire clearance for six (6) non-ambulatory residents. Facility has a required infection control plan, and a required emergency disaster plan. Hospice waiver approval is for two (2) residents. Per record review, facility is holding quarterly emergency disaster drills as required. Fire extinguisher was serviced and tagged as required. LPA reviewed four (4) resident files. Medications, and medication records were reviewed. LPA reviewed four (4) staff files, including training. LPA toured the facility with the Administrator. Hot water was checked at 110.4 degrees Fahrenheit, which is within regulation. LPA observed sufficient food supply. LPA observed sufficient supply of paper products, cleaners/disinfectants, hygiene products, and personal protective equipment (PPE). Facility had sufficient lighting in resident rooms, common areas, bathrooms, and hallways for resident use. Bathrooms had required grab bars, and non-slip flooring/mats in showers/bathtubs for resident use. Medications are stored in a closet that is locked, making medications inaccessible to residents in care. Cleaners/disinfectants are kept locked and inaccessible to residents in care. LPA discussed regulation regarding fire clearance and ensuring all fire exits are fully cleared/unobstructed at all times as required. Administrator stated their understanding. Administrator had staff move the table from blocking part of the slider door exit during the inspection. Continued on LIC809C.. Continued from LIC809.. LPA is requesting the following documents by 2/23/25. LIC308 - Designation of Administrator Responsibility LIC500 - Personnel Report LIC610E-Emergency Disaster Plan (ensure to review and update as needed/required) Infection Control Plan (ensure to review and update as needed/required) Copy of LIC400 Handling of Client Cash Resources (include copy of surety bond if handling cash) Copy of Current Liability Insurance Resident Roster Copy of current Administrator Certificate. LPA observed the following deficiency:: LPA observed a made-up rollaway bed in an open storage room in the facility's back room/ common area. S1 told the LPA that the bed was staffs. The staff that come in and work nights use the bed, it is brought out into the common area. LPA discussed regulation regarding facility bedrooms, common areas, and awake night staff versus sleeping night staff. This deficiency will be cited, 87307(a) Personal Accommodations and Services- Living accommodations and grounds shall be related to the facility's function. The facility shall be large enough to provide comfortable living accommodations and privacy for the residents, staff, and others who may reside in the facility, see LIC809D. Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Cuadra arrived unannounced to conduct an Annual Required inspection and met with Administrator, Sarah Araya. Two residents were home during the visit. LPA/staff initiated a tour of the facility at 12:00pm and observed the following: Facility was a comfortable temperature and pathways were free from obstructions. Resident rooms were furnished per regulation. Extra linens and hygiene products were available. Hot water temperature in resident's bathrooms measured at 107.8 and 108.2 F which is within allowable range of 105 to 120 degrees F. Medications were centrally stored and locked at time of inspection. Toxins are located in a locked cabinet in the garage. At least two days of perishable and one week of nonperishable food was available. Fire extinguishers were last serviced January 2024. Smoke detectors and carbon monoxide detector throughout the facility were tested and operational. Exit doors have auditory alert system that were functional at time of visit. Last disaster drill was conducted on 11/30/23. Medication and medication records were reviewed. Required postings were observed. Annual fees current. LPA conducted file review at 12:30 PM. Four staff files and four resident files were reviewed. Staff have required First Aid and CPR certificates. Administrator Certificate for Administrator, Sarah Araya 6010373740 expires 4/10/2025. Training records were reviewed. Four out of four resident's files have current medical assessments and care plans. Facility handles cash resources for three out of four residents and maintains documentation. Cash on hand is covered by the Surety Bond. Administrator to submit updates of the following documents by 1/29/24: Designation of Administrative Responsibility (LIC308), Personnel Report (LIC500), copy of Liability Insurance & surety bond.
