Type of visit: Required - 1 Year
At approximately 10:35 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year visit. Cinthia Junco arrived at 11:30 AM. Administrator Brisa Junco arrived at 2:10 PM. Green House East RCFE is Licensed as a Residential Care Facility for the Elderly (RCFE). The facility is a two (2) story ranch house with a staff apartment above the garage. The facility has an approved fire clearance for six (6) non-ambulatory residents. The facility has a Hospice Waiver for three (3) residents. Upon arrival, LPA was informed that there were five (5) residents in care. LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 11:00 AM, LPA toured the facility . All exits were clear and unobstructed. The facility's two (2) fire extinguishers were last serviced and tagged on 6/18/2026. The facility was sufficiently lighted. LPA inspected three (3) resident bedrooms and found all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. LPA observed a an unlocked utility shed in the back yard. The shed contained paints and other unsecured toxins. This deficiency will be cited. All other toxins were observed to be stored inaccessible to residents. The facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility is conducting fire and emergency drills quarterly. The last disaster drill was conducted on 7/15/2026. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors were observed to be operational. Continued on 809-C... ...Continued from 809 At approximately 11:50 AM, LPA reviewed five (5) resident files. Five (5) of five (5) resident files were observed with all required documentation. LPA reviewed four (4) staff files. Three (3) of four (4) staff files (for S1, S2 & S3) were observed not to have valid First Aid Certification or valid CPR certification. Regulations state that all staff members providing direct care have valid First Aid Certification. Regulations state that at least one (1) staff member with valid CPR certification be working at the facility at all times. The one (1) staff member that does have a vaild First Aid & CPR certification was not at the facility during most of today's inspection. These deficiencies will be cited. LPA audited Medication for five (5) residents. One (1) of five (1) residents (for resident R2) had one (1) medication that was observed to not be listed on the Centrally Stored Medication and Destruction records. One (1) of five (5) residents (for resident R1) had a PRN (pro re nata - take as needed) medication that did not have a PRN administration log. These deficiencies will be cited. LPA observed all other medications to be centrally stored, secure and with proper documentation. The facility does not handle residents’ monies for personal and incidental items. The facility's corporate entity, "GREEN HOUSE EAST, LLC." was listed as "SUSPENDED" on the California Secretary of State's website. Title 22 regulations state If the licensee is a corporation or an association, the governing body shall be active, and functioning in order to assure accountability. This deficiency will be cited. Brisa Junco’s Administrator Certification 7011129740 is current with an expiration date of 7/13/2027. LPA requested the following documents be submitted to Community Care Licensing by 8/28/2026: LIC 500 Personnel Report - Showing staffs' actual schedules LIC 610E Emergency Disaster Plan Proof of Liability Insurance (in 9/2026) Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency, on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. Exit interview conducted. Copy of report, LIC-809Ds, Plan of Corrections, 811 Confidential Names and Appeal Rights discussed and provided to Administrator Junco. Type of visit: Required - 1 Year
At approximately 8:45 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year visit and was greeted by Caregiver (CG) Nelva Ramirez. Administrator Brisa Junco arrived at 12:15 PM. Green House East RCFE is Licensed as a Residential Care Facility for the Elderly (RCFE). The facility is a two (2) story ranch house with a staff apartment above the garage. The facility has an approved fire clearance for six (6) non-ambulatory residents. The facility has a Hospice Waiver for three (3) residents. Upon arrival, LPA was informed that there were five (5) residents in care and one (1) staff member on-site. One (1) additional staff member arrived at 8:50 AM. At approximately 9:05 AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 9:10 AM, LPA toured the facility with CG Cinthia Junco. All exits were clear and unobstructed. The facility's two (2) fire extinguishers were last serviced and tagged on 3/19/2025. Fire Department inspected the facility on 6/3/2025 and found no violations. The facility was sufficiently lighted. LPA inspected three (3) resident bedrooms and found all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility per regulation, should be conducting fire and emergency drills quarterly. LPA observed that there have only been two (2) fire and emergency drills in 2025 on 1/3/2025 and 5/24/2025. This deficiency will be cited. During the inspection LPA observed medications for residents in distribution cups unlocked in a kitchen drawer. This deficiency will be cited. Continued on 809-C... ...Continued from 809 The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors were tested and observed to be operational. At approximately 10:10 AM, LPA reviewed four (4) resident files. Two (2) of four (4) resident files were observed to not have documents as required by Title 22 regulations. Resident R1's file was observed to be missing the LIC 625 Appraisal/Needs and service plan. Resident R3's file was observed to missing the LIC 613C Personal Rights for RCFE. These deficiencies will be cited. LPA reviewed four (4) staff files. Four (4) of four (4) staff files were observed with all required documentation including First Aid and CPR certification and proper training documentation. LPA audited Medication for three (3) residents. LPA observed all medications to be centrally stored. LPA observed that a medication for resident R2 had an incorrect prescription number marked on the Central storage and destruction log. This deficiency will be cited. The facility does not handle resident’s monies for personal and incidental items. Brisa Junco’s Administrator Certification 7011129740 is current with an expiration date of 7/13/2027. LPA requested the following documents be submitted to Community Care Licensing by 10/16/2025: LIC 500 Personnel Report LIC 610E Emergency Disaster Plan Proof of Liability Insurance Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency, on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. Exit interview conducted. Aug 22, 2024Facility evaluation reportReport on file
Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Christi Coppo arrived unannounced to conduct a required Annual inspection and was greeted by caregiver. Administrator Brisa Junco arrived later. Facility contact information was reviewed. At approximately 9:30am LPA and Admin toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed at least a 2 day supply of perishable and 7 day supply of non-perishable food. Food items found to be expired were can of turkey gravy with best if used by date of July 2024, frozen cheese with best if used by date of July 2024 and peanut butter with best if used by date of December 2023 (deficiency cited, see 809D). Food items were found to be stored in a safe manner with open items covered. Kitchen cabinet containing cleaning supplies was not locked, bottle of drain cleaner found outside in backyard, cleaning supplies and laundry soaps in unlocked garage, not in a locked cabinet (deficiency cited, see 809D). All bedrooms were equipped with lighting, night stand, and chest of drawers. Bed in room #4 for R1 and bed in the master bedroom for R2 were found to be not clean, containing black debris and brown debris. Bed for R1 found to not have good springs and was not comfortable, very hard and in spots not supported by good springs. Extra hygiene products and linens were available. Resident bathroom had required bath mat and grab bar. Water temperature in sink accessible to residents in care measured at 98.7 degrees F which is not within the allowable range of 105 to 120 degrees F (deficiency cited, see 809D). Cabinet in garage found to have rodent dropping and chewed pieces of paper and board from the cabinet in which the dropping are located. Cabinet with rodent droppings contained items bagged in garbage bags, pillow, shoes and linens. LPA noticed berry bushes in the backyard. Admin advised those are blackberry bushes. LPA had conversation with Admin about rodents and that sometimes blackberries can attract rodents. Continued on 809C... Continued from 809... Backyard of facility was found to have debris, shovel head, a pair of garden sheers and overgrown dried weeds and plants, presenting a tripping hazard. Backyard fence on all sides in disrepair: exposed nails, loose and missing boards, boards covered in green film in some areas, and a back up generator that is blocking the exit path and partial portion of the gate. Across from the generator in the left hand corner of the front part of the fence is an area that has a constructed space that is now in disrepair: boards missing, stacked up and leaning against the area, boards have piece of metal and exposed nails and screws. Gates on both sides not easily functional: hard to open and push closed, joint bracket and screws loose on left hand side gate. Gate on right hand side of the facility cannot latch properly due to board with gate latch not secured and in disrepair. Left hand back corner portion of fence is missing entirely. (deficiency cited, see 809D). Fire extinguishers were last inspected 10/12/2023. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Facility’s last quarterly disaster drills were conducted on 6/21/2024. Facility has a backup generator for use during a power outage. At approximately 11:30am LPA conducted review of 5 staff records. All required documents present, no deficiencies. All First Aid current, all training complete and current. At approximately 12:30pm LPA conducted a review of 5 resident records. All required documentation present. 1/2 rails on order. At approximately 1:00pm LPA and Admin conducted a spot check of medication and medication records. Medication is centrally stored in a locked cabinet. No deficiencies. Brisa Junco Administrator Certificate 7011129740 expires 7/13/2025. All fees current at this time. Continued on 809C(2)... Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: LIC500- Personnel Report LIC308- Designation of Responsibility LIC309- Administrative Organization Liability Insurance Mortgage Deed Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation and the Health and Safety Code.
