Type of visit: Required - 1 Year

On 4/22/2026, LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Administrator Sharon Wong and explained the purpose of the visit. LPA toured the facility inside and outside including a random sample of resident rooms, common areas, activity rooms and kitchen area. When LPA arrived residents are being prepped for activity in the activity room. While touring the facility it was observed that the temperature was at 69 deg F. Hot water was also tested. All personal belongings are intact. Facility has sprinkler system. All fire extinguishers have been checked and current. Resident bedrooms and bathrooms were observed to be in good repair equipped with grab bars and non-skid floors and strips. There is adequate amount of food, 2 days for perishables and & 7 days non-perishable. Emergency drills are logged and done every quarter. Five resident records and five staff records were reviewed. Resident records are updated, complete and signed. Staff records are complete, with training logs that have met the basic 20hr requirement. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. LPA received the following documents: LIC500 & Liability Insurance. On 5/9/2025, LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Administrator Sharon Wong and explained the purpose of the visit. LPA toured the facility inside and outside including a random sample of resident rooms, common areas, activity room and kitchen area. When LPA arrived residents were having an activity in the activity area and some were doing walks within the facility perimeter. While touring the facility it was observed that the temperature was at 72 deg F. Hot water was also tested. All personal belongings are intact. Facility has sprinkler system. All fire extinguishers have been checked and current. Resident bedrooms and bathrooms were observed to be in good repair equipped with grab bars and non-skid floors and strips. Resident call pendants are functioning. There is adequate amount of food, 2 days for perishables and & 7 days non-perishable. Emergency drills are logged and done every quarter. Five resident records and five staff records were reviewed. Resident records are updated, complete and signed. Staff records are complete, with training logs that have met the basic 20hr requirement. Facility has a certified administrator on site with complete certification and training requirements. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. LPA received the following documents: Resident Roster, LIC500 & Liability Insurance. No deficiencies are cited at this time. Type of visit: Case Management - Other

On 2/10/2025, LPA Grace Donato made an unannounced case management to the facility. LPA met with Administrator Sharon Wong. LPA explained the purpose of the visit. On 1/30/2025, LPA received an email regarding an inquiry on the refund for a resident (R1). R1 had a conservator sign the admission agreement. R1 did not move in to the facility, however, on 1/11/2024, R1s belongings were moved to the facility including clothing, toiletries and medications. R1, after several attempts to convince to move in, did not go through with living in the facility. The conservator (C1) did send an email of intent to vacate the room on 2/6/2024. Residents items were fully moved out on 2/13/2024. On 1/9/2024, C1 was authorized by the court to temporarily place R1 at Joyful Chapter or an equivalent assisted living facility for R1s safety due to dangerous living conditions in the home, including the presence of black mold, feces, animal blood and hazardous conditions, and to shield temporary conservatee from any violence or altercations in the home. Per the admission agreement signed by C1 it is stated that "Prior to moving in, resident shall pay a last month's service fee, which will be applied to resident's last month of occupancy." The facility followed the refund conditions in the admission agreement. The facility has provided the breakdown of the fees and the amount of refund given back to C1. No deficiencies cited today. On 5/7/24, LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Administrator Sharon Wong. LPA explained the purpose of the visit. LPA toured the facility inside and outside including a random sample of resident rooms, common areas & kitchen area. The indoor and outdoor passageways were free of obstruction. LPA observed some residents were in the activity room doing some activities or watching TV, there were also residents resting in the bedrooms. Hot water was also tested in the bathrooms and the temperature was 108 deg F. The residents have adequate amount of linens and incontinence care items. All personal belongings are intact. Carbon monoxide monitors are working properly. All fire extinguishers have been checked and current. Resident bedrooms and bathrooms were observed to be in good repair. Bathrooms are equipped with grab bars and non-skid mats. LPA checked the food supply and there is adequate amount of food, 2 days for perishables and & 7 days non-perishable. Emergency drills are logged and done every quarter. LPA reviewed five resident records and five staff records. Resident records are updated, complete and signed. Staff records are complete, with training logs. Facility has a certified administrator on site with complete certification and training requirements. Facility accepts hospice residents and are in compliance with the required waiver requirements. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. LPA requested submitted the following: LIC 500, Liability Insurance & Administrator Certificate. No deficiencies are cited at this time. Type of visit: Case Management - Other

On 10/9/23, LPA Grace Donato conducted an unannounced visit to conduct a case management visit. LPA met with Administrator, Sharon Wong. LPA explained the purpose of the visit. It was reported to Licensing that facility has not been reporting COVID-19 infection. A resident (R1) who came from a Skilled Nursing Facility was transferred here on a Sunday. When R1 arrived at the facility there was no confirmation of any COVID-19 test, but was requested by the facility. R1 was already coughing when R1 arrived at the facility, so a test was done and turned out positive. R1 was sent to the hospital right away. R1 came back after 3 days still infected but isolated in the room. No incident report was submitted to Licensing. Deficiency is cited today as the facility did not report said incident to Licensing. Deficiency is cited under California Code of Regulations, Title, 22 cited on the LIC 809D. Failure to correct the deficiencies may result in civil penalties.
