Licensing Program Analysts (LPA) Zabel Chochian conducted an required annual visit to the facility today. Upon arrival, there were two (2) staff, six (6) residents and three (3) visitors present. The LPA met with staff who then contacted the Administrator via telephone. LPA spoke with Administrator, Jennifer Hamilton and reason for the visit was explained. LPA was assisted by the lead staff on duty. LPA met with resident and the visitors in the common area. Entrance introduction was made. Brief discussion was held with residents and visitors. At approximately 2pm The LPA and staff toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. The following was observed: KITCHEN: The LPA inspected the kitchen/food service area. Knives and sharps were observed in a locked drawer at the time of the visit. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. Cleaning supplies were observed locked and inaccessible under the kitchen sink. The LPA observed an adequate amount of emergency food and water. COMMON AREAS: At the time of the visit, furniture in the common areas was observed to be in good condition. The facility maintained a comfortable temperature. Fire extinguisher was observed to be new with a purchase date of 04/17/2026. The LPA observed a fireplace adequately covered at the time of the visit. Night-lights observed throughout the common areas and bathrooms. The LPA observed required postings throughout the common space. Auditory alarms observed functioning at the time of the visit. Facility smoke/carbon monoxide detectors and fire alarm are all wired and connected with the local fire station. The system was confirmed operable via the phone system. (Continued on LIC 809c) BEDROOMS: There are six (6) bedrooms for resident use; all bedrooms are designated for private / single occupancy. Resident rooms were observed to be furnished appropriately with clean linens, appropriate furnishings, and sufficient lighting. The LPA observed two (2) closets with additional clean linens and towels for resident use. Staff bedroom observed on premises which is kept locked at all times. RESTROOMS: There are two (2) restrooms for resident use. Restrooms were observed to be clean and sanitary and in operating condition with grab bars and non-skid surfaces. The restrooms were sufficiently stocked with supplies and paper towels. The hot water temperature was measured in both bathrooms (105.8 and 109.3 degrees Fahrenheit). LAUNDRY ROOM: Washer and dryer were observed in a locked storage area adjacent to the kitchen. Detergents and toxins were observed locked and inaccessible at the time of the visit. BACKYARD: The backyard has a shaded patio area with adequate furniture for resident use. All passageways were observed to be clear and free of obstructions. The LPA observed three (3) sheds used for storage which were locked at the time of the visit. There is one (1) self-latching gate for emergency use. No bodies of water noted at the time of the visit. No obstructions or hazards observed inside or out. RECORDS: LPA reviewed resident records and staff records starting at approximately 2:45 p.m. -3:30 p.m. Six (6) resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with negative TB results, preplacement appraisal, and appraisal / current needs and services plan. All files were complete. Three (3) personnel files including the Administrator’s file were reviewed for, but not limited to: personnel records, health assessments with negative TB results, criminal record clearances, first aid/CPR training, and the appropriate yearly training. All records were in order. The facility's infection control plan was also reviewed; The facility’s policies and procedures as it pertains to infection control are adequate. An adequate amount of Personal Protection Equipment (PPE) was observed. In addition, the facility's emergency disaster plan was reviewed, which was observed to be complete and reviewed/updated annually. Emergency disaster drills are conducted quarterly as per regulation; with the last drill conducted on 06/10/2026. MEDICATIONS: The LPA conducted a medications review at approximately 3:45 p.m. -4:30 p.m.. Medications are centrally stored in a locked cabinet by the kitchen. Medications observed documented properly and appear to be administered as prescribed at the time of the visit. Exit interview conducted. Licensing Program Analysts (LPA) Martha Arroyo conducted an unannounced visit to the facility to conduct the annual inspection today. Upon arrival, there were two (2) staff and six (6) residents present. The LPA met with staff who then contacted the Administrator via telephone. The Administrator, Jennifer Hamilton arrived at the facility at approximately 10:20am. Entrance interview. The LPA along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. The following was observed: KITCHEN: The LPA inspected the kitchen/food service area at approximately 10:22am. Knives and sharps were observed in a locked drawer at the time of the visit. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. The hot water temperature was measured in both bathrooms starting at 10:28am and they measured between 105.8 and 110.3 degrees Fahrenheit. Resident rooms were observed to be furnished appropriately with clean linens, appropriate furnishings, and sufficient lighting. All records were in order. Administrator’s certificate valid until 10/17/2026. MEDICATIONS: The LPA conducted a medications review at approximately 12:25pm. Medications are centrally stored in a locked cabinet by the kitchen. Medications are being documented properly and appear to be administered as prescribed at the time of the visit. However, medications are being dispensed into separate containers 48 hours ahead of time. Staff placed medications back in original containers at the time of the visit. An adequate amount of Personal Protection Equipment (PPE) was observed. The LPA also reviewed the facility's emergency disaster plan, which was observed to be complete and reviewed/updated annually. Emergency disaster drills are conducted quarterly as per regulation; with the last drill conducted on 06/20/2025. Licensing Program Analysts (LPAs) Trevor Byrne and Angela Barutyan arrived at the facility unannounced to conduct a required annual visit at 08:55 AM. LPAs were greeted by facility staff who contacted the facility administrator via telephone. Administrator Jennifer Hamilton arrived at the facility at approximately 09:10 AM. Entrance interview conducted. Beginning at 08:56 AM, the LPAs, toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that facility is in compliance with Title 22 Regulations. The following was observed: KITCHEN: The LPAs observed the kitchen/dining area to be clean. Kitchen appliances were in operable condition. The facility has a sufficient supply of seven (7) days perishable and two (2) days non-perishable food. Sufficient emergency water and food is stored in the kitchen pantry. At 08:59 AM LPAs observed two (2) bags of expired lettuce dated 06/24/2024. Staff disposed of expired items immediately. The LPAs observed one (1) designated kitchen drawer where knives and sharps are stored locked and inaccessible to residents. Cleaning supplies are located in a locked cabinet under the kitchen sink. Medication is stored in a locked cabinet in the kitchen inaccessible to residents. The kitchen contains a fireplace that was adequately screened. A fire extinguisher was observed in the kitchen and was purchased 11/29/2023. LAUNDRY: The laundry room is located adjacent to the kitchen and is inaccessible to residents. Laundry supplies and chemicals are stored in the locked room along with the washer and dryer. Report Continued on LIC 809-C Continued from LIC 809 COMMON AREAS: This includes the living room, family room, and dining room areas. LPAs observed common areas to be clean and properly furnished at the time of the visit. Night lights were observed in common hallways leading to bathrooms. A fireplace was observed in the living room to be adequately screened. Common areas were observed to contain security cameras however, they do not record audio. Hardwired smoke detectors and carbon monoxide detectors were observed and were installed and tested on 07/09/2024. BEDROOMS: There are seven (7) bedrooms in the facility; six (6) bedrooms are designated for private resident use, as well as one (1) staff room. The staff room is kept locked. All 6 (six) resident rooms were observed to be furnished appropriately with clean linens, appropriate furnishings, night lights, and sufficient lighting. BATHROOMS: There are two (2) bathrooms for resident use. Restrooms were observed to contain nonskid mats. Grab bars were observed in the shower and near the toilets in both bathrooms. The water temperature was measured in both resident bathrooms and measured between 111.6 and 113.7 degrees Fahrenheit, which is in compliance with regulation. OUTDOOR SPACE: The backyard has a covered patio area with patio furniture including tables and chairs for resident use. Facility has one (1) self-latching gate for emergency exit use, all passageways were observed to be clear. There were no bodies of water on the premises at the time of the visit. The backyard contains three (3) unlocked storage sheds which contain extra supplies. One (1) unlocked shed was observed to contain a hand saw accessible to residents. Administrator removed and secured the saw immediately. RECORD REVIEW: Began at 09:37 AM. Staff and resident records were reviewed for documents including, but not limited to: health screening, TB test, staff training records, fingerprint clearance, resident physician's report, needs and service appraisal, and personal rights. Six (6) out of six (6) resident files contained all required documents. And four (4) out of four (4) staff files contained all required documents. Report Continued on LIC 809-C Continued from LIC 809-C MEDICATION REVIEW: Began at 10:21 AM. Medications for three (3) of six (6) residents were reviewed. All medications observed had the start dates written on the prescription labels. All medications reviewed were properly stored and logged in the centrally stored medication and destruction record. INFECTION CONTROL/EMERGENCY DISASTER PLANNING: During today’s visit, the LPAs reviewed the facility's infection control practices and the facility's emergency disaster plan. The facility’s policies and procedures as it pertains to infection control are adequate. Emergency drills are conducted quarterly with the last drill conducted on 06/10/2024. INTERVIEWS: LPAs interviewed two (2) staff and one (1) resident. LPAs attempted to interview an additional resident but were unable to obtain a response. During today’s visit LPAs obtained a copy of the facilities liability insurance. The following deficiencies were observed (See LIC 809-D) and cited from the California Code of Regulations, Title 22 and/or California Health and Safety Code. Licensee was advised that failure to correct the deficiency may result in civil penalties. Exit interview conducted.
