On 2/27/2026, LPA Grace Donato conducted an unannounced Case Management-Other visit. LPA met with House Manager Veronica Rozario and explained the purpose of the visit. LPA delivered an immediate exclusion letter for a staff member who is currently associated in the facility. The letter was given to and reviewed by the House Manager. Type of visit: Required - 1 Year

On 11/14/2025, LPA Yi Sam Jian made an unannounced annual visit to the facility. LPA met with staff, Veronica Rozario. The licensee, Victoria Alejandro, was contacted by phone to obtain permission for staff to sign the report. LPA explained the purpose of the visit. LPA toured the facility inside and outside including all of resident rooms, garage/laundry area, and kitchen area. All outdoor and indoor passageway were free and clear of obstruction. No accessible bodies of water or fire safety hazards observed. Kitchen was inspected, sufficient supply of food observed. Medications, toxins and sharps stored appropriately and inaccessible to clients, a comfortable temperature was maintained, hot water temperature inspected to be compliant, furnishing and lighting was sufficient for comfort and safety. Carbon monoxide detector and smoke detector system inspected and met the requirements. fire extinguisher checked and fully charged. Facility has a written emergency disaster plan. Licensee has at least one completed first aid kit. Facility has an updated log for emergency drill are done every month. A technical violation was noted due to the facility’s ongoing process of securing signatures for the reappraisal. No deficiencies are cited at this time. Type of visit: Case Management - Health Checks

On 4/19/2025, LPA Grace Donato conducted an unannounced case management-health checks visit. LPA met with House Manager (HM), Veronica Rozario and explained the purpose of the visit. This visit is to conduct health and safety check after the facility had to evacuate due to foul odor emitting from the bathroom. LPA toured the facility and observed that the bathroom tiles have been replaced due to it being torn to fix the leaking pipes under. There is no more odor emitting from the entrance of the bathroom. LPA took photos of the fixed bathroom and requested the before images taken by HM. Residents will be checking out of the hotel today at 11 am and move back to the facility. LPA requested work orders and permits and other documentation. LPA also requested and updated Emergency Disaster Plan. No deficiencies cited today. Type of visit: Collateral

On 4/14/2025, LPA Grace Donato conducted an unannounced collateral visit at Embassy Suites located in Burlingame, CA. LPA met with House Manager (HM), Veronica Rozario and explained the purpose of the visit. This visit is in response to emergency relocation due to ongoing bathroom repair in the facility because of bathroom pipes emitting foul odor. During the visit at the hotel, LPA observed 4 hotel rooms housing 1 client in each room. Clients rooms were observed with clients' personal belongings, water, snacks, bathroom, towels, bed. Clients are currently having breakfast at the lobby and will go out for day program. No staff members will be sleeping in the clients' rooms as all staff are wake-staff. HM indicated they will provide breakfast to clients; cereal, fruits and eggs with milk, coffee or juice and for lunch and dinner they will have take-out. According to HM, the tentative check out date from the hotel is on 4/16/2025 and might be extended depending on the ongoing repairs. HM will notify LPA before clients will be moved back to the facility. No citations are issued during the visit. On 12/3/23, LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Administrator Nilda Alpon, House Manager Veronica Rozario, Licensee Victoria Alejandro. LPA explained the purpose of the visit. LPA toured the facility inside and outside including all of resident rooms, garage/laundry area, and kitchen area. LPA observed all residents to have left for community outing. While touring the facility it was observed that the room temperature was at 69 deg F. Hot water was also tested in the bathrooms and the temperature was 118 deg F. The facility is observed to be clean, odorless, and well maintained. Residents bedrooms were observed to be well organized and fully furnished with adequate lighting. Sharps and toxic materials were observed locked in a cabinet. Food supply was observed with an adequate two day perishable and seven day non-perishable. Carbon monoxide/smoke detectors, and fire extinguisher were present throughout the facility. Facility has an updated log for emergency drill are done every month. Four resident records and four staff records were reviewed. Resident’s PNI money was counted and all accounted for. Centrally stored medication was locked in the medicine cart and inaccessible by residents. All medication was labeled and sorted by resident name. All medication logs are complete and updated. LPA requested licensee to submit the following: LIC 500 Personnel Report, Liability Insurance. No deficiencies are cited at this time. On this day, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced collateral visit in order to deliver documents related to a substantiated complaint allegation; complaint number 14-AS-20230223154313 for Cypress Home #415601119, which is the sister facility of Arbor House. Cypress Home is currently closed with no residents in care. LPA met with administrator Nilda Alpon and explained the purpose of today's visit. LPA delivered the documents to Nilda Alpon. LPA explained the purpose of the visit. LPA toured the facility inside and outside including all of resident rooms, garage/laundry area, and kitchen area. LPA observed all residents to have left for Adult Day Program. While touring the facility it was observed that the room temperature was at 67 deg F. Hot water was also tested in the bathrooms and the temperature was 118 deg F. The facility is observed to be clean, odorless, and well maintained. Residents bedrooms were observed to be well organized and fully furnished with adequate lighting. Sharps and toxic materials were observed locked in a cabinet in the garage area and inaccessible to residents. Food supply was observed with an adequate two day perishable and seven day non-perishable. Carbon monoxide/ smoke detectors, and fire extinguisher were present throughout the facility. Facility has an updated log for emergency drill are done every month. Three resident records and three staff records were reviewed. Resident’s PNI money was counted and all accounted for with proper log. Staff have criminal record and fingerprint clearances, First Aid/CPR certifications on file. Resident records were reviewed and were observed to be complete with documents such as Admission Agreements, Medical Assessments, and Needs and Service Plans. Centrally stored medication was locked in the medicine cart and inaccessible by residents. All medication was labeled and sorted by resident name. All medication logs are complete and updated.
